Introduction

Importance of uninterrupted sleep

We have all heard about the 8-hour rule – the idea that a healthy includes 8 hours of sleep. Yet in today's fast-paced lifestyle most people neglect their healthy sleep cycle. 

Most people have the habit of urinating at night which disturbs their sleep cycle and indirectly affects their mental and physical health. Interrupted or fragmented sleep can contribute to insomnia, depression, headache, daytime sleep and many other problems. The main noticeable feature of interrupted or fragmented sleep includes waking up in between their sleep cycle multiple times which also wakes up their bladder.

Common problem - Nocturia

If you wake up more than once during the night and directly rush to the washroom to urinate, you are likely experiencing a condition called nocturia. 

Nocturia is most common in older adults, typically those aged 60 years and above, and affects bothgenders. 

Understanding Nocturia 

Definition and frequency

Nocturia is a condition that causes an individual to wake up during the night to urinate, also known as nocturnal urinary frequency. While waking once per night is generally not alarming, but frequent nighttime urination may be a sign of an underlying condition or problem. 

It is important to distingiush nocturia from frequent daytime urination.

  • Frequent urination is the condition in which a person urinates more frequently during day time but limits their trip to the washroom at night 
  • Nocturia strictly refers to using the washroom more often after sleep and before waking up 

Symptoms of nocturia

  • If you wake up more than two times during your sleep, it is a sign that you may be affected by the condition
  • Increased urine volume, even if you are not waking up frequently 
  • Fatigue and sleeplessness during the day 
  • Morning headaches caused by inadequate sleep during the night 

Causes of nocturia

Nocturia is a multifactorial condition influenced by age-related changes, medical conditions, and lifestyle habits. 

Age related changes

Nocturia is most commonly present in older adults of 60 years and above. This is because aged kidneys are less able to concentrate urine which results in increased urinary frequency and decreased ability to store urine.

Medical condition

Nocturia is also seen in individuals with diabetes, kidney-related diseases, and those taking medications that contain diuretics. Diuretics cause the body to eliminate more salt which in turn results in increased urine volume and more frequent urination.

Lifestyle changes 

Maintaining a healthy lifestyle is crucial in today’s world as a healthy lifestyle is the key to sustaining a long and healthy life. Taking more fluids before going to bed, such as theconsumption of alcohol and water may lead to more frequent urination during night. Waking up more frequently at night due to uncertain reasons may also result in an individual going to the washroom to urinate even when they don’t need to.

Understanding these causes can help in effectively managing and reducing nighttime urination.

Practical changes to manage

Lifestyle adjustment

Timing of fluid intake

An individual with nocturia should limit their fluid intake, especially 2-4 hours before going to bed, as it will reduce excessive urine production at night.

Avoid use of diuretics (coffee, alcohol) before going to bed

Diuretics including coffee and alcohol cause the body to increase urine output by promoting the loss of salt and water, which in turn results in frequent urination.

Maintaining healthy weight

Overweight or obese individuals have higher chances of having nocturia. The reason behind it is that excess weight puts more pressure in the bladder area, leading to increased urinary frequencybetween sleep.

Behavioral Strategies 

Scheduled voiding before bedtime

Practice scheduled voiding – emptying your bladder every 1-2 hours before bed can help reduce the chance of waking during night to urinate.

Elevating legs

Elevating legs helps in redistribution of the fluids that may have accumulated in your lower limbs throughout the day back in the bloodstream, reducing the need for nighttime urination.

Sleep environment optimisation 

Night lights for safety bathroom trips

While going to the bathroom to urinate during the night, one should always use night lights for safety to prevent unwanted injuries. 

Comfortable bedding and temperature control

Sleeping on the left side reduces the pressure on the pelvic floor muscle, thus making it an ideal position for person affected by nocturia. It also aids in emptying the bladder more effectively. Maintaining a temperature that is not too cold during sleep also reduces the chances of nighttime urination more often.

Consultation with healthcare providers 

One should consult a urologist if suspected of having nocturia. As nocturia is related to frequent urination at night, a urologist will help in maintaining the frequency of fluid intake and exit from the body. Drinking plenty of water during day time and limiting the intake before 2-3 hours from sleep helps in tackling the situation.

Medication 

Anticholinergics are seen to be a success in nocturia. The main anticholinergics includes mirabegron, darifenacin, oxybutynin and tolterodine helps in regulating the amount of urine produced.
