Introduction

Tooth decay happens when mouth bacteria secrete acids that damage tooth enamel - the protective outer layer of teeth. This can lead to holes in the teeth, causing pain, infection, and potentially, tooth loss if treatment isn’t sought.1

Diet has a huge impact on oral health. High sugar and high carbohydrate foods and drinks can quicken bacteria growth in the mouth – and therefore a higher risk of tooth decay.2

Understanding tooth decay

Definition and causes of tooth decay

Tooth decay is commonly characterised as the breakdown of the tooth enamel structure caused by acids produced by bacteria. When sugary and starchy foods are consumed, mouth bacteria feed on these sugars and produce acids, as a by-product, which then attack tooth enamel. Thus, leading to small holes or cavities developing.

Many factors contribute to the development of tooth decay, such as poor oral hygiene, or frequent consumption of sugary or acidic foods and drinks. Understanding these causes can aid in the minimisation of tooth decay formation and good oral health maintenance.

Role of bacteria and plaque in the decay process

Bacteria have a crucial role in tooth decay. The mouth naturally holds various types of bacteria (e.g., Streptococcus mutans and Lactobacillus). These bacteria are very adept at sugar metabolization and acid production. When feeding on sugars from food, these bacteria create a sticky film, called plaque, on the teeth.

Plaque allows plaque to stick to the tooth surface and continue producing acids. Without regular brushing and flossing, plaque will build up, and harden into tartar – promoting tooth decay and gum disease.

Being knowledgeable about the role of bacteria and plaque, and the importance of maintaining good oral hygiene, such as brushing twice a day, flossing daily, and having regular dentist visits – are all important in preventing tooth decay.

  • Eat fresh fruits as snacks instead of sugary snacks
  • Consider hidden sugars in processed foods and sauces

  • Replace toothbrushes or toothbrush heads every 3 – 4 months, or sooner if required (if bristles become frayed or broken)
  • Brush and scrape the tongue to effectively remove bacteria and for fresher breath

Using fluoride toothpaste and mouthwash

Fluoride-containing toothpaste and mouthwashes can help to strengthen tooth enamel and minimise cavities from forming. Toothpaste and mouthwash recommendations:

  • Use fluoride toothpaste with at minimum,350-1,500 parts per million (ppm) fluoride concentration9
  • Spit out excess toothpaste after brushing, without then rinsing the mouth with water – this allows the fluoride to remain on your teeth
  • Utilise a fluoride mouthwash, for further teeth protection

Dental check-ups and professional cleanings

Regularly visiting the dentist for check-ups and professional cleanings, are very important for maintaining good oral health.

  • Visit your dentist for a check-up at least once every six months
  • A dentist will examine your teeth and gums for signs of decay or gum disease.
  • Professional cleanings will properly remove plaque and tartar build-up that cannot simply be removed by just brushing and flossing

Cultural and socioeconomic factors

People from lower-income households may face challenges in accessing fresh fruits, vegetables, and other nutrient-rich foods as a result of financial constraints or limited availability.10

Cultural dietary choices can also impact oral health. The diets of some cultures might be high in sugars, carbohydrates, or acidic foods and drinks. Furthermore, some cultural practices (e.g., using traditional remedies for dental issues) may have detrimental effects on oral health. Understanding and addressing the impact of cultural dietary practices on oral health is important in creating culturally sensitive oral health education and interventions.11

Diet has a hugely important role in tooth decay formation and prevention. Eating sugary, acidic, and carbohydrate-dense foods can contribute to the formation of cavities and enamel erosion. More research is required to better understand the complex interactions and interplay between diet, oral health, and socioeconomic factors. Health initiatives should focus on making nutritious foods and dental care more accessible. Prioritising a healthy nutritious diet and good oral health habits will minimise the likelihood of tooth decay.
