Introduction
Definition of alexia
Alexia is a rare condition in which people face trouble in reading and understanding written words as the result of a cerebral lesion, although they can write and speak normally.1 It is also known as "word blindness" or "agnosic alexia".2 In this condition, people face struggles to read or make sense of what they read. This disorder usually occurs by a stroke affecting the dominant side of the brain due to brain injury. People with this condition may also experience difficulty in writing(agraphia), trouble with numbers and calculations (acalculia), and challenges in understanding or producing spoken language (aphasia).
Importance of diagnosis and treatment
Diagnosis and treatment of alexia are important to understand the underlying causes to enable proper treatment plans that may include speech therapy or cognitive rehabilitation. Addressing alexia enhances reading abilities, improves independence and prevents the patient from developing secondary complications like frustration, isolation and depression. Effective management improves the quality of life of patients and also supports ongoing research for better treatment methods, improved outcomes and understanding of the condition in the long term.
Diagnosis of alexia
Clinical evaluation
Physical examination and patient history:
To evaluate the patient with alexia, doctors initially do a physical examination to understand how well the patient reads text compared to their normal reading ability before the brain injury. Doctors also evaluate the reading speed of patients with the use of short stories and test their spelling and writing abilities.
Diagnostic tests neuroimaging (MRI, CT scan)
A neurological examination is important to check for any other brain function issues. For this doctors often use neuroimaging techniques like CT scans or MRIs to locate and understand the severity of the problem in the brain. For example, in a case of pure alexia (difficulty reading without other neurological problems), a CT or MRI might show a stroke in the back part of the brain which causes the classic alexia without agraphia (difficulty writing).3 If a CT scan doesn't provide enough information then MRI should be considered because it is more sensitive. Additionally, a CT angiogram (CTA) is useful to detect problems in the left posterior cerebral artery that might cause strokes in the brain's back part (occipital lobe).
Neuropsychological tests
Neuropsychological tests are special tests that look at different parts of reading like recognising words, how fast someone reads and the mistakes they make. These tests are important because they help doctors figure out what kind of alexia someone has. Overall, these tests are crucial for understanding how severe the reading problem is and what exactly is causing it.
Types of alexia
These classifications help neurologists and psychologists understand how different areas of the brain contribute to reading abilities and how damage to specific regions can lead to distinct types of reading impairments.4
Alexia without agraphia (pure alexia or occipital alexia):
In this condition, a person has difficulty in reading but can write properly. It results from the damage in the brain's occipital lobe, which is responsible for processing visual information. In this form of alexia, people are able to recognise letters individually but struggle to read whole words or sentences.
Alexia with agraphia (Central alexia or parietal-temporal alexia)
In this form of alexia, individuals face difficulty in both reading and writing, as it results from damage in the parietal and temporal lobes of the brain, which involves language processing. Individuals in such a condition do not recognise letters properly and have trouble forming words while writing.
Frontal alexia
Frontal areas of the brain control language function and frontal alexia is associated with problems in the frontal areas. So people with this condition may face problems with more complex aspects of reading and understanding the meaning of written text.5
Spatial alexia
It results from the damage in the right hemisphere of the brain which involves spatial awareness, which means it affects the ability to understand the spatial arrangement of letters and words on a page.6
Treatment and management options
Addressing alexia is important to improve overall quality of life, there are different strategies for treating and managing alexia:7
Occupational therapy
This approach particularly focuses on improving the reading abilities of a person to make their life easier and make their quality of life better. Techniques like Multiple Oral Rereading (MOR) can make reading faster and more accurate. Therapists also use kinesthetic reading, which involves the use of fingers to recognise the letters and read more smoothly.8
Assistive technology
It involves the use of technology to manage alexia like special apps and e-readers which use letters of different sizes and colours to make reading easier for people with visual or cognitive issues from alexia. Text-to-speech software helps in dealing with reading problems by reading written words aloud. It allows the individual to manage their daily life activities by using practical technical solutions.
Speech and language therapy
It focuses on improving reading skills through specific exercises. These therapies involve customized treatment as per the type and condition of alexia. It makes reading easier by strengthening the ability to recognise and understand words. Techniques like phonological treatment help patients to understand written words better, especially in cases of phonological alexia.
Cognitive rehabilitation
It aims to improve reading abilities by using various strategies and exercises for cognitive strength. These exercises help patients identify letters and read more accurately in cases of severe alexia by involving touch and movement. Rehabilitation is personalised as per the need of each patient's cognitive strengths and weaknesses to help them recover their reading abilities.
Support groups and counselling
It provides emotional and psychological support to people who are facing the problem of alexia and to their families. In this problem individuals generally feel alone or frustrated, so these groups let people with similar problems come together and share experiences and practical ways to deal with them. One-to-one counselling helps to deal with the emotional impact of alexia on patient and on their families. It gives the feeling of being a part of a supportive community which improves overall well-being and mental health.
Medication
Although we can't consider medication as the main treatment for alexia, it helps to manage related conditions that contribute to the disorder like stroke symptoms or other neurological problems, which indirectly improve the reading abilities and patients feel better. However, there isn't much evidence on how well medication directly treats alexia, so more research is needed on using medicine for this purpose.
Prognosis and recovery in patients with alexia
Patient's outlook towards alexia significantly varies depending upon several factors, including the type of alexia, the cause of the injury and the extent of rehabilitation received post-injury. Some individuals experience improvement in their reading abilities, although complete recovery is not common.9 Studies reveal that some patients with pure alexia regain their reading abilities over time while some don't return to their pre-injury stage. Individuals have been observed with improved reading difficulties following a stroke from severe to mild stage of pure alexia. However, there is limited information available on how patients perceive their reading impairment.10
