Introduction 

Pregnancy is an amazing journey marked by profound physiological changes, it is orchestrated by the intricate relationship between the mother and developing fetus. Central to this process, the placenta plays an essential role in providing essential nutrients and oxygen to support fetal growth and development.1 The placenta serves as a lifeline. However, when the placenta fails to do what it's supposed to do, a condition known as placental insufficiency can happen. Placental insufficiency presents several challenges for both the mother's and fetus's health. In this article we will be exploring placental insufficiency, elucidating its diagnosis, treatment options and implications it poses.

What is placental insufficiency? 

  • Placental insufficiency occurs when the placenta is not able to deliver the necessary nutrients and oxygen to the growing fetus, hindering normal development2
  • This condition can arise due to various factors, including maternal health issues, problems with the placenta itself or pregnancy complications

Causes of placental insufficiency 

Placental insufficiency occurs when the placenta does not implant or grow properly in the uterus because it is damaged.3 Sometimes there is no cause for this condition but the risks of placental insufficiency are increased if a mother has any underlying medical conditions or other factors.

Maternal factors

  • Maternal health issues like hypertension, diabetes, autoimmune disorders or substance misuse can all compromise the normal function of the placenta and contribute to placental insufficiency3
  • The above conditions can all disrupt the delicate balance required for efficient nutrient and gas exchange between maternal and fetal circulation

Placental abnormalities

  • Abnormalities in the structure of the placenta or pathophysiological changes within the placenta itself,4 like
    • Placental infarctions: when blood supply to the placenta is interrupted, causing cells to die
    • Thrombosis: formation of blood clots in blood vessels
    • Inadequate villous development: the villus is responsible for facilitating the exchange of nutrients, oxygen, and waste products. When the villus is not developed properly these functions will not work

Pregnancy complications 

  • Complications like preeclampsia (high blood pressure), gestational diabetes, intrauterine infections (infection in the uterus), or chronic hypoxia (constantly low O2 saturation levels)5
  • These conditions impose additional stress on the placenta, exacerbating its compromised function and further endangering fetal well-being

Diagnosing placental insufficiency

  • Early detection of placental insufficiency is very important so that effective management strategies can be put into place to minimize the potential risk to both the mother and fetus
  • Healthcare providers will employ various diagnostic tools and tests to identify the condition

Ultrasound 

  • Ultrasounds are to this day the cornerstone in the assessment of fetal growth and placental health throughout a woman's pregnancy6
  • On-going ultrasound scans allow healthcare providers to monitor parameters like fetal biometry, amniotic fluid volume, and placental morphology6
  • Any abnormalities detected that deviate from the expected growth trajectory in the placental structure can be identified during scans

Doppler ultrasound 

  • Doppler ultrasound, specifically the assessment of blood flow velocity in the umbilical cord artery, uterine arteries, and fetal cerebral artery, provides very valuable insights into placental perfusion7
  • Abnormal doppler waveforms are characterized by increased resistance or altered velocity ratios, which may indicate impaired placental function and heightened risk

Biophysical profile (BBP)

  • A BBP combines an ultrasound evaluation with monitoring fetal heart rate to assess the well-being of the fetus 8
  • The test evaluates fetal movement, muscle tone, breathing movement, amniotic fluid volume, and fetal heart rate reactivity8
  • Abnormal BBP results may indicate placental insufficiency and the need for further intervention

Maternal blood test 

  • Maternal blood tests include measuring hormone levels or markers of placental function and can provide additional information into the health of the placenta and how it is functioning9
  • Abnormal levels or dysregulated ratios of these tests can signify placental dysfunction and warrant closer monitoring.

Fetal monitoring

  • Electronic fetal monitoring tracks the fetus's heart rate and uterine contractions during pregnancy and labour9
  • Deviations from normal patterns may suggest fetal distress due to placental insufficiency

Treatment and management 

  • Once placental insufficiency is diagnosed, healthcare providers can develop a treatment plan tailored to a patient's specific needs to manage the condition and mitigate potential risks

Treatment strategies include:

Monitoring 

  • Regular prenatal check-ups and monitoring are essential to track fetal growth, amniotic fluid levels and maternal health parameters
  • Frequent ultrasounds and fetal monitoring help healthcare providers assess the progression of placental insufficiency and adjust the treatment plan accordingly

Lifestyle modification 

  • Maintaining a healthy lifestyle is very important for managing placental insufficiency. Having a well-balanced diet, being adequately hydrated, regularly exercising, and avoiding the use of harmful substances like tobacco, alcohol and drugs10
  • Optimal maternal health promotes favourable pregnancy outcomes and mitigates any potential complications.

Medication 

  • There are cases where healthcare providers may prescribe medications to help improve the blood flow from the mother to the fetus's placenta to enhance fetal development and well-being11
  • These medications can include:
    • Vasodilators: medication used to lower high blood pressure
    • Anti-coagulants: medication used to prevent blood clots

Fetal surveillance 

  • In severe cases, healthcare providers can recommend intensive surveillance, such as non-stress tests or biophysical profiles performed more frequently
  • These tests will be done to help assess the well-being of the fetus and aid in the decision-making regarding delivery time

Delivery 

  • In cases where placental insufficiency is poses a significant risk to the health and well-being to the mother or fetus, early delivery may be recommended, even if this is before full-term (preterm birth)12
  • The timing and mode of delivery depends on various factors, including gestational age, severity of placental insufficiency and the well-being of the fetus.

Neonatal intensive care 

  • Newborns born to mothers with placenta insufficiency will require close monitoring and supportive care in the neonatal intensive care unit (NICU) 
  • Prompt identification and management of complications such as
    • Intrauterine growth restrictions (IUGR): poor fetal growth due to inadequate nutrient and oxygen supply13
    • Hypoglycemia: low blood sugar level 
    • Hypothermia: The body loses heat faster than it produces it optimizes neonatal outcomes

Potential complications 

If placental insufficiency is poorly managed or not treated it can lead to many complications

  • Intrauterine growth restrictions (IUGR): poor fetal growth due to inadequate nutrient and oxygen supply13
  • Preterm birth: early delivery to prevent further compromise
  • Fetal distress: lack of oxygen and nutrients can cause long-term health issues for the fetus 
  • Stillbirth: severe cases of placental insufficiency can increase the risk of stillbirth if not managed promptly and effectively
