Signs and symptoms
Tonsillitis can affect individuals of any age, but it most commonly affects young children and early adolescents.1 The main symptoms of tonsillitis are as follows:
- Sore throat
- Red, swollen tonsils
- Yellow/ white patches on the tonsils
- Difficulty swallowing
- Fever
- Swollen lymph nodes
- Bad breath
- Headache
- Fatigue1
Causes of tonsillitis
Tonsillitis is most commonly caused by viral infections but can also result from bacterial infections and, in rare cases, fungal infections. The symptoms of viral and bacterial tonsillitis are often the same, except that bacterial infections often require medication to go away, whereas virally-caused tonsillitis tends to go away by itself. Group A Streptococcus is the main bacterial cause of the condition. While tonsillitis itself isn’t contagious, the germs responsible for it, such as bacteria and viruses, can spread from person to person. 1

Recurrent tonsillitis is defined as several episodes of tonsillitis happening within a year. Frequent exposure to pathogens, such as bacteria and viruses, can trigger repeated bouts of the condition. Those with weakened or compromised immune systems are especially vulnerable to recurrent infections, making them more prone to tonsillitis. Additionally, individuals with large or deeply pitted tonsils may be anatomically predisposed to recurring tonsillitis, as these structures can trap bacteria and increase the likelihood of infection.3

- Joints and muscles: Rheumatoid arthritis, Psoriatic arthritis and Sjögren’s syndrome
- Endocrine system: Addison’s disease, Hashimoto’s thyroiditis and Graves’s disease
- Nervous system: Multiple Sclerosis and Guillain-Barre syndrome
- Other: Myasthenia gravis, Type 1 diabetes and vasculitis6

When tonsils are infected, they produce IgA as part of the immune response. Normally, once an infection resolves, this IgA is filtered out by the kidneys. However, in cases of chronic infections and recurrent tonsillitis, excessive IgA production can occur. This excess IgA, once filtered by the kidneys, may contribute to the worsening of IgAN. The theory suggests that the accumulated IgA in the kidneys exacerbates inflammation and damage associated with IgAN.9

- Laboratory tests can also be carried out, specifically blood tests as they can detect abnormal antibody activity that is commonly associated with autoimmune diseases
- Imaging studies can visualise internal organs and structures, helping to identify inflammation, joint damage, or organ involvement
- Biopsies can also be done to identify changes in the tissue that could be linked to autoimmune conditions5
Diagnosing tonsillitis
Tonsillitis, including recurrent tonsillitis, is generally easier to diagnose compared to autoimmune diseases due to its distinct symptoms, such as inflammation and swelling of the tonsils. The primary diagnostic approach involves a detailed health history and a physical examination by a medical professional. A strep test may be performed to determine if the condition is caused by Group A Streptococcus bacteria or is more likely due to a viral infection, helping to refine the diagnosis.1
Treatment options
The standard treatments for recurrent tonsillitis depend on the severity, frequency, and underlying cause of the condition. When tonsillitis is caused by a bacterial infection, antibiotics are typically prescribed to eliminate the infection. In addition, pain relievers and anti-inflammatory medications are used to alleviate symptoms like throat pain and swelling. For cases of recurrent tonsillitis, surgical intervention through a tonsillectomy is often recommended. This procedure involves the surgical removal of the tonsils, particularly when frequent infections lead to complications such as abscesses, breathing difficulties, or sleep apnea.1
Currently, there is no direct cure for autoimmune diseases, but symptoms can be effectively managed. The management approach varies for each person due to differences in genetics and the specific autoimmune condition. Common management strategies include the following:
- Medications: these include immunosuppressants, corticosteroids, and non-steroidal anti-inflammatory drugs
- Lifestyle changes: diet, nutrition, exercise, and stress management techniques can reduce flare-ups
- Monitoring and regular checkups
- Preventing flare-ups: this can be done by medication adherence, preventing infection, and avoiding environmental triggers5
