Irritable bowel syndrome (IBS) represents one of the most commonly diagnosed gastrointestinal diseases. In the absence of any other causative disease, IBS can be defined as the presence of abdominal pain or discomfort with altered bowel habits. It is estimated that between 5% and 10% of people are affected by IBS at some point in time and, in most cases, this condition reoccurs.1

In the past the importance of gut health was often underestimated. However, today as more studies are being carried out the results and information gained showed us how essential it is to maintain an optimal gut health. It is estimated that there are more trillions of bacteria in the digestive system that are involved in the normal functioning of the brain, immune function, and endocrine pathways.2 Hence, gut problems don’t result only in digestive issues but can have severe consequences on the normal activity of the whole organism.
Role of probiotics in managing IBS symptoms
As probiotics have become a hot topic right now both in the scientific community but also on social media it is fair to ask how effective they actually are to treat IBS associated symptoms. Looking at the current studies a definitive, yes or no answer cannot yet be given regarding their efficacy. However, depending on the number and types of bacteria used, as well as the particular circumstances of the person affected by IBS, probiotics can contribute to alleviating the symptoms.3

The disorder can be debilitating in some patients, while others may have mild or moderate symptoms. The most important risk factors are found to be female sex, younger age and previous gastrointestinal infections. The most common IBS symptoms reported are abdominal pain or discomfort, stool irregularities and bloating, as well as other somatic, visceral and psychiatric comorbidities.5

Depending on how severe the symptoms are, IBS can significantly affect, in some cases, the quality of life of affected individuals. Those suffering from IBS might have to avoid certain activities or have to take more days off the non-affected individuals. Moreover, avoiding certain social interactions and deciding to eat alone to avoid the fear of being judged, along with the gastrointestinal symptoms can also lead to depression, anxiety, and/or panic disorder.6

The triggers for IBS symptoms can differ as there is not a universal list of triggers that can be avoided to prevent or lessen the symptoms. However, for many people eating or drinking certain foods or beverages can make the symptoms worse. Foods rich in carbohydrates, as well as fatty food, coffee, alcohol and hot spices, were most frequently reported to cause symptoms.7 Stress also plays an important role. Most people with IBS experience worse or more frequent symptoms during periods of increased stress.

This effect can be observed in the increased number of people suffering from IBS in countries where there is high consumption of junk food as well as low-fibre diets.10

Probiotics can be defined as live non-harmful microorganisms that are administered to improve microbial balance, particularly in the gastrointestinal tract. If we were to classify them, two main groups would be Saccharomyces boulardii yeast and lactic acid bacteria ( Lactobacillus and Bifidobacterium species), which can be found as part of dietary supplements or naturally in some foods. Their positive effects occur through a series of mechanisms that they are involved in. The way probiotics work is that they reduce the number of “bad” bacteria that can survive and grow in the gastrointestinal gut, they can help with lowering the intestinal pH as well as improving the host immune response .11
Sources of probiotics: food vs. supplements
As mentioned previously there are two main sources of probiotics, natural, from specific types of food, or synthetic, from supplements. A high source of probiotics is represented by fermented foods. Thus, if you want to help your gut you should consider consuming more yoghurt, cheese, and kimchi pickles as well as drinking kombucha or unfiltered apple cider vinegar12 If fermented foods are not necessarily your cup of tea, you should know that probiotics can be found in some unfermented foods such as cereals, juices and smoothies, and milk. It is also important to mention that the amount of probiotics you can get by consuming these types of foods can vary based on when you eat them and whether or not they survive the intestinal transit.

Modifying one's diet by avoiding trigger foods such as certain carbohydrates (FODMAPs), caffeine, and fatty foods can help alleviate IBS symptoms.18 Practicing stress management techniques such as mindfulness, meditation, and deep breathing exercises can help reduce stress levels, which may in turn improve gut health and alleviate IBS symptoms.19 Regular exercise and adequate sleep play vital roles in maintaining overall health, including gut health. Engaging in regular physical activity and ensuring sufficient sleep can help manage IBS symptoms.20
