Often, this swelling occurs when fluid builds up outside your blood vessels due to various factors, including heart-related issues. Heart enlargement, known as cardiomegaly, occurs when the heart’s chambers grow larger, usually due to an increased workload or structural issues.1,2

When your heart expands, your pumping blood efficiency can be lost, leading to a backup of fluid in your legs and feet. This swelling, or edema, is more than just uncomfortable; but is a sign that your heart may be struggling to keep up with your body’s needs. Understanding how heart health connects with symptoms like edema can help you take proactive steps, ensuring you get the right care when it matters most.3

Connection between the heart and fluid regulation

Your heart plays a prominent role in regulating fluid balance and circulation, ensuring that each tissue receives the oxygen and nutrients it needs while efficiently removing waste. Noticing swelling in these areas may lead you to signs of your heart not effectively moving blood through your body, therefore creating a backup of fluid in the veins.

Fluid leakage

This high pressure in your veins can eventually force fluid out of the blood vessels and into the surrounding tissues, resulting in swelling, or edema. When this fluid leaks out, you may experience uncomfortable swelling, most often in your legs, ankles, and feet, where gravity tends to pull fluid downward. This swelling can be an early sign that your heart needs support to function properly.

Right vs left side heart failure

The side of the heart affected can impact where edema appears. In right-sided heart failure, the blood returning to your heart from the body encounters resistance, often leading to swelling in your lower limbs. Left-sided heart failure typically impacts the lungs, leading to fluid buildup and shortness of breath, but it can also place additional strain on the right side of the heart, contributing to PE as well. Recognising these signs can empower you to address your heart health before symptoms progress.6

Signs and symptoms of peripheral edema

Visible swelling

One of the most noticeable signs of PE is visible swelling, often occurring in your legs, ankles, and feet. This swelling may give your limbs a heavy, puffy appearance and can make everyday activities, like walking or standing for long periods, uncomfortable. You may also feel as though your shoes, socks, or pants are tighter than usual.

Pitting edema

Another common symptom of PE is called "pitting edema." Pressing on the enlarged area may cause a dent or "pit" that lasts for a short while before slowly going away. This pitting effect indicates that the tissue has accumulated extra fluid that is difficult to reabsorb.

Other associated symptoms

PE doesn’t just affect your legs and feet; it can come with other noticeable symptoms as well:

  • Shortness of breath: This can occur when the fluid buildup affects areas closer to your chest or abdomen
    • If you’re experiencing swelling along with breathlessness, it could indicate that your heart is under additional stress
  • Fatigue: Excess fluid in your body can make you feel unusually tired or drained, as your body has to work harder to maintain proper circulation
  • Difficulty lying flat: If lying down feels uncomfortable or makes breathing harder, this may be due to fluid accumulation affecting your lungs or abdominal area, which often accompanies more advanced edema

    • They’ll also look for other signs that could suggest heart failure or circulatory issues
  • Imaging tests: An echocardiogram may assess your heart’s size and function, while a chest X-ray can help confirm if your heart is enlarged. These tests give insight into how well your heart manages your blood flow
  • Blood tests: Blood tests are valuable for detecting biomarkers such as the brain natriuretic peptide (BNP) test, which can indicate heart failure or other underlying health issues related to fluid retention8

Treatment approaches

  • Compression stockings improve blood flow in the legs, while elevating them whenever possible can reduce fluid pooling.
  • Limiting salt intake is also essential, as it helps prevent fluid retention, making you feel more comfortable and reducing pressure on your heart.
