The inflammation or infection of the mediastinum, an area in the chest that contains the heart and other vital structures, is called Mediastinitis, a life-threatening infection that immediately requires treatment.1

It occurs when pathogens (disease-causing organisms) infect the mediastinum.

Mediastinitis can be categorised into:

Causes and risk factors

Causes of mediastinitis

Most studies show that cardiac surgery is the most common cause of mediastinitis. Other causes may include:

  • Severe dental infections
  • Upper respiratory infections
  • Endoscopy
  • Tuberculosis (It is the least common cause of mediastinitis)

Risk factors of mediastinitis

Diabetes and HIV/AIDS can increase the risk of infections and, therefore, lead to mediastinitis. Other factors that increase the risk of developing mediastinitis include:

  • Obesity
  • Immunosuppression
  • Old age 
  • Prolonged sternotomy

Importance of early management

Early management of mediastinitis can help prevent further complications (such as sepsis and abscess formation). Moreover, early management is essential as delayed treatment can lead to:

  • High risk of mortality
  • Infection and inflammation of multiple vital organs
  • Development of chronic mediastinitis
  • Risk of spreading infection to the aorta
  • Long-term complications

Signs and symptoms

Mediastinitis presents with various signs and symptoms. The most common one with which a patient usually presents is a history of upper respiratory infection. Some of the other symptoms may include:

  • Severe chest pain
  • Fever
  • Shortness of breath
  • Redness and swelling around the chest area
  • Fatigue
  • Difficulty in swallowing (dysphagia)

Note: Early management and treatment are important to prevent a patient from further progression.

Management approaches

Management of mediastinitis is very important. The management approaches for mediastinitis patients include the following:

  • Antibiotics: The well-directed antibiotic therapy is very important in the successful treatment of mediastinitis. These antibiotics are typically given through an IV. The following antibiotics are used:
    • Cephalosporins1
    • Penicillins
    • Carbapenems
    • Metronidazole
    • Clindamycin
  • Surgery (It also plays an important role in managing mediastinitis)
  • Debridement (removal of dead or infected tissue to improve the remaining healthy tissue)
  • Care of wounds (proper care of wounds to promote healing)
  • Complications management (septic shock management and mediastinal abscess)

Surgical treatment

Surgical treatment plays an important part in managing mediastinitis. The following are the main surgical treatments used for mediastinitis:

  • Surgical Drainage:
    • Open Drainage  Involves surgical treatment of the mediastinum to identify and drain abscesses or collections of infected fluid. This may bear the opening of the sternum or fresh thoracotomy lacerations
    • Closed Drainage Systems: Placement of rain spouts ( similar to casket tubes) directly into infected areas to ensure nonstop drainage of pus-filled material
  • Debridement:
    • Necrosectomy:  junking of necrotic or devitalized tissue within the mediastinum. This helps reduce the bacterial cargo and promotes mending
    • Surgical drawing: Thorough irrigation and cleaning of the mediastinal depression to remove debris and infected material
  • Closure ways:
    • Primary Closure: In cases where the infection is localised and well-contained, the surgical incision may be closed primarily after acceptable drainage and debridement
    • Secondary Closure: If the infection is extensive or there are concerns about crack mending, the crack may be left open to heal by secondary intention, with a posterior delayed check
  • Muscle Flap Reconstruction:
    • Pectoralis Major Muscle: Used to cover and protect vital structures in the mediastinum, similar to the heart and major vessels, particularly in cases where sternum stability is compromised
  • Omental Flap Transposition:
    • Omental Flap: The omentum (an adipose apron- suchlike structure in the tummy) can be mobilised and transposed to the mediastinum to give fresh vascularized tissue content and promote crack mending
  • Fresh Procedures:
    • Sternotomy modification: Modification of former sternotomy injuries to improve drainage and remove infected material
    • Decortication: Discard stringy or seditious towels ( decortication) around the lungs or other mediastinal structures to improve lung expansion and help intermittent infection
  • Postoperative Care:
    • Very extreme monitoring in the postoperative period for signs of ongoing infection or complications
    • Continued antibiotic remedy as guided by culture results and clinical response
    • Rehabilitation and activity to optimise recovery and address functional issues

Note: The choice of surgical approach depends on the extent of the mediastinal infection.

Complications

If not early managed and treated, mediastinitis can lead to serious complications, as the mediastinum is an area in the chest that contains the heart and other vital structures. The following are the complications associated with mediastinitis:

  • Sepsis and septic shock (Mediastinitis can cause systemic infection, which can be life-threatening)
  • Pulmonary complications (such as pneumonia, respiratory distress, or pneumothorax) 
  • Cardiac complications (such as myocarditis, pericarditis, and pericardial effusion)
  • Impaired mobility 
  • Chronic pain 
  • Long-term morbidity

In conclusion, the effective management and treatment of mediastinitis requires a multidisciplinary approach involving:

  • Early detection and management
  • Aggressive surgical intervention 
  • Adapted antibiotic remedy
  • Thorough post-operative care

The primary ambitions for the management and treatment of mediastinitis include;

  • Conserving towel integrity
  • precluding complications similar to sepsis and organ dysfunction
  • eventually promoting patient recovery
  • long-term health

Close collaboration between surgeons, contagious complaint specialists, intensivists, and other healthcare professionals is important to achieving optimal outcomes for cases suffering from this serious condition.
