- History taking: Diagnosis of macroglossia must begin with a proper history taking to note any underlying congenital or acquired conditions.
- Physical examination: The doctor examines the patient's tongue, oral cavity, head and neck. Macroglossia typically presents with an enlarged tongue protruding beyond the teeth. Other symptoms include eating, feeding and speaking difficulties, and loss of weight. Airway obstruction and signs of pulmonary hypertension are also seen in some cases that can be life-threatening
- Evaluation: The tongue should be carefully evaluated to identify any undiagnosed conditions. Changes in colour, consistency, and presence of masses are also noted
- Imaging studies: These studies help to show the extent and growth of tumours or masses in any underlying diseases
- CT scan: Computed tomography scans make use of X-rays and computers in order to create three-dimensional images of the head and neck region. CT scan is helpful in cases of obstructive sleep apnea.
- MRI scan: Magnetic Resonance Imaging produces clear images of the body's organs and structures using magnetic and radio waves
- Cephalometric and panoramic radiography: These radiography methods can detect any associated skeletal and dental deformities.
- Barium swallow study is used to rule out any upper airway obstructions.
- Ultrasonography detects the size of the lesion in the tongue
- Biopsy = Biopsy of tissues helps to identify if any underlying pathologic condition is causing macroglossia
How to prevent macroglossia?
Genetic conditions that cause macroglossia cannot be prevented. However, some cases of macroglossia associated with infections can be prevented to an extent by taking protection against infectious diseases.7
Treatment of macroglossia
Treatment of macroglossia includes medications, surgery, orthodontic treatment, and radiation therapy. In most cases, treatment begins with diagnosis and management of the underlying conditions, following the treatment of macroglossia.
- Medications like corticosteroids are used to reduce swelling of the tongue
- Sclerosing agents, cryotherapy, and electrocautery are also used to control inflammation and bleeding
- Orthodontic treatment is used to treat the malalignment of teeth associated with macroglossia
- Surgery is indicated in severe cases when there is an underlying neoplasia, or in cases of impairment in speech and difficulty in swallowing. The surgical procedures involved are partial glossectomy and reduction glossectomy. The glossectomy procedure involves the surgical reduction of an enlarged tongue to a size resembling that of a normal tongue and functioning more efficiently.
