Overview

Epstein-Barr Virus (EBV) is one of the most commonly transmitted viruses worldwide, often referred to as the "kissing disease" due to its ease of spread through bodily fluids, particularly saliva.1 It can also be transmitted by sharing utensils or through water droplets from coughing and sneezing. As a member of the herpesvirus family, EBV is so prevalent that approximately 90% of adults are estimated to have been infected at some point in their lives.2 The severity of EBV infection varies widely, ranging from completely asymptomatic cases to severe infectious mononucleosis.1

Currently, there is no vaccine available to prevent EBV, so the primary methods of prevention include avoiding close contact with infected individuals and refraining from sharing utensils. The most significant public health concern associated with EBV is infectious mononucleosis, which can sometimes result in hospitalisation, placing a strain on healthcare systems. Additionally, those infected are contagious and may pose a risk to individuals with weakened immune systems.3

Global prevalence of Epstein-Barr Virus 

EBV infection rates worldwide 

As mentioned, 9 out of 10 adults worldwide have been infected with EBV at some point in their lives. Among viral infections, EBV, along with other herpes viruses, is one of the most prevalent globally. EBV is widespread across the world, with nearly all populations showing signs of infection, even in rural areas. 4

Age-related prevalence

EBV can be transmitted at various stages of life, with infections occurring as early as infancy and extending into late adulthood, however, most primary infections occur during childhood or early adolescence. The epidemiology of EBV varies significantly by geography and is believed to be influenced by socioeconomic factors, particularly living conditions. For instance, young children in densely populated cities in Central Africa have an 80% chance of testing positive for EBV, compared to just 45% in rural areas of the United States.4

While it might seem concerning that infants and young children are exposed to the virus early in life, the risk of severe disease is actually higher when the virus is contracted during adolescence or adulthood. Those infected at these later stages are more likely to develop a severe form of the disease, approximately 50%, often resulting in infectious mononucleosis.4

EBV prevalence by sex

The prevalence of EBV infection differs between males and females. According to a study, individuals assigned female at birth are more likely to have anti-EBV antibodies in their bloodstream during childhood and early adulthood compared to those assigned male at birth. This suggests that females may be more susceptible to EBV infection than males.5

Transmission and risk factors 

Modes of transmission

EBV is primarily transmitted through bodily fluids, especially saliva, but it can also spread through blood and semen during sexual contact, blood transfusions, and organ transplants.3 The virus is most commonly spread through:

  • Kissing
  • Sharing utensils, food, or drinks
  • Coming into contact with objects contaminated with saliva3

Risk factors for EBV infection

The primary risk factors for infection include those previously mentioned, such as kissing, sharing utensils, and contact with items associated with individuals in the infectious stage. To reduce the likelihood of contracting either type of EBV, it is advisable to avoid these behaviors.3

There are two types of EBV, Type 1 and Type 2, which differ slightly in their genetic makeup but behave almost identically when a person is infected. One notable difference between Type 1 and Type 2 EBV is the risk of transmission through sexual contact. Type 2 is more likely to be transmitted through sexual activity than Type 1. Additionally, a study found that the use of contraceptives could reduce the overall likelihood of EBV transmission.6

Epidemiology and public health implications 

EBV and related diseases

As previously mentioned, teenagers and young adults who contract EBV often develop infectious mononucleosis, commonly known as glandular fever. 1 A study revealed that individuals who experience a primary EBV infection in adulthood have an 83% chance of being hospitalized, compared to 60% in children. 7 This condition is a public health concern because, in severe cases, it can lead to hospitalization. EBV has also been linked to various other conditions, including cancers and autoimmune diseases. Infection with EBV significantly increases the risk of developing Burkitt Lymphoma (BL), a type of cancer that originates in the lymphatic system, particularly in white blood cells.9 Additionally, EBV infection raises the likelihood of developing multiple sclerosis, an autoimmune disease that leads to the progressive degeneration of neurons in the brain and spinal cord.10

Importance of early detection and public awareness 

Early detection and public awareness of EBV are crucial for several important reasons, including the following:

  • Prevention of severe complications: Early detection of EBV helps prevent severe complications by enabling healthcare providers to manage symptoms more effectively
  • Reducing the spread of the virus: Understanding the importance of avoiding behaviors that increase the likelihood of infection is essential for controlling the spread of EBV
  • Lowering the risk of associated diseases: Early detection could lead to early intervention, potentially lowering the likelihood of developing associated diseases such as cancer3

Diagnosis and detection

Methods of diagnosing EBV

EBV cannot be easily diagnosed based on symptoms alone, as many of its symptoms, like fatigue and fever, are common to other viral infections. Instead, a combination of physical examination and symptom evaluation is used to form an initial assessment. This is followed by a blood test to confirm the presence of EBV. The blood test measures the levels of three types of antibodies that increase in response to EBV infection and are detectable in the blood.3

Direct detection of EBV

The most common method for quantifying and directly detecting EBV in whole blood or plasma is through a PCR assay. PCR amplifies any EBV DNA present in a sample, making it possible to confirm an infection directly. This provides definitive evidence that someone has been infected with EBV. 11

Mass screening of EBV 

While EBV is not currently mass-screened in the general population, EBV markers are being used to screen high-risk groups for nasopharyngeal carcinoma (NPC). In certain regions of Eastern Asia, these markers help identify individuals at higher risk of NPC, as those who test positive for EBV markers are significantly more likely to develop this cancer.12

Does EBV go away with time?

After direct infection with EBV, young children and some adolescents often remain asymptomatic. However, when symptoms do appear, they typically resolve within two to four weeks. In more severe cases, where symptoms like swollen lymph nodes or enlarged liver and spleen occur, full recovery may take several months. 1

Does EBV reduce life expectancy? 

Direct death from EBV infection is rare, but the virus can significantly increase the risk of developing other potentially fatal conditions, including lymphomas, carcinomas, and multiple sclerosis. Research indicates that approximately 2% of all cancers can be attributed to prior EBV infection.13

While the primary symptoms of EBV often subside after a few weeks, the virus remains dormant in the body. In some cases, it can reactivate, particularly under conditions such as stress, a weakened immune system, or hormonal changes.1
