Overview

A pyogenic granuloma (PG), also known as granuloma pyogenicum and lobular capillary hemangioma, is a benign vascular tumour that forms bright red lesions on the skin, mucous membranes, and capillaries. It grows very fast, bleeds frequently, and can recur after treatment. It can be triggered by injury and certain medications. Though it can occur at any age, it is common in older children and young adults.1,2 

In a study, it was found that a one-year-old female child was diagnosed with PG, a rare condition, likely caused by local trauma from tooth-brushing. Histological analysis confirmed PG with features like nonkeratinized stratified squamous epithelium, vascular proliferation, and endothelial cell growth. The lesion showed intense chronic inflammatory cell infiltration, emphasising its inflammatory nature.3

The exact cause of pyogenic granuloma is unknown. However, it is linked to an imbalance of pro-angiogenic and anti-angiogenic factors in the body that leads to rapid capillary proliferation. Minor trauma can contribute to 7% of the cases. Other possible reasons can be infections or vascular malformations (if present). 

Pyogenic granuloma can be treated through surgical excision, curettage, electrocautery, or vascular laser. Recurrence is possible. However, simple alternative treatments with home remedies can cause relief and help in reducing the symptoms to a great extent. Causes and location of pyogenic granuloma

  • Cutaneous (skin)— It occurs in the trunk area in adults, while it is more common in the head and neck area of children, followed by the trunk. A subungual or periungual pyogenic granuloma can occur around the nails and may affect multiple fingers. Two types of lesions can be found:
    • Satellite lesions: can occur around a primary lesion after treatment
    • Disseminated lesions: can occur rapidly with no preferred site. It is rare and unpredictable
  • Mucosal: Less common than cutaneous. It includes the lips and the gingival mucosa areas. It can also spread to the tongue, nasal mucosa, conjunctiva, vagina, and even cervix. During pregnancy, it can affect the buccal mucosa area
  • Vascular: This condition appears above vascular malformation types, including nevus flammeus. The lesions can occasionally exist beneath the skin or inside blood vessels
    • Subcutaneous PGs exist exclusively in female patients as well-defined nodules
    • PGs originating from intravenous vascular sites demonstrate slow growth patterns that usually emerge on the neck or upper extremities
    • The gastrointestinal tract rarely develops this condition, which closely resembles polyps, while also leading to possible anaemia through bleeding disorders

Most common home remedies for pyogenic granuloma

Salt therapy is the most common alternative treatment for PG that uses its high osmolarity to dry out the pyogenic granuloma. This further causes the tissue to reduce in size, making scars minimal and reducing the risks of recurrence. 

People can execute this cost-friendly treatment through accessible self-administered methods at home. The treatment offers appropriate benefits to three distinct patient populations that include: 

  • Children
  • Elderly adults
  • People who desire to avoid scarring

However, researchers require more extensive studies to prove its effectiveness across a wide range of cases.5

Method of application

Here is a recommended method for treating PG with the use of salt. 

  • Apply a thin layer of petrolatum or paraffin around the PG to protect the surrounding skin before covering the lesion completely with table salt
  • Secure the salt with an occlusive bandage and repeat the process daily for 14 days or until the lesion clears
  • If no improvement occurs or the lesion recurs, a biopsy should be considered to rule out malignancy4

Effectiveness of salt treatment for pyogenic granuloma: case studies

Case study 1:

A study showed salt therapy enabled 50 patients with pyogenic granuloma to resolve their lesions completely in 14.77 days, at which point they had no scarring. Additionally, 94% of patients had fast bleeding improvement. 

Only one patient reported a recurrence 11 months later. These findings have support through other case reports showing one patient healed completely with healthy nail growth after three weeks.5 

Case study 2:

A Saudi girl aged 11 developed an erythematous friable papule 1 cm large on her lower lip that bled whenever she pressed it or ate, thus causing her to lose weight. The swelling developed on her lower lip without any previous chronic health conditions or medication usage. 

Clinical evaluation with bleeding pattern pointed toward a diagnosis of pyogenic granuloma. Medical staff began the treatment with shave excision, followed by electrocautery and silver nitrate application until the bleeding persisted, so they decided to use sutures for further management. 

The patient received cryotherapy after incomplete excision, yet it proved intolerable to the patient, so they began home treatment using table salt together with Vaseline and gauze. The treatment period with salt exposure spanned two weeks until the lesion completely healed.6

Risks and limitations of home remedies

Some of the risks and limitations of the home remedy or salt therapy include:5-,8

  • Mild stinging or burning sensation: Some patients report mild and reversible stinging or burning sensations during the initial application of salt. This sensation can resolve quickly after the salt is applied. It doesn't remain for a longer time after treatment
  • Irritation of surrounding skin: In some cases, there might be slight irritation in the skin that surrounds the granuloma area. This may happen if the salt is applied directly in that area without any protective barrier. Hence, it is recommended to use petroleum jelly before applying salt
  • Occasional bleeding: Some patients may experience slight bleeding from the granuloma area during the application. This can be more intense if the lesion is raw and has previously been traumatised. But the bleeding may resolve quickly.
  • No major adverse effects: Studies show that there are no significant effects associated with the salt treatment for PG. The treatment is found to be safe and effective. In some cases, there is a complete resolution of this problem without residual scarring
