Introduction

Tattooing is a widely practised form of body art with deep cultural significance across various societies. In some cultures, traditional tattooing methods involve tools made from materials such as bone, wood, or metal, playing a significant role in heritage and identity. For example, traditional Polynesian tattooing employs needle-sharp bones to create intricate designs deeply embedded in cultural expression. However, this article will focus specifically on modern tattooing practices in professional studios, where hygiene standards are crucial for client safety.

Just as in medical and surgical procedures, the use of improperly sterilised tattoo equipment poses serious health risks. Tattoo artists have a professional responsibility to adhere to strict health and safety regulations, ensuring proper sterilisation and infection control measures. This article aims to educate both tattoo professionals and clients on the risks associated with non-sterile equipment and the best practices for infection prevention, ultimately promoting safer tattooing experiences.

Health risks of non-sterile equipment

The use of unsterilised or contaminated tattooing equipment can lead to various health risks, ranging from mild skin infections to serious bloodborne diseases. Proper sterilisation and strict hygiene protocols are essential in minimising these risks.

Infections and cross-contamination

Using inadequately sterilised equipment or contaminated ink can expose clients to bacterial, fungal, and viral infections. Since 2000, bacterial outbreaks, especially those caused by non-tuberculous mycobacteria, have increased, leading to greater scrutiny of tattooing as a potential public health concern. Research has shown that insufficient hygiene practices are a major cause of these infections, while contaminated tattoo inks have also played a significant role, resulting in 11 documented outbreaks and subsequent ink recalls.1 A systematic review analysing cases from 1984 to 2015 found 67 instances of serious bacterial infections following tattoo procedures. Notably, both opened and unopened tattoo ink bottles frequently contained clinically significant bacteria, suggesting that contamination can originate from the manufacturing process itself.2 Another study focusing on viral infections associated with tattooing identified herpes simplex virus, human papillomavirus, and molluscum contagiosum as the most frequently reported tattoo-related viral complications.3 Even when equipment is properly sterilised, infections can still occur if tattoos are performed in unsanitary environments, such as a client’s home, where hygiene controls are often inadequate.4

Bloodborne pathogen transmission

On the other hand, there is a risk associated with using needles that have been previously used on other clients, and tools that are either not properly sterilised or not changed in the case of being single-use. The risks associated with this are infection with bloodborne pathogen transmission. Mainly viral diseases, including Hepatitis B, Hepatitis C, HIV, and other diseases involving other bloodborne pathogens, such as parasites.5 Under optimal biosafety conditions, the risk of transmission becomes very low,5 but it is crucial for tattoo artists to adhere strictly to these guidelines at all times.

Delayed healing and medical complications

Beyond infections, the use of non-sterile equipment can lead to complications such as allergic reactions, scarring, and, in extreme cases, tattoo rejection. While these issues are often linked to individual skin sensitivities or allergies to ink ingredients, improper hygiene practices can exacerbate the risks. Maintaining sterile conditions during the tattooing process helps promote faster healing and reduces the likelihood of medical complications.

Sterilisation protocols in tattooing

Maintaining a clean and sterile tattooing environment requires adherence to several best practices. These include:

  • Single-use vs. reusable equipment: disposable needles, ink caps, and gloves should always be single-use and properly discarded after each client. Reusable tools must undergo thorough sterilisation before each use to prevent cross-contamination
  • Proper sterilisation techniques:
    • Autoclaving for reusable equipment: tattoo needles, piercing equipment, and reusable devices, using high-pressure steam
    • Use of hospital-grade disinfectants: can be used on surfaces to clean countertops, furniture, and workstations
    • Items such as plastic wraps and disposable covers should be used on tattoo machines, light switches, and work surfaces to prevent contamination
  • Safe handling and disposal: preventing cross-contamination through proper waste management of single-use and reusable materials. Used needles and other biohazardous waste must be disposed of in designated sharps containers

In addition to sterilising tattooing tools and equipment, a tattoo artist must use personal protective equipment (PPE) to safeguard both themselves and their clients. Essential PPE  includes: 

  • Disposable gloves should be replaced between clients, or if it is damaged during the tattooing session, or if the session is too long
  • Face masks
  • Gown or protective aprons can provide a physical barrier against potentially infectious materials, and depending on the material of the apron, can protect the tattoo artist from splatter with biological material, particularly blood from the customer
  • Eye protection shields the eyes from splashes of ink, disinfectants, or bodily fluids
  • Closed-toe footwear can offer protection from any dripping liquid or fluid

Regulatory standards and best practices

To ensure the highest safety standards, tattoo studios must comply with local and international health regulations. In the United Kingdom, for example, the "Tattooing and Body Piercing Guidance Toolkit" was developed as an official resource endorsed by the Chartered Institute of Environmental Health, the UK Health Security Agency, and other industry professionals. This document provides essential guidelines to promote safe tattooing practices nationwide.

Additionally, health department regulations require tattoo studios to obtain proper licensing and undergo regular inspections to ensure they meet health and safety standards. These regulations are designed to protect both clients and artists by enforcing strict hygiene practices, proper sterilisation procedures, and safe waste disposal methods. Regular inspections help identify any violations or risks, ensuring that studios maintain a clean and sanitary environment.

Ensuring client and artist safety

Getting a tattoo is an exciting and personal experience, but safety should always be a top priority for both clients and artists. Proper hygiene, sterilisation, and infection control practices are essential to prevent health risks such as infections, allergic reactions, and bloodborne diseases. Here’s how clients can ensure safe practices and the responsibilities that tattoo artists and studios must uphold.

How tattoo clients can verify safe practices

Before committing to a tattoo, clients should take the time to research and observe the safety measures followed by their chosen artist and studio. 

Here’s what to look for:

  • Choosing a licensed, reputable tattoo artist
  • Observing sterilisation methods and the use of disposable equipment. You can ask about the artist’s training in bloodborne pathogens and infection control
  • Ensure the artist washes their hands and wears new gloves before starting
  • Watch for proper sterilisation techniques, such as the use of autoclaves to clean non-disposable equipment
  • Needles, ink caps, and gloves should always be single-use and disposed of after each client
  • The workstation should be covered with protective barriers that are changed between clients
  • A responsible artist should provide clear aftercare instructions to prevent infection and promote healing
  • The studio should have readily available antiseptics, bandages, and medical-grade cleaning solutions

Responsibilities of tattoo artists and studios

Tattoo professionals must maintain a clean and safe environment. This involves:

  • Continuous education on infection control
  • Implementing strict hygiene protocols by disinfecting all surfaces, chairs, and equipment between clients
  • Using barrier films on high-contact surfaces like tattoo machines, light switches, and power supplies
  • Storing tattoo inks, needles, and medical supplies in a sanitary manner to prevent contamination
  • Following local regulations for biohazard waste disposal, including the proper disposal of used needles and contaminated materials

By prioritising safety, both clients and tattoo professionals can ensure a positive and risk-free tattooing experience. Whether you're getting inked or providing the service, maintaining high hygiene standards protects everyone involved.
