Introduction 

Pleurisy, the inflammation of the lung's lining, can cause sharp chest pain. A doctor can employ several methods to diagnose pleurisy and determine the underlying cause.1


The history of pleurisy diagnosis goes back centuries. Hippocrates, the "Father of Medicine," first described its symptoms in the 5th century BC, including chest pain, fever, chills, and difficulty breathing. He even categorised pleurisy based on perceived causes like "bilious" or "sanguineous."2

Galen later expanded on Hippocrates' ideas, linking pleuritis to inflammation of the lung lining and noting a rapid pulse as a sign. Fast forward to the 18th century, physicians like Baglivi continued to refine the definition, emphasising chest pain, fever, and a fast pulse as key indicators.2

Morgagni, in the 18th century, distinguished pleurisy (inflammation of the lining) from lung tissue infection ("peripneumony"). He eventually combined these terms into "pleuropneumonia" after realising they weren't separate conditions.2

The invention of the stethoscope by Laennec in the early 19th century added another diagnostic tool – "egophony" (a sound change during speaking). He also streamlined the terminology, using "pleuritic" for inflammation and "pleurisy" for the overall disease.2

History and physical exam

Pleurisy often presents with sharp, localised chest or shoulder pain. This pain worsens with breathing, coughing, sneezing, or any movement of the chest wall. The pain itself can vary, feeling sharp, dull, burning, or like a sudden "catch." The duration and recurrence of these symptoms can offer clues to the underlying cause.2

Similar to diagnosing other causes of chest pain, pinpointing the cause of pleurisy relies heavily on a detailed medical history and physical exam.

Here's a breakdown of what doctors look for:

  • Timing and progression: How quickly the pain started (sudden or gradual) and how it has changed over time can help narrow down potential causes
  • Medical history: Underlying conditions like lupus, HIV, or tuberculosis can contribute to pleurisy
  • Lifestyle habits: Travel history, smoking/vaping, alcohol use, or intravenous drug use might provide clues2

Physical exam

  • Percussion and breath sounds: Dullness to tapping and weak breath sounds on one side of the chest can indicate fluid buildup (effusion) compared to a more resonant sound associated with collapsed lung (pneumothorax)
  • Pleural rub: A scratching sound heard during inhalation with a stethoscope may be present. This differs from a pericardial rub, which is heard during both inhalation and exhalation, even when breathing stops

  1. Listening in: A physical exam with a stethoscope allows the doctor to hear distinctive sounds produced by the inflamed pleura rubbing together. Abnormal breath sounds, like crackling or rattling, can also offer clues
  2. Bloodwork: Blood tests help identify the underlying cause, such as a viral or bacterial infection
  3. X-ray vision: Chest X-rays provide a visual picture of the lungs and surrounding areas, potentially revealing fluid buildup or other abnormalities. In some cases, more detailed imaging techniques like CT scans or ultrasounds may be used
  4. Fluid analysis: In a procedure called thoracentesis, a small sample of pleural fluid is extracted and examined to pinpoint the cause of inflammation
  5. Airway exploration (optional): In certain situations, a bronchoscopy might be employed. This involves inserting a thin camera-equipped tube into the airways to directly examine them for potential causes

By employing these methods, doctors can effectively diagnose pleurisy and determine the most appropriate course of treatment.2

X-ray insights into pleurisy and beyond

Diagnosing pleurisy involves a comprehensive strategy that combines a detailed history, physical examination, and various imaging and laboratory tests. While a physical exam can offer valuable clues, X-rays, and other tools provide a deeper look to pinpoint the cause and guide treatment decisions.2

A chest X-ray serves as a valuable initial imaging tool for pleurisy. It can reveal several key findings:

  • Lung consolidation: Areas of inflamed or fluid-filled lung tissue appear whiter and denser on an X-ray, indicating potential pneumonia or other infections
  • Pleural effusion: When fluid accumulates between the lung and chest wall, the X-ray might show a blurring of the normally sharp outline of the diaphragm (the muscle separating the chest from the abdomen). In some cases, a complete layer of fluid might obscure parts of the lung
  • Pneumothorax: A collapsed lung appears as a collection of air within the pleural space on an X-ray, causing a dark, air-filled area next to the chest wall2

While chest X-rays are a readily available and relatively inexpensive tool, they have limitations. Smaller abnormalities such as:

  • Tiny nodules: These can be early signs of lung diseases like cancer and might be missed on an X-ray
  • Loculated effusions: Pockets of trapped fluid within the pleural space can be obscured by overlying structures on an X-ray
  • Minimal effusions: Small amounts of fluid might not be readily detectable on an X-ray

Beyond conventional x-rays: unveiling the full picture

In some cases, additional imaging tests might be necessary for a more definitive diagnosis. These include:

  • Computed tomography (CT) scan: A CT scan provides a detailed, cross-sectional view of the chest. It offers superior resolution compared to an X-ray and can detect smaller nodules, pinpoint the location and extent of pleural effusions, and visualize blood clots in the lungs (pulmonary embolism). In certain situations, intravenous contrast dye might be injected during the CT scan to highlight specific structures like blood vessels2

Laboratory testing: unmasking the underlying cause

Blood tests play a crucial role in identifying the underlying cause of pleurisy. Here's a breakdown of some key tests:

  • Complete blood count (CBC): This test evaluates various blood cell types, including white blood cells, which increase in response to infection. An elevated white blood cell count suggests a potential infectious cause of pleurisy
  • Metabolic panel: This assesses various electrolytes, kidney function, and liver function. It can help identify underlying conditions that might contribute to pleurisy, such as autoimmune diseases or kidney problems
  • Serum protein, albumin, and lactate dehydrogenase (LDH): These tests can provide additional clues about inflammation and potential causes. For example, abnormal protein levels might suggest an autoimmune condition, while elevated LDH could indicate tissue damage

By employing a combination of these diagnostic tools - X-rays, CT scans, laboratory tests, and fluid analysis - doctors can effectively diagnose the underlying cause of pleurisy and create a targeted treatment plan for optimal patient outcomes.2

Thoracentesis in pleurisy diagnosis

Thoracentesis, also known as pleural tap, needle thoracostomy, or needle decompression, is a minimally invasive procedure used to address fluid buildup or air accumulation in the space surrounding the lungs (thoracic cavity).3 It serves a dual purpose:

  1. Diagnosis: By removing a sample of the fluid or air, doctors can analyse it to pinpoint the underlying cause of pleurisy3
  2. Treatment: Thoracentesis can also be therapeutic, helping to relieve breathing difficulties caused by excess fluid in the chest cavity3

The procedure involves inserting a thin, hollow needle (cannula) into the chest wall after applying local anaesthesia to numb the area. This allows for minimal discomfort during the process..3

Preventive measures of pleurisy

Pleurisy, often triggered by bacterial pneumonia, can be tackled with some proactive steps.4 Here's how to potentially reduce your risk:

Get vaccinated: One of the main culprits behind bacterial pneumonia can be prevented with a vaccine.4 Consider getting vaccinated if you fall into a high-risk group, including:

  • Young children
  • Adults over 65
  • Individuals with chronic health conditions like diabetes
  • People with weakened immune systems

Early intervention is key: Addressing any respiratory infection promptly can significantly decrease your chances of developing pleurisy.4 Don't hesitate to seek medical attention if you experience persistent coughing, fever, or difficulty breathing.
