Introduction

A country's healthcare system is its fundamental infrastructure. A significant part of the population of most regions lives in remote areas and frequently faces significant barriers to accessing necessary health care. However, constantly changing technologies have altered the delivery of health services. Innovative ideas, like telemedicine and drones, are changing the way that communities get medical care.

In this article, we will look at the healthcare problems faced by people in remote and underserved areas and how telemedicine can help resolve those issues, using the medical disorder cardiomegaly as an example.

What is telemedicine?

“Tele” is a Greek word meaning "distance, “and “mederi” is a Latin word meaning “to heal." Time magazine called telemedicine “healing by wire." Although initially considered “futuristic” and “experimental,” telemedicine is a reality today and has come to stay. Telemedicine has a wide range of applications, including patient care, education, research, management, and public health. It is a comprehensive approach to providing effective healthcare services. It works by connecting patients with healthcare providers who are not in the same place.1

The World Health Organisation (WHO) defines telemedicine as the delivery of healthcare services, where distance is a critical factor, by all healthcare professionals using information and communication technologies for the exchange of valid information for diagnosis, treatment, and prevention of disease and injuries, research and evaluation, and for the continuing education of healthcare providers, all in the interests of advancing the health of individuals and their communities.

People who live in rural or remote areas around the world do not get timely, high-quality medical care. This could be because of several factors, such as a lack of specialist doctors in these areas and many more.2,3

What are the healthcare issues faced by people living in remote and underserved areas?

Healthcare delivery in rural and underserved communities presents unique issues that have a considerable impact on access, quality, and efficacy of medical treatments. The health outcomes in these areas are different from those in urban areas because they are geographically isolated, have few resources, and face socioeconomic problems. The most commonly seen health services issues in these areas are:3

  • A shortage of healthcare providers 
  • Inadequate or limited healthcare infrastructure
  • A higher prevalence of chronic diseases
  • A considerable distance to obtain specialised medical care

Telemedicine is a promising solution that uses technology to bridge these gaps. It helps provide access to medical expertise and improve healthcare delivery for people in geographically remote or underserved areas.

Now, let us look at how telemedicine can help manage some of the most serious medical issues that require the expertise of specialists. Here, we will look at one such medical problem called heart enlargement or cardiomegaly to understand how telemedicine can help manage this in remote or underserved areas.

First, let us have a better understanding of this illness.

What is heart enlargement?

Our heart is a muscle that works like a pump. It distributes blood to every part of the body for the most basic life processes. The medicos describe its size as that of a clenched fist. Imagine such a small organ playing a huge role in our human body. “Don`t take it on your heart”, truly said by everyone. Yes, our tiny heart can get sick if its workload increases.

Some physiological or pathological conditions can increase the workload on the heart. It may cause our hearts to beat more forcefully than normal. Cardiomegaly, or an enlargement of the heart, may result from this.

Cardiomegaly is an umbrella term for various conditions leading to an increase in the size of the heart. It is not a condition in itself but a symptom of the underlying problem that makes the heart work harder than normal. It has become more common lately and has a high mortality rate. It is also known as heart enlargement, which occurs when the heart's size nearly doubles the inner diameter of the rib cage.7

The underlying conditions that cause heart enlargement could be physiological, such as pregnancy, or pathological, involving heart muscle disease. Many possible causes of heart enlargement include:7

How can telemedicine help in monitoring and managing heart enlargement cases in remote areas?

Telemedicine can be an effective tool for educating and supporting patients with cardiomegaly (enlarged heart) in remote areas. To achieve the best results, patients must be closely monitored and educated about their illness. Here's how telemedicine can help in these cases:

Remote consultations with specialists

Remote consultations allow patients to communicate with healthcare providers and specialists via telecommunications technology, removing the need for in-person visits. Patients can discuss their medical concerns, receive medical advice, and seek expert opinions from healthcare professionals via video conferencing or phone calls. 

Remote consultations offer several advantages:2,3

  • They overcome geographical barriers, enabling patients to receive specialist care that might not be accessible locally in underserved and remote places
  • They reduce travel burdens and associated costs for patients living in remote areas 
  • Remote consultations also save time by eliminating the need to wait in healthcare facilities and providing easy and timely access to medical expertise

Remote monitoring 

Telemedicine can be utilised for remote patient monitoring, enabling healthcare providers to remotely track patients' vital signs, symptoms, and treatment progress. Through wearable sensors, mobile health applications, and telemonitoring devices, patients can receive continuous monitoring and timely interventions.

Remote patient monitoring can also help detect complications or changes in health status early on, allowing for proactive and preventive care. In addition, remote patient monitoring has helped reduce hospitalisations, enhance self-management, and improve patient outcomes.4

Real-time data sharing

Telemedicine has been used to transmit medical data electronically, allowing healthcare providers to review patient records, diagnostic images, and laboratory results from a distance. 

This promotes effective collaboration among healthcare professionals and informed decision-making.1,3

Prompt medical care

Telemedicine allows for quick access to medical care if the condition gets worse and time is critical. Healthcare practitioners can reduce treatment delays and can provide immediate care through virtual visits and remote consultations.2,5

Patient education and support

Telehealth consultations can encourage patients to make lifestyle changes that are necessary for managing cardiomegaly, such as:2

  • Dietary changes to improve overall heart health
  • Exercise plans designed specifically for the patient's physical abilities, with a focus on improving heart strength and function 
  • Mindfulness and relaxation exercises are stress management techniques that can help reduce strain on the heart

Prescription delivery and medication management

Telemedicine has made it possible to deliver prescriptions to patients remotely, which is more convenient and efficient. Healthcare providers can review patient medical records, assess symptoms, and prescribe medications remotely via telecommunications channels. Patients can get their prescriptions electronically, and they can be sent straight to pharmacies or sent through the mail.

Telemedicine-based prescription delivery provides several advantages. It eliminates the need for patients to make in-person visits for prescription refills, saving them time and money on travel. It also helps people take their medications as prescribed by making sure they can get them on time, keeping treatment as uninterrupted as possible, and lowering the chances of not following through with their prescriptions.6

Summary

People living in remote areas around the world face significant challenges in accessing timely, high-quality specialist medical care. As specialist doctors are more commonly based in densely populated urban centres, residents of remote regions often have limited access to advanced healthcare services.

Telemedicine offers a promising solution to this disparity by enabling the remote delivery of healthcare. It enhances the monitoring and management of serious conditions, such as cardiomegaly (heart enlargement), through virtual consultations, real-time data sharing, remote patient monitoring, and digital prescription services.

By reducing geographical and logistical barriers, telemedicine improves continuity of care and empowers patients through education and timely intervention. While it may not replace all in-person services, it plays a vital role in strengthening healthcare access and outcomes in underserved areas.
