Introduction

Testicular trauma refers to injuries sustained by the testes due to external force, with blunt trauma being the most common cause.1 Minor testicular trauma includes mild contusions, bruising, or small hematomas (better term -bruises) that do not result in testicular rupture (too technical) or torsion/twisting. This type of injury often occurs due to sports-related accidents, direct blows, falls, or straddle injuries. While severe testicular trauma may require surgical intervention, minor injuries can often be managed conservatively/at home. Conservative management focuses on pain relief, reduction of swelling, and preventing complications while allowing natural healing. Given the significant physical and psychological impact of testicular injuries, proper management is crucial to maintaining reproductive and overall waterworks health.2 This paper discusses the conservative management approaches for minor testicular trauma, including clinical assessment, non-invasive treatment methods, and the importance of follow-up care to prevent complications.

Clinical presentation and diagnosis

Patients with minor testicular trauma typically present with localised pain, tenderness, swelling, and sometimes mild bruising. Pain may move to the lower abdomen or groin, and in some cases, patients report nausea and vomiting due to the response of the testicular nerves. While minor trauma does not usually cause significant internal damage, it is crucial to rule out more serious complications. Clinical assessment begins with a thorough patient history, focusing on the mechanism of injury, onset of symptoms, and any previous testicular conditions. A physical examination is then performed to assess the extent of swelling, change of colour, and tenderness. The absence of severe tenderness, loss of testicular contour, or an abnormal testicular position suggests that the injury is minor and can be managed conservatively.4

Diagnostic imaging plays a crucial role in differentiating minor injuries from severe cases. Scrotal ultrasound with Doppler imaging is the preferred method for evaluating testicular trauma. It helps identify testicular structure, the presence of bruises, and blood flow to rule out death of the organ due to twisting and interruption of the blood supply. A normal or mildly heterogeneous testicular parenchyma with intact tunica albuginea confirms that the trauma is minor - way too formal - must be in lay person's terms.5 Doppler ultrasound can also assess blood flow disturbances and detect small intratesticular hematomas or swelling. In cases where there’s blood in the urine or significant swelling is present, urine testing is conducted to rule out associated urinary tract injuries. Based on clinical findings and imaging results, conservative treatment is advised if there is no evidence of testicular rupture or compromised blood flow.

Conservative management approaches

The primary goal of conservative management in minor testicular trauma is to alleviate pain, reduce swelling, and promote natural healing while preventing complications. The initial approach involves rest, cold therapy, scrotal support, and nonsteroidal anti-inflammatory drugs (NSAIDs).

Immediate first aid and home care

Rest is a fundamental component of conservative management. Patients are advised to avoid strenuous physical activities, sports, and any activities that may exert additional pressure on the testicles. Immobilisation reduces the risk of exacerbating the injury and promotes faster recovery. Applying ice packs to the affected area helps in reducing swelling and numbing pain. Ice should be wrapped in a cloth and applied for 15–20 minutes every few hours during the initial 24–48 hours following injury.6 This method constricts blood vessels, minimising inflammation and preventing excessive swelling.

Scrotal support is another essential component of conservative management. Wearing snug-fitting underwear, such as athletic supporters or briefs, provides external support, minimises movement, and reduces discomfort.7 Elevation of the scrotum using a rolled towel while lying down can further reduce swelling and improve comfort. Over-the-counter NSAIDs, such as ibuprofen or naproxen, help in managing pain and inflammation. However, aspirin should be avoided due to its blood-thinning properties, which could increase the risk of bruising.8 In cases of significant discomfort, physicians may prescribe stronger analgesics.

Medical supervision and monitoring

Although minor testicular trauma does not usually require hospitalisation, close medical supervision is necessary to ensure proper healing. Patients are advised to monitor their symptoms for any signs of worsening pain, persistent swelling, or changes in testicular consistency.9 Follow-up examinations are recommended to evaluate progress and identify any delayed complications, such as hematoma enlargement or secondary infection. In some cases, follow-up scrotal ultrasound may be performed to assess resolution of hematomas and ensure normal blood flow.10

Anti-inflammatory medications may be prescribed if persistent swelling or discomfort is present. If there is any suspicion of secondary infection, antibiotics may be recommended, especially if symptoms such as fever, redness, or pus drainage develop. Epididymitis, an inflammatory condition that can arise as a secondary complication of testicular trauma, may require a short course of antibiotics. Patients are also advised to avoid sexual activity until symptoms resolve, as sexual arousal and ejaculation can exacerbate pain and discomfort.11

When to seek further medical attention

While most minor testicular injuries heal with conservative treatment, certain warning signs indicate the need for further medical evaluation. Increasing pain, persistent swelling, new onset of nausea and vomiting, or changes in testicular shape suggest possible complications. A firm, painful lump developing within the scrotum could indicate a growing hematoma or infection. Blood in urine or persistent pain during urination may suggest an associated urethral injury, necessitating further urological assessment (12). Any signs of testicular asymmetry, severe tenderness, or absent cremasteric reflex - jargon raise concerns about torsion, a urological emergency requiring immediate intervention. Patients who experience worsening symptoms despite conservative management should seek urgent care to rule out underlying complications.13

Preventive measures for minor testicular trauma

Preventing testicular trauma is an essential aspect of urological health, particularly for individuals engaged in sports or physical activities. Protective athletic gear, such as jockstraps and athletic cups, significantly reduces the risk of testicular injury.14 These protective devices are particularly important for contact sports, cycling, and activities with a high risk of blunt trauma. Proper fitting and high-impact resistance are key factors in selecting effective protective gear.

Safe practices in daily activities also play a role in reducing the risk of testicular trauma. Being mindful of surroundings, avoiding risky movements, and using caution while engaging in physical tasks helps prevent accidental injuries. Cyclists should ensure that bicycle seats are well-padded and properly adjusted to avoid excessive pressure on the scrotal region (15). Additionally, educating individuals about the importance of early symptom recognition and self-care measures ensures timely intervention in case of minor injuries.16
