Overview

Mania, typified by an elevated or irritable mood, increased activity, and other symptoms, is a leading characteristic of bipolar disorder. Accurately understanding the differences between acute and chronic mania is essential for correct diagnosis and effective treatment. Here, we take a close look at the two conditions to provide organised descriptions of their symptoms, biological bases, and treatment methodologies.

Acute Mania

Acute mania is characterised by intensity and rapid onset. Typically occurring in bipolar disorder, this condition can rapidly and dramatically change an individual's everyday life. The symptoms of acute mania appear suddenly; a person may move from a normal state to an intensely emotional state within moments.

In acute mania, a person may experience a highly elevated mood. This intensely lively state is not simply heightened happiness; it may also present as irritability or even angry outbursts. Alongside this shift in emotion, energy levels are often greatly increased. People may find themselves attempting many things at once, unable to sit still or relax. This surge of energy can occasionally lead to foolish or dangerous behaviour, such as impulsive spending sprees, reckless driving, or risky sexual practices.

Another key symptom of acute mania is accelerated thought. People experiencing this cognitive state often seem to have a continuous stream of ideas, words, and thoughts flowing from them, with difficulty maintaining focus on any one point or topic. Such cluttered thinking is often accompanied by rapid, pressured speech, in which a person talks faster than usual and leaps abruptly from one theme to another.

Another characteristic of acute mania is impulsivity, acting without thinking about the consequences. Acting on whims without regard for the consequences can lead to patterns of behaviour that are not only socially inappropriate but also potentially dangerous.

The results of such impulsive actions are generally negative, including financial problems, relationship discord, and conflict with the law.

The presence of increased self-esteem, or grandiosity, is also characteristic. In acute mania, a person may come to regard their abilities or personal qualities as exceptional. This sense of grandiosity can lead to unrealistic plans or vain pretensions, which may strain personal and professional relationships.1

Biological Mechanisms of Acute Mania

Changes in brain function and brain chemistry lie at the heart of acute mania. The neurotransmitters dopamine and norepinephrine are intimately involved in regulating mood and energy levels. In acute mania, these neurotransmitters are often imbalanced, and elevated levels can account for the heightened mood and activity. Moreover, brain circuits critical to decision-making and emotional regulation may show increased activity during manic episodes.

Treatment for Acute Mania

Treating an acute manic episode typically involves a multifaceted approach to stabilise the patient and effectively manage their symptoms. Medication is a cornerstone of treatment during an acute episode of this disorder. Antipsychotic medications, such as olanzapine or quetiapine, are commonly prescribed for symptoms like agitation, delusions, and hallucinations during an episode. Mood stabilisers, including lithium or valproic acid, may also be used to smooth out the highs and prevent further escalation of symptoms. In some cases, benzodiazepines may be used to calm acute agitation and help people sleep.

During an acute manic episode, it is often necessary to hospitalise the individual, particularly if they are a danger to themselves or others. This environment offers a safe place where intensive treatment can be given and carefully monitored. The primary goals in such situations are to ensure the person's safety, stabilise their mood, and begin long-term treatment planning.

In the acute phase, psychotherapy is not emphasised. However, it can offer valuable long-term support. Cognitive behavioural therapies such as CBT can help people identify underlying issues, manage triggers and stressors, and develop strategies to prevent future episodes.

Chronic Mania

Compared with acute mania, which tends to come and go, chronic mania refers to a persistent state of manic symptoms that can last for weeks, months, or even years. This form of the disorder manifests as prolonged periods of mood disturbance, which can place a tremendous strain on an individual's ability to function normally in daily life.

One symptom of chronic mania is long-lasting mood elevation. Unlike acute mania, where an individual may suddenly develop an elevated mood, chronic mania involves a continuous or background mood of elevation or irritability that persists across every aspect of life for months or years. It can take many forms, from work to personality, and has lasting implications for those affected.

Chronic mania is also marked by continuing impulsivity, although the nature of these acts may change over time. Unlike the sporadic and extreme acts seen in acute mania, impulses in chronic cases can become more embedded in daily life. This persistent impulsivity can lead to poor decisions and risk-taking behaviour, affecting an individual's personal and professional development over the long term.

As with acute mania, a persistent inability to recognise or accept limits is common in chronic mania. Over long periods, individuals may develop increasingly exaggerated views of themselves, leading to ongoing interpersonal conflict and continued difficulty managing daily responsibilities. Cognitive impairment is also typical of chronic mania; prolonged exposure to manic symptoms can result in difficulties with memory, attention, and executive functioning. This means that problem-solving and decision-making abilities can become further impaired, compounding an already difficult life.2

Biological Mechanisms in Chronic Mania

Both neurotransmitter imbalances and underlying changes to brain structure appear to be at the root of chronic mania. Elevated levels of neurotransmitters such as dopamine and norepinephrine may circulate persistently, and over time this can lead to structural changes in brain regions responsible for mood, including the prefrontal cortex, which governs executive function and emotional self-regulation. Studies have shown that this region's metabolic activity, as observed on MRI scans, can differ over prolonged periods compared with healthy individuals. The limbic system, which underlies emotional processing, may be similarly affected.3

Treatment for Chronic Mania

Management of chronic mania involves a comprehensive, long-term effort. The typical treatment strategy features a combination of medication, psychotherapy, and lifestyle changes.

Medication forms the mainstay of chronic mania management. Long-term maintenance regimens, often including lithium in combination with antiepileptics, help stabilise mental health and prevent recurrence of episodes severe enough to be life-threatening. Persistent manic symptoms may also be controlled using atypical antipsychotics, which can address coexisting symptoms such as delusions and hallucinations.4

Psychotherapy plays a major role in the treatment of chronic mania, although the approach often depends on the person's individual circumstances. Cognitive behavioural therapy (CBT) is particularly useful for helping people identify and modify disordered thought patterns. Therapy also helps develop coping strategies to manage stress and prevent the recurrence of severe episodes. In addition, psychoeducation can help individuals and those around them better understand the condition and recognise signs of an impending manic period.

Lifestyle management is also a key part of treating chronic mania. Maintaining a regular daily schedule supports long-term management by reducing symptoms and preventing the onset of manic episodes. This includes regular, good-quality sleep, a healthy balanced diet, and consistent physical exercise. Stress relief techniques, ranging from mindfulness to relaxation exercises, can further support mood stability.

Regular follow-up with a mental health team is essential. Continuous assessment allows for treatment adjustments and ensures the individual receives the support they need to manage their condition effectively. Family and social support also play an important role, though caregivers of those with chronic mania must also attend to their own well-being given the ongoing demands of care and understanding.5

Summary

Acute mania and chronic mania represent different manifestations of manic symptoms within the bipolar spectrum. Acute mania is characterised by a sudden, severe onset requiring immediate stabilisation and treatment. By contrast, chronic mania requires long-term management strategies to address the persistent presence of these symptoms. Although both conditions affect mood, behaviour, and thought processes, the treatment approaches for each vary considerably.

Understanding these distinctions is crucial for both patients and clinicians. Effective management of acute mania focuses on rapid intervention and stabilisation, whereas chronic mania calls for maintenance treatment, including lifestyle management and careful ongoing monitoring, to restore stability. By recognising and addressing the specific needs of people with both acute and chronic mania, healthcare providers can make better decisions about intervention and achieve improved outcomes.
