Overview
Alcohol consumption has been found to have a complex effect on heart health. Excess alcohol consumption can lead to heart problems such as alcoholic cardiomyopathy, high blood pressure, heart failure and stroke.1 Heavy drinking (4 or more standard drinks per day) has been linked with almost all irregularities in heart functioning that can be found. What are the overall effects of alcohol on the body?
There are numerous ways in which alcohol damages your body. It is a ‘depressant’ which means it slows down the brain and other bodily functions. This manifests in the following ways:

- Slurred speech and coordination

- Liver damage (cirrhosis, liver cancer)
- Brain damage (alzheimer, dementia)
- Weakened immune system

However, this article will mainly focus on the damage that alcohol does to the heart. For example, cardiomegaly is a condition in which the heart becomes abnormally enlarged in sizewhich is a common issue associated with excessive drinking. High alcohol consumption can also strain your heart muscles, increasing the risk of heart attack and stroke.3
How does excess alcohol consumption lead to heart enlargement?
Chronic alcohol drinking leads to the dilation of heart chambers, reducing the heart’s overall efficiency and ability to circulate oxygenated blood to other body parts. Alcohol consumption increases troponin levels in the body, which is a marker of heart muscle damage. This leads to irregular heart rhythms, for example a potentially life-threatening condition called atrial fibrillation. Chronic alcoholism often results in multiple organ failures including heart failure. According to research, it has been very challenging to detect whether heart damage is alcohol-induced in a particular individual. This is because the body tries to compensate by activating the stress response (sympathetic signal firing), which sends warning signals to the kidneys, which then release compensatory hormones. All these mechanisms are compensatory and lead to increased blood volume, they also have destructive effects such as left ventricular dilation and decreased cardiac output.
Moreover, on a cellular level, excessive alcohol consumption damages the heart cells. Alcohol creates harmful chemicals (oxidative stress) and disrupts the balance of calcium in these heart cells, which is crucial for proper heart function. People with a certain genetic makeup and those with vitamin B1 deficiency are particularly susceptible to alcohol toxicity.2
What risks are associated with the development of alcohol-related heart disease??
Alcohol has gained cultural value within modern society, especially in the 18-30 age group. Studies show that at social gatherings, men tend to drink 3 to 4 drinks in 2 hours and women drink 1 to 2 drinks in that time. This is considered binge drinking, meaning excessive consumption of alcohol. As for risk factors that increase vulnerability to alcohol-related disorders, age plays a role, as well as hypertension (high blood pressure), type II diabetes mellitus (high blood sugar levels) and obesity. Statistically, low-income individuals who are living with high stress levels are more prone to heavy drinking, as are those with mental health issues such as depression and anxiety.5
What are the preventative measures?
Preventing and managing alcohol-induced cardiomyopathy requires a multifaceted approach. It is important to understand that addiction of any kind is difficult to cure. Therefore, gradually reducing alcohol intake, i.e. by cutting down to less than 2 drinks per day for men and less than 1 drink per day for women, is a good starting point. The idea is to avoid binge drinking in particular, which poses a high risk for heart problems. Individuals with existing heart problems are advised to avoid alcohol completely.
A healthy, balanced diet plays a vital role in overall health, including heart health. Focus on consuming whole grains, fruits, vegetables, lean proteins, and healthy fats, which contain essential nutrients like minerals, vitamins, and omega-3 fatty acids. Lifestyle modification like engaging in regular exercise of at least 30 minutes of moderate-intensity physical activity every day, is also beneficial.
Regular monitoring and support are essential for effective management. Attending health check-ups and engaging with regular blood pressure and lipid profile checks, as well as cardiac function tests (echocardiograms, ECGs) as recommended by your healthcare practitioner.
