Introduction
Nocturia, a condition characterised by waking up during the night to urinate, is a common and often disruptive problem, particularly among older adults. The prevalence of nocturia increases with age, affecting an estimated 70-80% of people assigned male at birth (AMAB) over 60 years old.6 This condition significantly impacts quality of life by disrupting sleep, which can lead to daytime fatigue, decreased productivity, and an increased risk of accidents and falls.19
Understanding nocturia
Physiological mechanism
Nocturia involves the kidneys producing more urine at night or the bladder being unable to hold urine long enough during sleep. Normally, antidiuretic hormone (ADH) reduces urine production at night. However, this balance can be disrupted by various factors, including age-related changes, which may lead to increased nocturnal urine production.
Common causes
Several factors can cause nocturia, including excessive fluid intake before bedtime, certain medications (like diuretics), and various health conditions such as diabetes and heart failure. However, in people AMAB, prostate conditions are a particularly common cause.4

Overview of prostate anatomy and function
The prostate is a small gland in people AMAB that surrounds the urethra, the tube through which urine passes out of the body. It plays a crucial role in producing seminal fluid. As these people age, the prostate can enlarge or become inflamed, leading to urinary symptoms including nocturia.1
Prostate enlargement (benign prostatic hyperplasia - BPH)
BPH is a non-cancerous enlargement of the prostate gland, common in older people AMAB. This condition can compress the urethra, causing urinary symptoms such as increased frequency, urgency, and nocturia.16 Research shows that up to 50% of men in their 50s and 90% of people AMAB in their 80s may experience BPH.2
The mechanism by which BPH causes nocturia involves both increased bladder outlet resistance and changes in bladder function. The enlarged prostate can obstruct urine flow, leading to incomplete bladder emptying and residual urine, which in turn prompts more frequent urination, especially at night.17
Prostate inflammation (prostatitis)
Prostatitis refers to inflammation of the prostate gland and can be acute or chronic. Acute bacterial prostatitis is characterised by severe symptoms, including fever and chills, while chronic prostatitis may present with more subtle symptoms such as pelvic pain and urinary difficulties, including nocturia.14 Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is a common type that can significantly affect a person's quality of life by causing persistent discomfort and urinary issues.8
Prostate cancer
Prostate cancer is one of the most common cancers in people AMAB. It can cause urinary symptoms similar to those of BPH, including nocturia. Treatments for prostate cancer, such as surgery, radiation, and hormone therapy, can also affect urinary function and contribute to nocturia. For instance, radical prostatectomy, a common surgical treatment, can lead to urinary incontinence and frequent urination.22
Diagnosis and assessment
Medical history and symptom evaluation
A comprehensive medical history and symptom evaluation are essential in diagnosing nocturia and its underlying causes. Physicians will ask about the frequency and volume of nighttime urination, fluid intake, and other urinary symptoms. They may also inquire about related symptoms, such as pain or changes in urine flow, which can provide clues about prostate conditions.5
Diagnostic tests
To diagnose nocturia and its relation to prostate conditions, several tests may be conducted:
Urinalysis: This test can detect infections, blood, and other abnormalities in the urine.
Bladder Diary: Patients record their urination patterns, including timing and volume, which helps in identifying the severity and potential causes of nocturia.
Prostate-Specific Antigen (PSA) Test: Elevated PSA levels can indicate prostate enlargement, inflammation, or cancer.
Digital Rectal Exam (DRE): This physical exam allows the physician to feel the size and texture of the prostate gland.
Imaging Studies: Ultrasound or MRI may be used to visualise the prostate and urinary tract.
Treatment and management
Lifestyle and behavioural modifications
Lifestyle changes can significantly alleviate nocturia. Recommendations include reducing fluid intake before bedtime, limiting alcohol and caffeine consumption, and practising bladder training exercises. Maintaining a healthy weight and managing other health conditions, such as diabetes, can also help reduce nocturia symptoms.
Medications
Medications play a vital role in managing nocturia, particularly when related to prostate conditions. For BPH, alpha-blockers (such as tamsulosin) can relax the muscles of the prostate and bladder neck, improving urine flow and reducing nocturia. 5-alpha-reductase inhibitors (such as finasteride) can shrink the prostate over time, providing longer-term relief.23
For prostatitis, antibiotics are used to treat bacterial infections, while anti-inflammatory medications and alpha-blockers can help manage chronic prostatitis symptoms.14
Surgical and minimally invasive procedures
When medications are insufficient, surgical and minimally invasive procedures may be considered. Transurethral resection of the prostate (TURP) is a common surgical procedure for BPH that removes part of the prostate to relieve obstruction. Other options include laser therapy and newer minimally invasive techniques such as UroLift and Rezūm, which aim to reduce symptoms with less recovery time.
Managing underlying prostate conditions
Specific treatments for prostate conditions can also help manage nocturia. For instance, advanced prostate cancer may be treated with surgery, radiation, or hormone therapy, each of which has implications for urinary function. Managing these treatments effectively can help mitigate nocturia.

New medications and technologies are continually being developed to improve the management of nocturia and prostate conditions. For instance, combination therapies that use both alpha-blockers and 5-alpha-reductase inhibitors have shown promise in providing more comprehensive symptom relief for BPH.17 Innovations in minimally invasive procedures also offer less invasive options with faster recovery times and fewer complications.24
