Introduction

Leishmaniasis is a severely neglected tropical infectious disease caused by protozoan parasites, ranking as the ninth largest contributor to the global burden of infectious diseases and posing a significant risk to public health worldwide.1 The parasites responsible for causing the disease belong to the genus Leishmania, which is part of the family Trypanosomatidae. They are transmitted through the bites of infected female sandflies, primarily those from the genera Phlebotomus in the Old World and Lutzomyia in the New World.  Currently, at least 93 sandfly species are recognised as verified or probable vectors worldwide.2,3  The illness has been increasingly disseminated, spreading to various geographical areas, infecting numerous hosts, and engaging multiple vector species.4 Currently, over 23 species of Leishmania are recognised as causative agents of human infections.5

The role of community awareness in prevention

Raising awareness within communities plays a vital role in preventing and controlling leishmaniasis. Since the disease predominantly affects civilians living in remote or rural areas, empowering these people living in impoverished conditions with knowledge about leishmaniasis is one of the most effective tools for reducing its impact. 

An informed community can recognise symptoms early, adopt preventive behaviours, and support disease control efforts, ultimately leading to healthier and more resilient populations.

One of the most significant benefits of community awareness is the early recognition of symptoms and prompt health-seeking behaviour. Initially presenting with non-specific or mild symptoms, Leishmaniasis can be easily overlooked. 

When looking at the subtypes of Leishmaniasis, its cutaneous form can present with small skin lesions that may be mistaken for minor injuries, leading to delays in treatment. In the case of visceral leishmaniasis, symptoms such as prolonged fever, weight loss, and anaemia may be misattributed to more common infections. When people are unaware of what to look for or where to go for help, they may turn to traditional healers or even ignore symptoms altogether until the disease becomes severe. Education campaigns help individuals identify signs early and encourage them to seek treatment from trained healthcare providers, improving outcomes and preventing disease progression.

Understanding how Leishmaniasis spreads is essential for community awareness. Many people in affected areas do not know that the disease is transmitted through the bite of infected sandflies. This lack of knowledge can lead to myths, fear, and even fatalism, which decreases the community’s willingness to adopt preventive measures. Awareness campaigns can explain the breeding habits and peak activity times of sandflies, enabling people to take protective steps such as using bed nets, wearing long clothes, applying insect repellent, and avoiding outdoor activities at dusk and dawn. Knowing about environmental factors—like poor waste disposal or cracks in mud walls—can also encourage community involvement in vector control efforts. With proper understanding, individuals are more likely to protect themselves and support wider prevention strategies.

Another significant benefit of community awareness is the reduction of stigma associated with the disease. In many societies, visible lesions caused by cutaneous Leishmaniasis or the association of the disease with poverty can result in social ostracism, particularly for women and children. Fear of discrimination often leads individuals to hide their symptoms or avoid seeking care. Public health campaigns can address these challenges by sharing stories of successfully treated patients, emphasising that the disease is curable, and correcting false beliefs about transmission. Reducing stigma promotes early diagnosis and treatment while fostering a culture of compassion and inclusion within the community.

Community awareness also mobilises people to participate in prevention and control initiatives. When communities understand the rationale behind vector control measures—such as indoor residual spraying, environmental cleanup drives, or the distribution of insecticide-treated nets—they are more likely to cooperate with health workers and even initiate their own preventive actions. Awareness builds trust between public health agencies and the population, which is essential for the success of any disease control program. Community members can help identify high-risk individuals, report suspected cases, and support monitoring and surveillance activities, transforming them from passive recipients of aid into active contributors to public health.

Lastly, increasing community awareness fosters long-term behavioural change. When people consistently receive accurate and relevant information, they are more likely to adopt and maintain preventive habits over time. These include maintaining clean living environments, reinforcing housing structures to prevent sandfly entry, and seeking regular health checkups. Over time, these behaviours become embedded in community norms, strengthening resilience not only against leishmaniasis but against other vector-borne diseases as well. An informed and engaged community becomes better equipped to demand health services, advocate for policy support, and maintain vigilance against future outbreaks.6

Strategies for community engagement in leishmaniasis prevention

Effective community engagement is essential for the sustainable prevention and control of leishmaniasis. Because the disease primarily affects vulnerable and underserved populations, empowering communities with knowledge, tools, and resources can drastically improve early detection and reduce transmission. Below are elaborated strategies that have proven effective in mobilising communities for leishmaniasis prevention:

Health education and awareness programs

Health education is the cornerstone of disease prevention and control. In regions where leishmaniasis is endemic, misconceptions about its cause, transmission, and treatment are common. Structured health education programs can correct misinformation and promote healthy behaviours.

Community workshops

Organising regular workshops led by trained health workers or NGO volunteers can help demystify the disease. Sessions should cover sandfly behaviour, signs and symptoms of leishmaniasis, prevention methods, and the importance of early treatment.

Visual aids and materials

Posters, illustrated booklets, and videos designed with low-literacy populations in mind can be displayed in public spaces such as schools, clinics, and community centres. Culturally appropriate messaging in local languages greatly improves impact.

Role play and drama

Community theatre or storytelling sessions can engage audiences more deeply than lectures, particularly in rural or tribal areas. These performances can dramatise real-life scenarios of infection, diagnosis, and recovery, promoting both empathy and awareness.

Empowering local health workers and volunteers

Frontline health workers play a critical role in leishmaniasis surveillance and case management. Involving them in community engagement ensures that awareness translates into timely action.

Training and capacity building

Training programs should equip local healthcare providers, Accredited Social Health Activists (ASHAs), and volunteers with knowledge about disease transmission, diagnostics, and reporting protocols. They should also be trained in interpersonal communication skills to better educate and reassure community members.

Door-to-door campaigns

Trained health workers can conduct home visits to identify early symptoms, refer suspected cases, and share preventive information, especially targeting high-risk populations like children and agricultural workers.

Referral networks

Establishing a clear and accessible referral pathway between the community and health facilities ensures that cases are not lost to follow-up.

Partnerships with community leaders and institutions

Trust and cultural acceptability are crucial for the success of public health interventions. Collaborating with influential figures and institutions ensures that messaging is respected and adhered to.

Engaging religious and tribal leaders

Community leaders often hold significant sway over public opinion. Involving them in awareness campaigns not only increases credibility but also encourages collective participation in preventive actions, such as environmental clean-up drives or acceptance of vector control activities.

Involving schools and teachers

Schools are effective platforms for educating the next generation about leishmaniasis. School-based programs can include special awareness days, science projects, or the integration of health topics into the curriculum.

Women's self-help groups and youth clubs

These groups can serve as change agents within the community, disseminating information and advocating for enhanced health services.

Leveraging mobile technology and mass media

In areas with access to digital infrastructure, mobile phones and mass media platforms can broaden the reach of health education campaigns.

SMS alerts and voice messages

Periodic messages containing reminders, facts about prevention, or motivational content can reinforce learning and influence behaviour, especially in areas where health workers have limited access.

Radio and community broadcasting

Local radio programs in vernacular languages are highly effective in reaching rural populations. These can include interviews with healthcare providers, success stories of patients who have been cured, or live Q&A sessions.

Social media engagement

In urban and semi-urban areas, platforms such as Facebook, WhatsApp, and YouTube can be utilised to share educational content and provide real-time updates on outbreaks or control measures.

Community-led environmental management

Since sandflies breed in dark, humid environments such as animal shelters and garbage heaps, community involvement in environmental sanitation is crucial.

Clean-up campaigns

Community-led drives to remove organic waste, fill cracks in walls, and improve drainage can significantly reduce breeding grounds for sandflies.

Housing improvements

Encouraging the use of plastered walls, window screens, and insecticide-treated bed nets is are simple but effective intervention.

Sustainable practices

Engaging communities in sustainable, long-term environmental management fosters ownership and ensures continued impact even after external support ends.

Conclusion

Community awareness is not a supplementary aspect of leishmaniasis control—it is foundational. By fostering informed, vigilant, and empowered communities, we can reduce the incidence of leishmaniasis, improve treatment outcomes, and move closer to long-term disease elimination. Investing in public health education and preventive infrastructure, particularly in high-burden areas, remains a critical priority in the global fight against leishmaniasis.
