Introduction 

Crohn’s disease is a lifelong condition that causes inflammation of your digestive tract, causing symptoms like abdominal pain, diarrhoea, fatigue, and frequent weight changes. Managing any chronic illness usually means learning to cope with daily symptoms while also considering long-term health needs.  This could mean juggling medications, adjusting your diet, and carefully adapting your lifestyle. This may all sound intimidating, especially since you may still experience flare-ups and side effects even with these efforts. Individuals with Crohn’s disease may seek other ways to improve comfort and quality of life. 

For this reason, there is a rising interest in cannabinoid-based treatments for chronic conditions, such as Crohn’s disease. Cannabinoids are naturally occurring compounds derived from the cannabis sativa plant.1 The most studied cannabinoids are tetrahydrocannabinol (THC) and cannabidiol (CBD). They work by interacting with the body’s endocannabinoid receptors, in the endocannabinoid system (ECS), which plays a role in mood regulation, perception, appetite, and gastrointestinal activity [1]. e. Because of this, researchers have set out to explore the relationship between cannabinoids and gut health in Crohn’s disease.

Understanding crohn’s disease and gut health

Crohn’s disease is a type of inflammatory bowel disease (IBD), characterised by chronic inflammation of the digestive tract, the small intestine, and the colon.2. Ongoing inflammation damages the intestinal lining, making it harder for the body to digest food, absorb nutrients, and maintain a healthy gut.2 This is often followed by abdominal pain, diarrhoea, and fatigue, which can affect daily life. Hence, making solution-focused management of Crohn’s an urgent conversation.

A growing amount of Crohn’s research focuses on the gut microbiome, made up of trillions of bacteria that help in regulating and stimulating the body’s processes. This includes metabolism, digestion, immune function, and the nervous and endocrine systems.3 A healthy microbiome supports digestion, strengthens immune function, and protects the gut lining.3 In Crohn’s disease, this balance is disturbed. The result is a cycle where immune responses are triggered unnecessarily. Demanding the body’s energy for repairing damage rather than maintaining good health. Because of this, researchers are increasingly exploring treatments that influence both inflammation and the microbiome, such as cannabinoid formulations. 

What are cannabinoids?  

Cannabinoids are naturally occurring compounds derived from the cannabis sativa plant.1 THC and CBD are the most well-known cannabinoids, and are compounds coming from the cannabis sativa plant.1 THC is the psychoactive component responsible for the “high” usually associated with cannabis, while CBD is non-intoxicating and is increasingly studied and distributed for its potential therapeutic effects. 

Both compounds interact with the body’s endocannabinoid system (ECS), or a specific network of receptors found throughout the brain, nervous system, and gut.4 The ECS helps regulate cognitive and emotional processes in the nervous system, including behaviour, mood, perception of pain, and gastrointestinal motility.4 By binding to ECS receptors, cannabinoids directly interact with gut function and inflammation. 

You can get cannabinoids in different medical formulations, including oils, sprays, and capsules, which a medical specialist may prescribe. Over-the-counter products, such as CBD oils, are widely sold, but quality and strength may vary. Unregulated products may be mislabeled, contain variable doses, or carry contaminants.1 Access to cannabis depends on local laws. For example, in the UK, they are tightly regulated, and some are only available through specialist prescription, whereas in the US, they are more broadly available. 

How cannabinoids may affect gut health

Research suggests that cannabinoids may affect gut health through several mechanisms:

  1. Reducing inflammation:
    1.  Animal studies suggest that cannabinoids weaken inflammatory signals in the gut, potentially helping to relieve the accompanying inflammation and the discomfort of flare-ups5 
    2. However, evidence from human studies is uneven. Many patients show symptom relief, but reductions in measurable intestinal inflammation are less consistent
  2. Regulating gut motility: Crohn’s can cause unpredictable bowel movements, ranging from diarrhoea to constipation.
    1. The ECS  plays a key role in regulating gut movement, and cannabinoids can help normalise bowel function, reducing urgency or frequency6 
  3. Stimulating appetite and weight maintenance: Many individuals with Crohn’s disease experience loss of appetite. Cannabinoids, specifically THC, are known to stimulate appetite, which may help patients maintain a healthy weight
  4. Pain and symptom relief: Cannabinoids influence how pain signals are processed in the nervous system
    1. This may help reduce the burden of abdominal cramping and pain. Some patients report better sleep, which improves overall quality of life by reducing pain1

Even with these benefits, cannabinoids are not proven to heal the gut lining in the same way as the standard treatment for Crohn's disease does. Conventional therapies include steroids or exclusive enteral nutrition. Instead, view cannabinoids as a supportive way to ease symptoms of Crohn's disease and make life better, rather than replacing standard care. 

Risks and considerations

Scientists are doing their best to study the benefits of cannabinoid formulations to manage the symptoms of Crohn’s disease. However, it is important to note that there are risks and limitations of using cannabis based treatments. The area of research is still new, so there is limited clinical evidence. It is unclear whether cannabinoids improve Crohn’s disease in the long term. Most studies focus on smaller populations for short periods,  generally showing no significant improvement in inflammation or a reduction in the disease (compared with placebo). [8] Suggesting that cannabinoids can help relieve your discomfort, but they do not tackle the cause of the condition.

 Health organisations such as the European Crohn’s and Colitis Organisation (ECCO) and the Crohn’s & Colitis Foundation suggest that cannabinoid-based treatment should be used alongside regular care.9 Still, the use of cannabinoid treatments needs more large-scale, representative, and long-term clinical trials to properly understand how it should be used.  

There are also safety and dependency concerns to consider. For example, you may experience short-term side effects of cannabinoids, including drowsiness, dizziness, changes in mood, or nausea. Especially when the main compound is THC, there is a risk of the psychoactive effects, which impair learning and memory, as well as the ability to concentrate. 

Understanding the long-term effects is critical, especially in children and adolescents. Developing brains may be more vulnerable to the effects of THC, and most guidelines do not recommend cannabinoid use in pediatric Crohn’s (child cases), unless a specialist claims it is necessary. Studies indicate that long-term use of cannabis, particularly when starting during adolescence, can impair memory, attention, and learning.7 There is also a risk of developing cannabinoid use disorder (a cannabis addiction), with symptoms such as tolerance, craving, and withdrawal.1 Overuse of THC-containing products has been associated with an increased risk of anxiety, depression, and in severe cases, psychosis.8 

Overall, it is best to use cannabinoids under medical supervision and as part of a monitored clinical plan. If you or your family are considering using cannabinoid formulations for treatment, you must proceed with caution, with the potential risks and effects in mind.

You might have read this article out of curiosity or caution about using cannabinoid formulation in managing Crohn's disease.  It is natural to seek supportive options that improve your or your loved one's comfort and daily life. Cannabinoids may help with symptoms, but they do not address Crohn’s disease at the source – inflammation. Living with Crohn’s disease is not something to fear, but rather something to understand with awareness of options for support. While the journey can be challenging, it does not define your, your family or your child’s future.  Progress in medicine, nutrition, and new therapies, such as cannabinoids, creates more possibilities for care. With the right mix of treatment, guidance, and compassion, individuals with Crohn’s can lead healthy and more comfortable lives.
