Introduction

Male fertility plays a very important role in a couple’s ability to conceive. Research shows that male factors contribute to around 50% of all infertility cases. Men are solely responsible in around 20% of situations and partially responsible in another 30–40%. Despite this, many men are actually unaware of subtle issues that can affect their reproductive health.1

Semen quality is more than just sperm count; it can reflect infection, inflammation, and overall reproductive function. Pyospermia refers to when there is an unusually high number of white blood cells (or leukocytes) in the semen. White blood cells are a normal part of the immune system and usually help the body fight off infections. However, when they release reactive substances, they can damage sperm.

A semen analysis is the best method for detecting pyospermia and other hidden issues. This simple laboratory test examines the sperm count, motility, shape, and other important factors, and offers essential insights into male fertility. This article explores how semen analysis helps detect pyospermia, what it means for reproductive health, and why an early diagnosis can make a real difference in improving fertility outcomes.2

What is pyospermia? 

Pyospermia is also known as leukocytospermia and refers to having too many white blood cells in semen. These cells are part of the body’s immune system, and defend against infections and inflammation and are usually only present in small amounts in the semen.

However, when the number of white blood cells rises significantly, they can cause more harm than good. While fighting infection, these cells release reactive oxygen species (ROS), which are chemicals that destroy bacteria and viruses and, unfortunately, can also damage healthy sperm by breaking down their outer membrane, slowing their movement, and damaging their DNA, making it harder for sperm to fertilise an egg.

Pyospermia isn’t necessarily a disease but rather signals that something else might be going on, like an infection or inflammation in the prostate, seminal vesicles, or other parts of the male reproductive system. Some men may experience symptoms like fever, pain when urinating, or yellowish semen, but many have no symptoms at all. Because pyospermia often goes unnoticed, it is usually found during a semen analysis done for fertility or other issues.2

Why pyospermia matters

Pyospermia is important because it can quietly affect a man’s fertility and overall reproductive health. Too many white blood cells in the semen leads to inflammation or infection and ultimately disrupting sperm function. Studies have shown that men with pyospermia often have sperm that swim more slowly, are less likely to reach the egg, and may have damaged DNA, all of which lower the chances of conception.

The reactive oxygen species released by overactive white blood cells harm healthy sperm, changing their structure and energy levels and can, over time, degrade sperm quality, making natural conception much more challenging. In some cases, pyospermia may indicate an underlying issue such as prostatitis, epididymitis or even a sexually transmitted infection.

What complicates pyospermia is that it often presents with no symptoms. Many men feel completely healthy and discover the condition only after having a semen analysis. However, the good news is that having pyospermia doesn’t make pregnancy impossible, but it may lower the odds until the underlying issue is treated. Doctors may recommend fertility treatments like intrauterine insemination (IUI) or in vitro fertilisation (IVF) to improve the chances of conception.2 3

How is pyospermia diagnosed, and the role of semen analysis

The first step in identifying pyospermia is typically a semen analysis. A semen analysis is a straightforward test offering a detailed look at the patient’s reproductive health. It helps doctors evaluate not just the sperm but also the quality of the semen, revealing signs of infection or inflammation.

A typical procedure for a semen analysis requires the collection of a semen sample, usually after a few days of abstaining from ejaculation to ensure reliable results and examination in the laboratory where specialists view the sample under a microscope and assess several key features, including the volume of semen, the number of sperm, and their concentration within each millilitre. The movement (motility) of the sperm shows how effectively they can swim towards an egg, while their shape (morphology) indicates how many have a normal structure. The lab will check all of these things and may also check the acidity (pH) and how long it takes the sample to liquefy, since abnormalities can signal underlying problems.4

When looking for pyospermia, scientists search for an excess of white blood cells among the sperm. These cells can be spotted as extra “round cells” under the microscope. A special dye test using a myeloperoxidase stain is used to confirm that the extra “round cells” are actually white blood cells and if the concentration of white blood cells exceeds one million per millilitre of semen, then the patient’s sample can be diagnosed with pyospermia.5

However, sometimes, the semen analysis alone might not actually explain why the white blood cell levels are elevated. In these instances, the doctor may order further tests like a urinalysis to look for infection, or imaging tests like an ultrasound or MRI scans to examine the prostate and surrounding structures.

Some men may feel uneasy about semen testing, but rest assured, it is a private, simple, and valuable procedure where results can uncover hidden inflammation or infection well before symptoms arise, allowing for prompt treatment. Early detection from a semen analysis not only protects male fertility but also supports the overall reproductive well-being of the patient.2 

Common causes and contributing factors 

Pyospermia can arise from a variety of factors:

  • Infections: Bacterial infections of the prostate, seminal vesicles, or epididymis are common causes. Sexually transmitted infections like chlamydia and gonorrhoea can also raise white blood cell levels in the semen
  • Non-infectious causes: Conditions such as oxidative stress, extended periods of sexual abstinence, and inflammation related to autoimmune disorders can contribute to pyospermia
  • Lifestyle factors: Smoking, excessive alcohol consumption, poor hygiene, stress, and obesity are linked to a lower semen quality and higher white blood cell count in semen
  • Idiopathic cases: In some instances, no clear cause can be found, and the condition is termed idiopathic pyospermia

Understanding the underlying cause of pyospermia is essential for effective treatment. Accurate diagnosis through semen analysis and additional tests helps to ensure appropriate management strategies.2

Treatment and management 

The treatment for pyospermia depends on its cause, and sometimes, it may resolve on its own, especially if linked to temporary inflammation or mild infection. However, if tests indicate a bacterial cause, doctors often prescribe a course of antibiotics to clear the infection and reduce the number of white blood cells in semen. Even when cultures don’t show bacteria, antibiotics may still be recommended to target hidden infections. It is crucial to complete the full course of antibiotics as prescribed, because stopping early can allow the infection to return and make it harder to treat.

If the inflammation isn’t due to bacteria, doctors may suggest non-steroidal anti-inflammatory drugs (NSAIDs) to reduce swelling and improve sperm quality. Alongside medication, simple lifestyle changes can make a big difference. Drinking plenty of water, eating a balanced diet full of antioxidants, avoiding smoking and excessive alcohol, and maintaining a healthy weight can all support better semen health.

After treatment, a follow-up semen analysis will help to confirm whether the inflammation has cleared and whether sperm function has improved. If pyospermia persists and conception is still unsuccessful, a fertility specialist may recommend assisted reproductive techniques like IUI or IVF.

Most importantly, men should avoid self-medicating. Seeking proper medical advice from a urologist or fertility expert ensures the correct diagnosis and the best chances of restoring reproductive health.2

When to get tested 

Some men with pyospermia notice no symptoms, while others may have pelvic discomfort, pain during ejaculation, changes in semen colour or consistency, or a burning sensation when urinating. A history of prostate or epididymal infections can also increase the likelihood of pyospemia, and common contributors often include untreated urinary infections, recent illnesses, sexually transmitted infections, and certain lifestyle factors. So, if you and your partner have been trying to conceive unsuccessfully, or if a previous semen analysis indicated elevated white blood cell levels, it may be time to consider testing for pyospermia. The process is simple and usually available at fertility clinics or diagnostic centres. Early testing helps to identify any hidden inflammation or infection and allows for timely treatment, enhancing both fertility outcomes and overall reproductive health.6

Summary

Pyospermia can be reliably identified through a routine semen analysis. Detecting elevated white blood cells early is important because it allows doctors to address underlying infections or inflammation before they significantly affect sperm quality and fertility. Timely treatment, whether through medication, lifestyle changes, or assisted reproductive techniques, can greatly improve the chances of conception.

Regular check-ups and open communication with healthcare providers are crucial for maintaining long-term reproductive health. Men should feel empowered to discuss concerns about their fertility without embarrassment, as early intervention often has the greatest impact. By prioritising testing and seeking professional guidance, it’s possible to protect male fertility and ensure the best outcomes for those trying to start or expand a family.
