Introduction
Mucosal melanoma, a rare and aggressive form of melanoma, arises in the mucosal linings of the body, including the nasal passages, oral cavity, anorectal (pertaining to the anus and rectum region), and external female genital area (vulva and vaginal region). Unlike the more common cutaneous melanoma, which is linked to ultraviolet (UV) radiation exposure from the sun or using sunbeds, the causative factors of mucosal melanoma remain less understood.1,2,3,4,5

What is mucosal melanoma?
Where and how does mucosal melanoma anatomically and physiologically occur?
Mucosal melanomas originate in the melanocytes present in mucosal tissues. These melanomas can affect various sites, such as the head and neck (including nasal passages and oral cavity), the gastrointestinal tract (especially the anorectal region), and the genitourinary tract (including the vulvovaginal region).1,2,3,4,5

Disease prevalence

Ultraviolet (UV) light
While UV radiation is a well-known risk factor for cutaneous melanoma, its role in mucosal melanoma is less direct. Studies suggest that individuals with high cumulative UV exposure may have a higher incidence of mucosal melanoma, possibly due to systemic immunosuppression or DNA damage in mucosal tissues indirectly. However, the evidence remains inconclusive, and further research is needed to establish a definitive link.7
Occupational hazards
Certain professions expose individuals to higher levels of harmful substances, increasing the risk of mucosal melanoma. For example, workers in the chemical industry, construction, and those handling certain dyes and solvents are at elevated risk. Implementing stringent safety measures and regular health monitoring can mitigate these risks.6

Diet and nutrition
Diet plays a crucial role in overall health, including the health of mucosal tissues. Diets high in processed meats and low in fruits and vegetables have been associated with an increased risk of various cancers, including mucosal melanoma. Antioxidants and nutrients found in fruits and vegetables can help protect mucosal tissues from oxidative stress and inflammation, reducing the risk.9,10
Smoking and alcohol consumption
Smoking is a major risk factor for mucosal melanoma, particularly in the oral and nasal cavities. The carcinogenic compounds in tobacco smoke cause direct damage to the mucosal linings, leading to mutations and cancer development. Alcohol consumption, especially heavy drinking, is another significant risk factor. It can cause chronic irritation and inflammation of mucosal tissues, facilitating cancer development.11,12
Sexually transmitted infections
Human papillomavirus (HPV) infection, often transmitted through sexual contact, has been linked to an increased risk of mucosal melanoma, particularly in the anorectal and genital regions. Safe sexual practices and vaccination against HPV can significantly reduce the risk of infection and subsequent cancer development.13,14
Genetic factors predisposing individuals to mucosal melanoma
Genetic factors play a significant role in the development of mucosal melanoma. Mutations in genes such as KIT, NRAS and BRAF (genes which control cell growth, cell diversity with specific functions, and survival within the body) are frequently observed in mucosal melanoma patients. These genetic mutations can drive oncogenic processes, leading to tumour formation.3,15

Family history

Genetic counselling and testing can help identify high-risk individuals and implement early detection strategies.
Prevention and early detection
Screening and monitoring
Currently, there are no standardized screening protocols for mucosal melanoma due to its rarity and the difficulty in accessing mucosal sites for examination. However, regular check-ups and monitoring of symptoms can aid in early detection. Symptoms include:
- Unexplained bleeding
- Persistent sores
- Unusual growths in mucosal regions
Advanced imaging techniques and biopsy methods are also being developed to improve early diagnosis.
Public health initiatives
Public health initiatives aimed at raising awareness about the risk factors for mucosal melanoma are crucial. Educational programs that highlight the importance of reducing exposure to carcinogens, adopting healthy lifestyle practices and recognizing early symptoms can significantly impact prevention efforts.
