Introduction

Felty Syndrome is a type of rheumatoid arthritis characterised by presence of multiple anomalies including a swollen spleen, multiple infections, abnormal WBC (white blood cell) count imbalance, implied as a rare disease having less prevalence.

Overview of cardiovascular disease (CVD)

Cardiovascular disease, (CVD) is a major complication affecting the heart due to vascular changes, atherosclerosis, and other factors. It is one of most prevalent and highly fatal diseases affecting developing countries, and highly prevalent in the global world.

Relation of Felty syndrome and CVD

Rheumatic diseases have many haematological manifestations. Blood disorders and other haematological abnormalities are major signs of rheumatic disease. In addition, novel antirheumatic biological agents may cause cytopenias.

Aetiology and pathophysiology

It is unclear how the body may lose its immune tolerance to itself, but researchers have linked HLA-DR4 homozygosity and environmental factors, such as immunosuppressive drugs, to pre-existing aggressive and seropositive rheumatoid arthritis (RA). In one prospective cohort study, about 86% of patients with Felty’s syndrome (FS) tested positive for HLA-DR4.

Genetic and environmental factors

It has been shown that the presence of two HLA-DRB1*04 alleles which encode shared epitopes that are predisposing factors for extra-articular manifestations of RA. There is a large body of evidence that RA patients with extra-articular complications are prevalent in CVD patients. 

Clinical presentation

Common signs and symptoms(e.g., rheumatoid arthritis, splenomegaly, neutropenia) along with multiple infections are the most important clinical signs in felty syndrome. 

Diagnosis

Felty’s syndrome can be a differential diagnosis of any systemic disease with neutropenia. LGL leukaemia, Hodgkin and non-Hodgkin lymphomas are among the neoplastic differential diagnoses. T-LGL may behave as a chronic inflammatory disease and is the main differential diagnosis of any RA patient who presents with neutropenia.

The mainstay of management of Felty syndrome is three-fold. Firstly, to address and treat the underlying RA. Secondly, to provide the immune system support for better resistance against infections. For that, prompt diagnosis and early management are key to getting the best results for multiple-syndrome patients.

There is no specified diagnostic procedure however, many healthcare professionals employ the use of genetic history taking and gene testing procedures like PCR.

Management approaches

Lifestyle modifications include modifying dietary habits, eating foods low in salt and fat. Addition of foods high in natural oxidants like garlic, ginger and blueberries is encouraged. 

Pharmacological treatments 

This includes anti-inflammatory drugs and cardio-protective medications.

Medications 

Rheumatoid arthritis should be treated as normal (e.g., appropriate exercise, heat treatments, nonsteroidal anti-inflammatory drugs (NSAIDS), etc).

Surgery

In many cases, treatment may include removal of the spleen (splenectomy).

  • RA is a precursor: Felty's Syndrome usually appears in individuals who already have RA, frequently after many years of active illness
  • Similar autoimmune mechanisms: In both cases, aberrant immune reactions result in tissue damage and inflammation
  • Similar genetic risk factors: Felty's syndrome and RA are both predisposed to develop when specific genetic markers are present
