What is nosophobia and cyberchondria?
Nosophobia is a type of health anxiety where someone lives in constant fear of getting a specific illness, such as cancer, even without any real medical evidence. Some signs are:
- Panicking over normal body functions like thinking a headache means a brain tumour or aneurysm, or mistaking a mole for skin cancer
- Often seeking constant reassurance from doctors, friends, or online platforms
- Avoiding places they associate with illness
- Excessive anxiety about developing a particular disease
Cyberchondria is similar, but it is driven by obsessive and persistent online searches for health information.1 Some signs are:
- Compulsive online searches for health information and increased anxiety after
- Spending excessive time researching diseases
- Avoiding medical care altogether or visiting too frequently
Both conditions create high levels of anxiety without any medical proof of illness. They are related to hypochondria (illness anxiety disorder [IAD]), another health anxiety, but it is slightly different:
- Nosophobia focuses on the fear of getting a specific disease
- Hypochondria involves general worry about having a range of illnesses
- Cyberchondria is triggered by excessive online searching of symptoms
Prevalence
Nosophobia:
- Increased during and after COVID-19
- Studies show that it is more prevalent among medical professionals who are just starting their career
- A study in Pakistan found that nearly 40% of doctors reported a fear of COVID-19, particularly affecting younger females and less experienced doctors2
Cyberchondria:
- Increasingly common, especially among students and young adults
- Prevalence varies between 23-56%3
- In India, 46% of adults showed signs during the pandemic3
- University students are particularly vulnerable due to heavy internet use and academic stress
- Medical students may search differently than non-medical students because of their training, but both groups are affected3
Understanding nosophobia
Common triggers
Nosophobia can be triggered by a number of factors:
- Genetic factors: nosophobia often overlaps with other anxiety-related conditions like phobias and panic disorders, which suggests that there is a shared genetic link. Studies show that people with close family members who have specific phobias are much more likely (up to 10 to 20 times more) to develop similar issues themselves. This indicated a genetic tendency towards intense fear responses, which in some cases can show up as nosophobia. Anxiety disorders in general are thought to be 30-44% heritable, meaning they can run in families. If a parent struggles with a phobia, a child might develop a related anxiety issue4
- Environmental influences: children who grow up with a parent who is frequently ill or anxious about their health can internalise those worries, leading them to develop similar beliefs and behaviours about illness. Being exposed to these patterns from a young age can increase the risk of developing health-related fears5
- Childhood experiences: personal experiences during childhood can also be significant. Being seriously ill as a child, caring for someone with a chronic illness, or losing a loved one to disease can leave a lasting impression and contribute to the development of nosophobia later in life
Psychological profile
People with nosophobia often show traits like perfectionism and neuroticism.
Perfectionists may obsessively monitor their health, constantly seeking reassurance, and misinterpreting normal symptoms as signs of something more serious, as their need for perfect health can cause this obsession. This can create a vicious cycle where anxiety and avoidance behaviours get worse. Research links perfectionism to many mental health issues, especially anxiety disorders.6
Neuroticism is another personality trait where someone is more prone to stress and worry. People high in neuroticism tend to fixate on health concerns and may overreact to normal bodily functions. Studies suggest that neuroticism or other personal characteristics can lead to an increase in health concerns and anxiety.7,8
A heightened sensitivity to bodily functions is a very common feature. Someone with nosophobia might interpret things like stomach aches or muscle tension as signs of cancer or chronic disease. While they may seek reassurance from doctors or ask to get frequent tests, this can often backfire as more medical attention will often increase their anxiety and reinforce the fear.8
Impact on daily life
Nosophobia can seriously affect everyday life. Having persistent paranoia about symptoms causes high levels of unnecessary stress and mental exhaustion, this ongoing stress can take a serious toll on mental health. It can also lead to many avoidance behaviours like staying away from hospitals, crowded places, or public transport out of fear of contracting certain viruses or the fear of being diagnosed with something serious. Some may even avoid reading articles about specific diseases to avoid triggering their anxiety. Over time, this fear and avoidance can start to affect their social life, often resulting in reduced social interactions and strained relationships.
A lesser-known aspect of nosophobia is fear triggered by smells, like cleaning products, smoke, or fragrances. Even when the exposure is far below harmful levels, some people may panic due to the belief that these scents carry toxic substances. Media coverage plays a big role here, when the news links common chemicals (like BPA in baby bottles or plastic toys) to health risks such as cancer, it can spark fear and avoidance, even if the actual danger is minimal.9
Understanding cyberchondria
What influences cyberchondria?
The internet has completely changed how we get health information. Majority of people, especially the younger generation, now resort to online sources before seeing a doctor. Doctors are no longer the primary source of health information since online sources are faster, easier, and do not involve waiting for weeks for an appointment. But while the internet is convenient, it also comes with disadvantages such as outdated information or misinformation. A study showed that university students often rely on forums and private clinic websites rather than official government sources. Those who use social media, magazines, or newspapers for health advice tend to have higher levels of cyberchondria.11
The younger population are especially vulnerable since they rely so heavily on social media platforms such as TikTok for health information. However, some may struggle with health literacy, and being able to understand and use reliable medical information and terminology. This makes it easier to fall for misleading or exaggerated content, which the media is known to do.11 Social media can further worsen health anxiety as it bombards readers with alarming health content that is not always accurate. Without the right knowledge to interpret what they are seeing, people can spiral into anxiety and try to self-diagnose.12
One explanation for this behaviour is the reassurance-seeking model. People with anxiety often look for comfort from others, or in the case of cyberchondria, from the internet. While researching symptoms online might calm anxiety in some people, others will continue to search, fueling more anxiety. This creates a never-ending cycle of worry and compulsive online health searches.10
Impact on daily life
Cyberchondria can take over someone’s daily routine, starting the day searching symptoms online and ending it feeling overwhelmed by anxiety over something minor, can deeply affect mental health. Since COVID-19, the amount of online health searching has increased due to people looking for updates on diseases aetiology, prevention, and treatment. While it might be helpful at first, this becomes a daily habit. The more you search, the more anxious and concerned you become, and that reassurance people look for rarely lasts long. The constant searching can interfere with daily life by taking up time meant for work, hobbies, or relationships. It can even cause people to avoid responsibilities, argue with loved ones, or overuse healthcare services. For some, this behaviour begins to look like a form of behavioural addiction, especially when it feels out of control. In more serious cases, it can worsen mental health conditions such as depression, further worsening overall quality of life.13 Research shows a clear link between cyberchondria and real-life struggles like struggling to concentrate, strained relationships, or unnecessary medical visits and tests.1
Internet’s role in health anxiety
Accessibility to health information
Why it’s helpful: the internet makes health information easy to access, fast, and available anytime. It helps people learn more, manage their health independently, and find support communities. It can also raise awareness and knowledge about conditions they may not have known about.
Why it’s a problem: not all online health information is accurate. Misinformation and complex medical language can cause confusion, panic, or lead to self-diagnosis. Many people rely too much on online sources, which can fuel anxiety, especially for those with low health literacy.
Misinformation and misinterpretation
Social media, while helpful for sharing health awareness, is also a hotspot for misinformation, especially post-COVID. Rumours, clickbait headlines, and biased content can mislead people, influencing their health choices in harmful ways. This is especially dangerous for younger audiences who may not know how to spot credible sources. The prevalence of misinformation has drastically increased which has become a threat to public health due to its damaging outcomes.14
Misinterpreting normal body functions is a key part of health anxiety. A study found that it is not just the thought itself, it is how people think about their thoughts, known as metacognition. They found that people with negative beliefs about their thinking, experience even higher anxiety levels. So, it is not just the misinterpretation, but also how out of control they feel that makes it worse. They concluded that metacognition was a stronger factor when it comes to health anxiety.15
Algorithm
Search engines and social media use algorithms that show you more of what you have already looked or searched up. So if you are anxious and constantly searching about illnesses, you will keep seeing more of that kind of content. This creates a filter bubble where people keep seeing content that supports their existing beliefs, limiting their exposure to different viewpoints or reliable alternatives. In terms of health, constantly being shown negative information can lead to doom scrolling, which has been linked to an increased stress anxiety, and even depression over time.16
Summary
While the internet offers a variety of valuable health-related information, it can also become a harmful space for vulnerable individuals, particularly those struggling with anxiety. Excessive or misguided use of online resources can significantly worsen conditions such as nosophobia and cyberchondria, both of which can severely impact mental health.
A major contributing factor is the inability to distinguish between credible medical sources and misleading information. Many individuals, especially those who are unfamiliar with medical terminology, are the most vulnerable to misinformation, which increases their vulnerability to false health claims. This highlights the urgent need to improve health literacy and guide users towards trustworthy resources such as Klarity, NHS, Word Health Organisation (WHO), Centers for Disease Control and Prevention (CDC), National Institute of Health (NIH), Mayo Clinic, PubMed and so on. These platforms are considered reliable because they base their information on scientific evidence and peer-reviewed research. In contrast, online forums, chatrooms, and product-driven websites are not reliable sources for health information due to their biased or outdated content, lacking professional validation. Medical knowledge evolves rapidly, making it essential to rely on current research. Academic databases like PubMed, JSTOR, and Google Scholar are excellent sources of up-to-date, peer-reviewed research which provides a solid foundation for accurate and reliable information.
To combat the negative effects of online misinformation, it’s important to teach individuals, especially the younger population, how to distinguish and recognise online credible sources. Addressing the growing public health concern of internet-induced anxiety requires a multidisciplinary approach consisting of healthcare professionals, educators, and technology developers to help create a safer and more informed digital health environment.
