Introduction

Have you ever wondered how oral surgeons are revolutionising their approach to procedures, making them less invasive and more patient-friendly?

In the past, oral surgical operations were linked to lengthy recovery periods, large incisions, and intrusive techniques. However, in recent years, a remarkable transformation has taken place in oral surgery, with a growing emphasis on minimally invasive techniques.

These innovative approaches aim to achieve surgical goals while minimising disruption to surrounding tissues, ultimately leading to quicker recovery times and improved patient outcomes. Let's delve into the realm of minimally invasive techniques in oral surgery and explore their significance in enhancing patient care and recovery.

What is minimally invasive oral surgery?

When we talk about minimally invasive oral surgery, we are talking about procedures that take place in the mouth and use specific techniques to ensure that only the targeted tissues are affected. These procedures aim to attain the needed surgical result with minimal discomfort (meaning as little pain as possible post-surgery), reduced scarring, and a quicker recovery period as opposed to when using standard traditional surgical methods.1,2

These surgeries often involve small cuts and the application of certain advanced tools, such as endoscopes and lasers, which can help reach where necessary or treat the affected area without much destruction on other healthy tissues. 

Applications of minimally invasive techniques in oral surgery

Minimally invasive methods can be applied in different types of oral surgeries ranging from tooth removals to dental implants, gum surgeries, and even jaw operations. Whether it is a simple extraction or a complex reconstruction procedure, a minimally invasive approach is an option. Following are some of the conditions where minimally invasive surgeries are proven beneficial.

  • Tooth extractions
  • Gum surgery (gingivectomy/gingivoplasty)
  • Dental implant placement
  • Jaw surgery (orthognathic surgery)3
  • Treatment of oral lesions (biopsy and excision)
  • Management of temporomandibular joint disorders (TMJ)4

Types of minimally invasive techniques

Laser surgery

Laser dentistry has completely transformed the field of oral surgery as a minimally invasive substitute for conventional surgical methods. Dental lasers produce extremely concentrated light beams that are useful for a number of procedures, such as periodontal therapy, soft tissue surgery, and even tooth whitening. Laser dentistry offers several benefits, such as less discomfort, quicker healing, less bleeding, and a decreased need for sutures.1

Piezosurgery

Piezosurgery is a specialised procedure that uses controlled and accurate ultrasonic vibrations to cut bone without causing damage to the surrounding soft tissues.5 With the use of this technology, oral surgeons may carry out operations including bone grafting, sinus lifts, and tooth extractions with more precision and less stress. Patients who undergo piezosurgery benefit from quicker recovery, less oedema, and less discomfort following surgery.

Computer-aided surgery

Cutting-edge 3D imaging and CAD/CAM (computer-assisted design/computer-assisted manufacturing) software work hand-in-hand with computer-navigated implant placement to ensure dental procedures are executed with uncanny precision. Instead of blindly delving into the surgery, oral surgeons craft a virtual representation of the patient’s mouth which unveils every intricacy from optimal implant position to ideal angulation and depth. This method pledges minimal invasion, promising less time in the dental chair whilst upholding superior rates of implant success without compromising on damage to neighbouring tissues.6 

Microabrasion

Microabrasion is a procedure that removes surface enamel defects and discolourations with minimal damage to the teeth using sharp, shiny tools. Instead, it works by physically scraping off the outer layer of the enamel using a high-speed drill and gritty paste.1

This technique is most effective on pesky fluorosis stains and small white spots on teeth when you have hypoplasia, which can help improve the look of certain teeth greatly without any major workup or invasive treatment. Typically microabrasion will be applied along with other minimally invasive procedures, such as bonding or bleaching. Optimal outcomes are achievable through this selective approach.

Platelet-rich plasma (PRP) therapy

PRP treatment – or platelet-rich plasma – is increasingly favoured in oral surgical procedures due to its ability to hasten tissue re-growth and repair. The procedure involves drawing a small quantity of the patient's blood to obtain platelets and growth factors, which are later processed and reinjected into the surgical site as PRP. This approach significantly enhances the body's natural capability for tissue repair, and curbs inflammation effectually, while expediting the pace of healing post-surgery.7

Resin infiltration

The therapy for non-cavitated or incipient carious lesions - particularly those that impact the enamel - calls for resin infiltration. In simpler terms, it is about introducing a low-viscosity resin into the structure of porous enamel.

This act not only halts the growth of the lesion but also ensures an aesthetic outcome by closing off gaps in the enamel and discolourations caused by carious lesions. The resin infiltrant works by strengthening the structural integrity of enamel,  which in turn helps prevent the destruction of good tooth structures while stopping the advancement of early caries.8

Resin infiltration alone cannot offer comprehensive treatment for dental cavities; however, it is often combined with other minimally invasive restorative procedures to ensure that all aspects are addressed in totality.

Bleaching

One common approach, widely accepted due to its minimal invasion in treating tooth discolouration and enhancing smile aesthetics, is tooth whitening. This procedure is typically done using bleach chemicals (hydrogen peroxide or carbamide peroxide) that ensure the removal of both intrinsic and extrinsic stains from the tooth structure. 

Bleaching can be conducted at home through specific trays plus gels, or professionally in an office setting under guidance because it doesn’t compromise intrusive treatments yet plays a significant role in augmenting brightness as well as hue levels within the teeth. 

Its primary beneficiaries are individuals with discoloured teeth resulting from factors, such as age, tobacco use, or certain dietary patterns. For comprehensive results, bleaching can be combined with other minimally invasive procedures like microabrasion or composite resin restorations. 

All in all, bleaching presents itself as a conservative and affordable means of achieving an improved smile with no major procedures involved.1

Benefits of minimally invasive techniques

The main benefits of minimally invasive techniques are:9

  • Reduced trauma to tissues
  • Faster recovery time
  • Lower risk of complications
  • Minimal scarring
  • Enhanced precision and accuracy
  • Patient satisfaction and better quality of life

Challenges and limitations

Minimally invasive oral surgery has its advantages: less post-operative pain, less swelling and bleeding, shorter hospitalisation stays, and a faster return to normal activities. However, these techniques are not applicable to all cases, as their effectiveness varies with the specific condition under consideration and the patient's overall health status. Some of the challenges include:1

  • The complexity of each individual case
  • Limitations in the availability of the materials and their durability
  • Higher cost and financial considerations
  • Limitations in the surgeon’s ability and skill

Summary

In recent years, oral surgery has moved dramatically to less invasive treatments. The departure is significant because previously these procedures were known by large cuts and long recovery periods. This strategic shift comes from an industry-wide development of operation success, minimisation of negative impact on surrounding tissues, and ultimately patient comfort.

Minimally invasive oral surgery - various operations carried out inside the mouth cavity - are all aimed at preserving healthy tissues while minimising unintentional harm. Such procedures offer advantages due to their ability to accomplish surgical objectives with less discomfort and scarring as well as faster recovery than traditional approaches.

Integration of state-of-the-art tools and technologies in surgical procedures is crucial for the adoption of minimally invasive approaches. Among these, lasers and endoscopes play a pivotal role as they enable targeted access for surgeons with minimal incisions - ensuring precision and efficiency at optimal levels. Laser surgery is widely used in oral minimally invasive procedures thanks to its use of high-energy light beams and due to its minimally invasive nature (low discomfort, quick healing, and minimal bleeding). 

Another notable technique in the realm of minimally invasive oral surgery is piezosurgery which involves using ultrasonic vibrations that are finely controlled to cut bone while minimising damage to surrounding soft tissue areas. This specific approach is particularly suitable for procedures, such as tooth extractions or bone grafting, where preservation of tissue integrity plays a critical role in determining patient outcome quality.

Another stride in oral surgery is computer-assisted surgery. By integrating CAD software with advanced imaging technologies, surgeons can pre-plan procedures down to the last detail, ensuring utmost precision during operation, and minimal complications post-surgery. This paves the way for successful treatment outcomes.

While these methods mark significant milestones within the realm of oral surgery, their adoption does face impediments. The appropriateness of patients, complexity of procedural requirements, and availability of specialised equipment sometimes stand in the way of the feasibility and effectiveness of minimally invasive techniques. Nevertheless, a continuous influx of innovation coupled with research diversifies the scope that these methods cater to - thereby promising more enhancements not only in patient care but also in results obtained through treatment.

Overall, the acceptance of minimally invasive procedures signifies a paradigm change in oral surgery, motivated by a desire to raise the standard of care generally and improve patient outcomes. These procedures can potentially revolutionise oral surgery by putting the patient's comfort, efficiency, and precision first. This will open up new avenues for treatment and recovery.
