Sports injuries
Sports injuries can happen to athletes of any level, regardless of whether they are recreational enthusiasts or professional athletes. These injuries can vary in severity, from minor sprains and strains to more severe fractures and dislocations. These injuries can occur in any part of the body, including the bones, muscles, ligaments, tendons, and joints.
To reduce the risk of these injuries, athletes should follow proper training techniques, and warm-up routines, and use appropriate equipment. Adherence to safety guidelines is also essential in preventing sports injuries. Furthermore, prompt and proper treatment is crucial for effective recovery and preventing long-term complications.
Types of sports injuries
Sports injuries are generally classified as acute or overuse injuries. Acute injuries result from a sudden traumatic event such as fractures, dislocations, sprains, strains, contusions, and concussions, while overuse injuries occur due to repeated stress over time such as rotator cuff tendonitis, patellar tendonitis, Achilles tendonitis, medial and lateral epicondylitis etc.2
It is essential to understand the mechanisms that cause the pain itself to find the appropriate way of reducing it. Musculoskeletal pain is a common condition that can be caused by acute muscular injuries resulting from both direct and indirect trauma (as shown in figure 1). Direct trauma injuries are usually noticeable and can be caused by falls, sprains, and collisions. These injuries result in contusions at the point of impact. Indirect trauma occurs when the muscle is strained or forced excessively, disrupting the myofibers. This type of injury can be further categorized into passive and active injuries.2

Passive injuries happen when the muscle is over-stretched without contracting, while active injuries are caused by eccentric overload of the muscles. DOMS (Delayed Onset Muscle Soreness) is an indirect injury that may occur when starting a new exercise program or in athletes following weight lifting or other eccentric exercise activities. DOMS onset symptoms occur approximately 24 hours after exercise, peak within 72 hours, and slowly resolve over 5 to 7 days. Muscle pain associated with DOMS may be similar to immediate exercise-induced muscle soreness.
Inflammation is a natural response that occurs as part of the healing process following acute injury. Inflammation resulting from an acute injury such as an ankle sprain leads to oedema, hyperalgesia, and erythema. Unfortunately, inflammation can potentially increase tissue damage and delay healing. For these reasons, it is important to understand the mechanisms of pain reduction to alleviate acute musculoskeletal pain using one of the most widely used treatment options heat therapy.1
Heat therapy and its impact on sports injuries
Heat therapy is a widely used therapeutic technique that involves the application of heat to the body. This process results in an increase in tissue temperature, which promotes the dilation of blood vessels and increases the flow of blood to the affected area. As a result, more nutrients and oxygen are supplied to the site of injury, which helps in the healing process. Heat therapy is commonly used to alleviate pain, reduce muscle stiffness and spasms, and enhance tissue repair. It is an effective and non-invasive method of treatment that can be used alone or in combination with other treatments to hasten recovery.
There are two types of heat therapy, dry heat and moist heat
Dry heat has been used for a long period of time due to its effectiveness in decreasing pain and increasing mobility. However, it can cause burns on skin, fat, and other superficial tissues. Dry heat can be applied to the affected area using hot water bottles, heat pads, electric heat pads, heat wraps, and heated stones.
Moist heat is another form of superficial heat therapy where damp heat packs are applied to the injured area. Studies have shown that moist heat penetrates deeper and faster into injured tissues compared to dry heat. This is because moist heat is better able to transfer heat energy to the tissues.
Two other methods of providing heat to the body are Infrared Lamp and Short wave Diathermy.
Infrared lamp
This is a type of heat therapy that provides superficial heat to the body. This method is often used to treat pain and improve the healing of injuries involving superficial tissues, especially when a large area is involved. During treatment, the lamp is generally kept around 1-2 feet away from the patient, and the session usually lasts for 10-15 minutes. It is important to note that when using an infrared lamp, patients should avoid looking directly at the light to prevent the risk of cataract formation. Also, this method should not be used during the acute stage of inflammation. The heat produced from an Infrared Lamp does not penetrate more than 5mm beneath the skin surface.
Short-wave diathermy
This is a type of heat therapy that uses high frequency electromagnetic waves such as microwave and short-wave diathermies. This method provides heat to deeper tissues and is often used to provide relief from pain and promote healing. Unlike Infrared Lamp, this type of therapy can penetrate even tissues lying 5 cm deeper to the level of the skin. It is important to note that Short-wave Diathermy is contraindicated during acute stages, pregnancy, malignancy, infections, and in those with metal particles present in the body. Therefore, before using this method, it is essential to consult a healthcare provider to ascertain its suitability.2
Complications of heat therapy
Heat therapy is a useful modality to treat pain and promote the healing of injured tissues. However, it is important to note that it can lead to complications, especially for patients with certain conditions. For instance, individuals with multiple sclerosis, poor circulation, spinal cord injuries, diabetes mellitus, or rheumatoid arthritis are at an increased risk of experiencing disease progression, burns, skin ulceration, and increased inflammation if heat therapy is used improperly. It is also important to protect the skin of heat-sensitive or high-risk patients, especially in areas with decreased sensory function, to avoid adverse effects. Despite these precautions, superficial heat for low back pain remains a popular and generally safe treatment option, with minor side effects such as skin pinkness.1
Heat therapy at home
If you want to use heat therapy safely at home, there are several ways to do so.
- Usage of hot water bottles by wrapping them in several layers of towels so that they are warm to the touch rather than hot. Avoid putting the full weight of the limb on the heat pack as this may increase the likelihood of a burn. When using heat therapy, never use boiling water, and make sure the heat pack or hot water bottle is wrapped in enough layers of towelling to avoid burns.
- During treatment using hot water in a bowl, the temperature should be between 40 and 45 degrees Celsius. Leave it on for 10 to 15 minutes before removing it. This treatment can be repeated up to three times a day.
- Hot water bottles and wheat bags are good ways to apply heat. When using a heat pack, be sure to place it over the area to be treated and avoid lying, sitting, or sleeping on it to reduce the risk of burns. Heat should be applied for 15 to 30 minutes and can be reapplied every two hours. If the area starts to feel too warm, place an extra towel between the heat pack and your skin to reduce the temperature.
- Commercially available heat packs are an option, but it's important to follow the manufacturer's instructions for heating and application.3,4,5
