Introduction 

Chronic infections are usually infections which are prolonged or persistent. This means that they can frequently reoccur in those who are infected and are extremely problematic for those that have to live with them. These types of infections can be caused by a variety of pathogens. These usually include bacteria, viruses or parasites. Some of these can also develop into lifelong infections, these are chronic infections.

After an initial and acute infection, the period of infection may become persistent. This is when the immune system is fully stimulated.

Types of chronic infections 

  • Chronic lung infections
  • Tuberculosis (TB)
  • HIV
  • Chronic lyme Disease

These infections can leave people with symptoms even after the infection clears up. Because of the immune system set up to deal with infections, sweating and weakness are common symptoms which accompany these types of infections regardless of what the infecting agent is. 

Sweating and weakness occur as a result of the immune system working to manage and eliminate the infection from the body. These symptoms are usually accompanied by a fever, which is needed in an immune reaction. As the body must exert itself to kill the infective agent, weakness is commonly experienced and expected from the body.

Some chronic infections are curable where others are not. Persistent symptoms of sweating and weakness should therefore be acknowledged to have a possible link to a chronic infection.

Mechanisms behind symptoms of sweating and weakness

When an infection occurs, the immune system becomes involved to try to counter and eliminate the foreign pathogen from the body. When this happens, a series of steps takes place to allow for this. Sweating and weakness are both a result of this immune reaction. These symptoms usually mean that the body is adequately responding to infection.

Causes of chronic infections

Often when people have recurring chronic infections this is related to a number of existing conditions. Some people may be immunocompromised meaning that their immune system is not strong nor sufficient enough to effectively fight off the infection.

In certain infections like lung infections for instance, structural damage to the lung can mean that a person is more prone to chronic infection.

Lastly, chronic infections can be as a result of problematic organisms causing the infection. Some organisms (pathogens) may be harder to kill and get rid of than others. Mycobacterium for example, which is the bacteria that causes tuberculosis, is a very difficult bacteria to eliminate from the body and so people with TB will often have some with recurrent infections.

Sweating 

In the context of infection, sweating is often associated with the fever mechanism of the immune reaction. During infection, a fever usually occurs as part of the innate regulatory immune response to attempt to eliminate the pathogen from the body. It is a protective response to foreign bodies and often a hallmark of infection.

When suffering from an infection, body temperature can be raised from 1 degree celsius, all the way up to an increase of 4 degrees celsius.6

Sweating occurs as a way to regulate the body's natural temperature. It's important to dissipate heat which is generated in the immune reaction and maintain a healthy body temperature.

A fever is first induced from recognition of pathogens by the immune cells in the innate immune system. In response to this, there is a cascade of different signals which eventually triggers an output signal to neurotransmitters to raise the body core temperature. 

This can be very helpful in increasing the immune cells within the immune system effectively. Fever however can be dangerous when unregulated and so the body must take care to regulate body temperature effectively to avoid tissue damage and other dangers. 

Thermoregulation occurs through sweating. Sweating is important to dissipate heat to ensure the body returns to a safe and normal temperature and can function as usual.11

Night sweats in particular are very common in the case that a person has HIV or TB infection.5 This type of sweating often presents itself, this means that the degree of sweating is so much that bedsheets are required to be changed.5

Sweating is often seen as a distressing symptom but it is a very important part of the immune reaction. However in chronic infections it can become much more problematic. A fever is not necessarily beneficial in all cases and one where inflammation is significant, an uncontrolled fever can be dangerous.

This excessive sweating is also what contributes to the feeling of weakness during chronic infection. Weakness is a commonly reported symptom and is expected in chronic infection. The process of regulating temperature can fatigue the body.11

Weakness

The body works hard in times of danger. An infection is taken very seriously by our immune system and all effort must be made by the system to conserve energy. Weakness is as a result of this need.

When fever is induced in the human body, it also requires an increase in metabolic rate.

People with chronic infections and sweating can experience loss of fluids due to excessive sweating, this can dehydrate the body.

In some viral infections, muscle weakness and fatigue are common symptoms of the type of infection.10 In chronic infections such as HIV and lyme disease weakness can be a severe symptom.

Weakness can vary but some people can present with severe muscle weakness in particular.9 This means that people with chronic infections can become decreasingly mobile and even become bed ridden.14

Examples of chronic infections which can present with sweating and weakness

Sweating and fever will often show together especially in the case of infection. Some of these infections are treatable and curable whilst others can be managed but not cured. TB and HIV are some of the most significant chronic infections that people are diagnosed with.

  • TB. Symptoms of TB usually include night sweats and fever, as well as fatigue. The chronic infection involved with TB is due to the bacteria Mycobacterium tuberculosis. This causes a persistent immune response and has the ability to avoid the initial immune response1,2
  • HIV. HIV also results in fatigue as well as night sweats. The issue with HIV is that it is caused by a virus rather than a bacteria. This virus has an ongoing replication cycle, which means that it can multiple rapidly. As well as the persistent replication of the virus it is able to dysregulate the immune system meaning that the immune system, which usually deals with pathogens like viruses, cannot effectively eliminate the virus. Unfortunately viruses are able to bypass the regulatory effects of the immune system, which results in recurrent infection3
  • Malaria. Malaria is a good example of a chronic infection not caused by bacteria or a virus, but rather a parasite. The malaria parasite cycle occurs in the bloodstream, which provides excellent conditions for parasitic disruption to the immune system. This type of chronic infection is, however, treatable as long as a person has the necessary access to antiparasitic therapeutics.

In regions where malaria is prevalent, lack of treatment is the cause of the chronic infection.7,8

Diagnosis and management of fever and weakness

  • Diagnosing chronic infection 

Sweating and weakness can be the initial signs of infection. When they are severe or recurrent this can point to more severe chronic infections. Both HIV and TB for example have significant and typical symptoms of night sweats and weakness. These can be clinical symptoms which lead to a diagnosis of the infection. Consistent symptoms

like these should be looked out for.

  • Treatment of underlying infection

Targeting the underlying cause of infection is the most efficient way of treating chronic infection. This means eliminating the pathogen usually through drug interventions like an antibiotic, antiviral or anti parasitic drug.With the help of these drugs, the infective agent can be eliminated.However not all these drugs work at all or very well on all types of chronic infection. Both HIV and TB are examples of chronic infections which may need an alternative method of management.

  • Symptoms management

HIV and TB are both manageable, though it was not always like this. This means that antiviral and antibacterial drugs will not always effectively cure the infection. For HIV, these drugs can help however in managing symptoms and stopping the transmission of the virus.12

How can chronic infections be prevented?

  • Vaccinations. Some chronic infections like TB or malaria do have vaccinations. Vaccination programs can be implemented to prevent infection and transmission of the pathogen which causes these types of infection. Though for infections such as HIV and certain respiratory lung infections, a vaccination cannot help
  • Public awareness campaigns to identify chronic infection early. Being aware of symptoms and the infections that spread is important in order to take the measure to avoid or treat these chronic infections accordingly13

Summary

Chronic infections can manifest with symptoms including sweating and weakness. These symptoms are driven by an immune reaction due to the body's efforts to regulate the internal environment. Common chronic infections such as tuberculosis, HIV and relapsing malaria illustrate how prolonged infection can lead to these debilitating effects. 

Early recognition and targeted treatment of the underlying infection are crucial for alleviating symptoms and improving patient outcomes.
