Introduction

CHARGE syndrome is a rare and complex genetic condition. It is characterised by a group of congenital anomalies reflected in the acronym CHARGE:1,2

These represent the most commonly affected organ systems. Though its global prevalence varies, CHARGE syndrome is estimated to occur in approximately 1 in 8,500 to 10,000 live births, making it a condition of notable significance in paediatric and developmental medicine. 

The clinical features of CHARGE syndrome can range from severe, life-threatening abnormalities detectable at birth to more subtle signs that may only become apparent with time.1,2 

Most cases are associated with mutations in the CHD7 gene, which plays a crucial role in early embryonic development.3,4 Due to the multisystem nature of this disorder impacting auditory, visual, cardiac, craniofacial, endocrine, and neurodevelopmental systems, patients often require sustained and highly integrated care.5

In such a medically intricate landscape, a multidisciplinary team (MDT) approach is not just recommended, it is essential. Many children face global developmental delays, balance and motor coordination issues due to vestibular dysfunction and significant communication limitations.1,2 As such, developmental paediatricians are vital in setting and monitoring individualised care goals.5

Occupational and physical therapists help children gain independence in daily living and address fine and gross motor challenges. SLPs continue to support communication through speech development or AAC, particularly for those with combined vision and hearing impairments.3 Psychological evaluations, often led by child psychologists, assess cognitive functioning, learning styles, and behavioural issues, all of which are essential for educational planning.2

In the classroom, a customised ‘Individualised Education Plan’ (IEP) ensures that each child receives the support and accommodations they need. This may include assistive technology, mobility devices, or specialised teaching methods. Collaboration between educators, therapists, and medical providers is crucial to creating a consistent and effective support network.1

Equally essential is the emotional well-being of both the child and the family. Many caregivers of children with CHARGE experience chronic stress, isolation, or difficulty navigating complex healthcare systems. Social workers, care coordinators and family counsellors provide crucial support, helping families build resilience and gain access to resources.2 Comprehensive psychosocial care improves not only the child’s outcomes but also the long-term health and stability of the family unit.1,5

Coordinated long-term care and transition to adulthood

As children with CHARGE syndrome grow into adolescence and adulthood, their needs evolve considerably. Planning for this transition should begin in early adolescence, involving all members of the care team.4 Emerging medical concerns at this stage may include endocrine issues like growth hormone deficiency or delayed puberty, along with mental health conditions such as anxiety or depression.5 Fertility and sexual health counselling are also important, yet often overlooked, topics.3

Adolescent medicine specialists and adult providers experienced in complex congenital conditions are instrumental in managing this transition. A structured handoff from paediatric care to adult healthcare systems helps avoid disruptions in critical services, particularly in cardiology, audiology, and endocrinology.4

The role of the primary care provider becomes increasingly central in adulthood, overseeing routine health maintenance, coordinating specialist referrals, and supporting autonomy.5 Even when major medical issues are no longer active, consistent follow-up is essential to detect late-onset complications and ensure continued functional progress.3

A successful transition empowers the individual to actively participate in their own care and make informed decisions. The ultimate goal extends beyond survival to achieving autonomy, meaningful social participation and the best possible quality of life.1
