Introduction

Fanconi anaemia (FA) is a rare genetic disease that mostly affects the bone marrow and is handed down through generations. The spongy region called bone marrow, located in the middle of your bones, is where stem cells give rise to red, white, and platelet-forming blood cells. 

Your bone marrow is unable to produce healthy red blood cells and platelets if you have FA. Individuals with FA are more likely to experience blood problems and some types of malignancies.

Gastrointestinal symptoms, such as inadequate food intake, nausea, abdominal pain, and/or diarrhoea, are common in FA patients. Significant discomfort is brought on by these symptoms, which may also be a factor in FA patients' poor weight gain.1

Pathophysiology of FA

FA is inherited in an autosomal recessive fashion, meaning that each cell's two copies of the gene carry a mutation. An autosomal recessive disorder affects only one copy of the defective gene in each parent, however, the parents do not usually exhibit symptoms or signs of the condition.

This disorder is inherited in an X-linked recessive form, albeit extremely rarely. As one of the two sex chromosomes, the X chromosome is home to the gene linked to X-linked recessive FA. The disease is caused by one mutated copy of the gene in each cell in males (who have one X chromosome). A mutation would need to happen in both copies of the gene in females (who have two X chromosomes) in order to induce the illness. Men are far more likely than females to be affected by X-linked recessive illnesses since it is improbable that females will have two mutated copies of this gene. The inability of fathers to pass on X-linked qualities to their sons is one of the characteristics of X-linked inheritance.2

Common gastrointestinal problems in FA

Gastrointestinal symptoms, such as inadequate food intake, nausea, abdominal pain, and/or diarrhoea, are common in FA patients. Significant discomfort is brought on by these symptoms, which may also be a factor in FA patients' poor weight gain. Since patients frequently do not voluntarily disclose gastrointestinal issues, physicians should encourage patients and their families to report symptoms during routine clinic appointments.

Various conditions can contribute to poor food intakes, such as the exacerbation of anatomic gastrointestinal abnormalities (narrowing of the digestive system or complications after repair), persistent infection or inflammation, adverse drug reactions, and neurological or behavioural issues.

While there are numerous causes of nausea, illnesses, delayed stomach emptying brought on by diseases, or drug side effects are frequently to blame. Most of the time, nausea is only momentary and goes away after the illness is treated or the drug is discontinued. In addition to causing nausea and abdominal pain, psychological stress, worry, and depression can exacerbate pre-existing gastrointestinal issues.

There are several causes of diarrhoea, such as medications, short bowel syndrome, bacterial overgrowth in the small intestine, and opportunistic infections of the gastrointestinal tract. It is possible to confuse constipation with diarrhoea if there is an unintentional stool release.3

Diagnostic approaches

  • Physical abnormalities: Approximately 60% of individuals with FA have physical abnormalities affecting the size and shape of their organs and body parts
  • Progressive bone marrow failure: Bone marrow failure symptoms are present in approximately half of adults and children with FA. By age 10, the majority of children with FA experience symptoms of bone marrow failure. By the time they turn 50, almost all persons with FA have these symptoms
  • Cancer: Acute myeloid leukaemia (AML) and malignancies of the head, neck, skin, gastrointestinal tract, or genital tract are common in FA patients. Approximately 30% of persons with FA were being treated for cancer at the time of their diagnosis4

Management and treatment strategies

Generally, medical professionals concentrate on managing blood abnormalities caused by FA, such as:

  • Bone marrow transplant: In order to cure leukaemia, preleukaemia, or bone marrow failure, medical professionals may advise a bone marrow transplant
  • Androgen therapy: This medication increases the synthesis of red blood cells in your body. If you have anaemia, your healthcare professional might suggest this course of action
  • Synthetic growth factors: Growth factors increase the production of red and white blood cells in your bone marrow. Physicians may suggest synthetic versions to increase the creation of bone marrow cells.
  • Surgery: To treat physical anomalies or restore damaged organs, medical professionals may employ surgery4

Challenges and future directions

If you have FA, you should keep an eye on your general health and notify your doctor of any changes you detect in your body that might indicate symptoms of FA. Here are a few instances:

  • When anything changes some normal genes into aberrant genes, FA occurs. Physicians refer to this phenomenon as intrinsic genetic instability. Genetic instability may make it more likely for chemicals like tobacco that cause cancer to cause more harm than usual
  • FA raises the risk of cancer, particularly skin cancer. Everyone should safeguard their skin and keep an eye out for any indications of skin cancer, but those with FA should exercise extra caution
  • Blood problems are caused by FA. If you have FA, you should take precautions to avoid cuts that could bleed. Moreover, keep an eye out for any physical changes, such as unexpected bleeding, bruises, or unusually high or low levels of fatigue. These could be indicators of blood diseases like leukaemia or anaemia4

Fanconi anaemia (FA) is an uncommon genetic condition. FA patients may experience severe blood problems. They also have a higher chance of getting some types of cancer. Significant physical disparities brought on by FA can have an impact on a person's growth and attractiveness. Some of the medical issues that FA produces can be treated by healthcare professionals. Not every FA patient experiences the same health issues. FA frequently resembles a chronic illness with various symptoms that might appear at any time.
