Are you familiar with the psoas muscle? This essential muscle group is found deep within your abdomen1. This muscle is pivotal in ensuring your body operates seamlessly. The psoas muscle is a silent hero that stabilises your pelvis, supports your spine, and enables simple movements such as standing and walking. 

What is psoas syndrome?

Psoas Syndrome is not just a physical discomfort; it can lead to frustration and uncertainty. It disrupts your daily routine, making even the simplest tasks seem daunting. This condition reminds us that even the most overlooked muscles can significantly impact our overall well-being.

Don't worry—there is a solution to every problem. By learning more about Psoas Syndrome, its causes and symptoms, and actively seeking treatment options, you can regain control over your body and eliminate the pain that comes with it.

Why is it important to understand about psoas syndrome? How does it change the impact of knowing earlier?

Firstly, recognising the causes of Psoas Syndrome allows individuals to take proactive measures to prevent its onset. Individuals can modify their behaviours and lifestyle choices by understanding the factors contributing to this condition's development, such as muscle imbalances, poor posture, or repetitive movements. 

Secondly, Psoas Syndrome can present with a variety of symptoms, including lower back pain, hip pain, and groin discomfort. These may overlap with those of other musculoskeletal conditions. Therefore, recognising these symptoms and distinguishing them from those of other conditions is essential for accurate diagnosis by healthcare professionals.

The structure and function of the psoas muscle 

The psoas muscle1 is like a lifeline for your spine and pelvis. It helps to stabilise your spine when you stand up straight or bend forward. It's also in charge of lifting your leg towards your chest, during walking, running, or even climbing stairs.

You might not think about your psoas muscle much, but it's working hard behind the scenes to keep you moving smoothly and without pain. So next time you step or bend over to tie your shoes, thank your trusty psoas muscle for keeping you going!

What are the causes of psoas syndrome? 

  1. Overuse: This can happen when you repeatedly engage in activities that involve bending forward or lifting your legs, such as prolonged sitting, running, or cycling. Over time this can strain the psoas muscle, leading to discomfort and pain.
  2. Muscle Imbalance: where certain muscles are stronger or tighter than others, can put extra stress on the psoas muscle
  3. Poor Posture: Slouching or sitting with poor posture for extended periods can also contribute to Psoas Syndrome. Slouching puts pressure on your lower back and pelvis, straining the pelvic muscle over time. Improper ergonomics at work or during daily activities can worsen muscle imbalances, increasing the risk of Psoas Syndrome. 
  4. Trauma or Injury: Any sudden impact or trauma to the lower back or pelvis, such as a fall or car accident, can injure the psoas muscle and lead to Psoas Syndrome. Similarly, overstretching or tearing of the psoas muscle during physical activity or sports can cause acute pain and discomfort, which may develop into chronic Psoas Syndrome if not adequately treated.
  5. Repetitive Movements: Engaging in repetitive movements that strain the psoas muscle, such as excessive twisting or bending, can contribute to developing Psoas Syndrome. This is common in professions or activities requiring repetitive motions, such as dancing, gymnastics, or manual labour.
  6. Psychological Stress: Believe it or not, stress can also play a role in Psoas Syndrome. When you're stressed, your body tenses up, including the psoas muscle. Chronic stress can cause muscle tension and imbalances, increasing the risk of Psoas Syndrome.

What complaints are seen in patients with psoas syndrome?

  1. Lower Back Pain: One of the hallmark symptoms of Psoas Syndrome is lower back pain. This pain typically manifests on one side of the lower back and may range from a dull ache to sharp, stabbing sensations. The pain often worsens with specific movements, such as bending forward or lifting the knee towards the chest.
  2. Hip Pain: Psoas Syndrome can also cause discomfort or pain in the hip region. This pain may be felt deep within the hip joint or on the outer surface of the hip. It may worsen with hip movement activities, such as walking or climbing stairs. Symptoms include a dull ache or a sharp, stabbing sensation.
  3. Groin Pain: Another common symptom of Psoas Syndrome is groin pain. This pain typically radiates from the front of the hip joint towards the inner thigh and groin area. It may feel like a deep ache or a sharp, stabbing pain and can be exacerbated by activities that involve hip flexion, such as bending forward or lifting the knee.
  4. Pelvic Pain: Psoas Syndrome can also cause discomfort or pain in the pelvic region. This pain may be localised to the front of the pelvis or may radiate towards the lower abdomen. It may feel like a dull ache or a sharp, stabbing sensation and can be aggravated by movements that engage the psoas muscle, such as bending forward or lifting the leg.
  5. 5. Abdominal Discomfort: Some individuals with Psoas Syndrome may experience abdominal discomfort or pain. This pain may be felt deep within the abdomen and may be accompanied by a feeling of tightness or pressure. It can worsen with activities that engage the abdominal muscles, such as coughing or sneezing.
  6. Limited Range of Movement: Psoas Syndrome can lead to a reduced range of movement in the hip joint and lower back. Individuals may experience stiffness or tightness in these areas, making it difficult to perform certain movements, such as bending forward, lifting the knee, or rotating the hip.
  7. Postural Irregularities: Psoas Syndrome may also cause postural irregularities, such as an anterior pelvic tilt or excessive lumbar lordosis.2 These postural changes can alter the alignment of the spine and pelvis, further straining the pelvic muscle and exacerbating symptoms.
  8. Radiating Pain: In some cases, the pain associated with Psoas Syndrome may radiate beyond the lower back, hip, groin, and pelvic regions. It may travel down the leg, mimicking sciatica3 symptoms or radiate towards the lower abdomen or buttocks.

Diagnosis of psoas syndrome 

Diagnosing Psoas Syndrome requires a comprehensive evaluation. This includes considering the patient's symptoms, past medical conditions and risk factors physical examination findings, and sometimes additional diagnostic tests to see whether a patient has Psoas Syndrome and rule out other potential causes of symptoms. The full details of what happens when you see your doctor about symptoms that may be related to Psoas Syndrome to reach a diagnosis are outlined below.

  1. Medical History Assessment: The diagnostic process typically begins with a thorough medical history assessment by a healthcare provider. This involves discussing the patient's symptoms, including the nature, duration, and intensity of pain or discomfort, as well as any relevant medical conditions, previous injuries, or activities that may contribute to the development of Psoas Syndrome. The healthcare provider may also inquire about the patient's occupation, lifestyle factors, and recent changes in physical activity.
  2. Physical Examination: A comprehensive physical examination is essential for evaluating the patient's symptoms and identifying signs suggestive of Psoas Syndrome. During the examination, the healthcare provider may assess the patient's posture, range of motion, and muscle strength, paying particular attention to the lower back, hips, and pelvis. Pressing on the psoas muscle may reveal tenderness, tightness, or trigger points, areas of increased sensitivity or discomfort that can indicate muscle dysfunction.
  3. Reproducing symptoms Specific actions may be performed during the physical examination to reproduce or exacerbate symptoms associated with Psoas Syndrome. These typically involve stretching or contracting the psoas muscle in various positions to elicit pain or discomfort. For example, the Thomas test4 involves flexing5 one leg at the hip while the patient lies on their back, which can accentuate symptoms related to pelvic muscle tightness or dysfunction.
  4. Imaging Studies: In some cases, imaging studies such as magnetic resonance imaging (MRI) or computed tomography (CT) scans may be ordered to evaluate further the structures surrounding the psoas muscle and rule out other potential causes of symptoms, such as herniated discs, spinal stenosis, or hip joint pathology. These imaging modalities can provide detailed visualisation of the psoas muscle and adjacent structures, such as the lumbar spine, pelvis, and hip joints, helping to confirm the diagnosis of Psoas Syndrome and guide treatment planning.
  5. Alternative Diagnosis: When diagnosing Psoas Syndrome, it's essential to consider other potential causes of lower back, hip, or pelvic pain. Differential diagnoses may include sciatica, sacroiliac joint dysfunction6, hip labral tear7, and appendicitis8. A thorough evaluation, including a review of symptoms, physical examination findings, and diagnostic test results, can help differentiate Psoas Syndrome from other musculoskeletal or visceral conditions with similar presentations.

Treatment options

Conservative management

  • Rest and Activity Modification: In the acute phase the affected muscle and avoiding activities that exacerbate symptoms can help reduce pain and promote healing. Activity modification may involve avoiding prolonged sitting, repetitive movements, or high-impact activities that strain the psoas muscle.
  • Physical Therapy: Physical therapy addresses muscle imbalances, improves flexibility, and strengthens the surrounding musculature. It may include massage or myofascial release, and specific exercises to stretch and strengthen the psoas muscle and improve overall posture. 
  • Stretching and Strengthening Exercises: Targeted stretching and strengthening exercises can help alleviate tightness and weakness in the psoas muscle, improving flexibility and function. Typical exercises include hip flexor stretches, lunge variations, and core stabilisation exercises that engage the psoas muscle and surrounding hip flexors.
  • Pain Management: Over-the-counter pain medications, such as non-steroidal anti-inflammatory drugs (NSAIDs) or muscle relaxants, may be prescribed to alleviate pain and discomfort associated with Psoas Syndrome. However, these medications are typically used as add onsand should be used cautiously under the guidance of a healthcare provider.

Medical treatments

  • Trigger Point Injections: Trigger point injections involve the administration of a local anaesthetic or corticosteroid into trigger points or tender areas within the psoas muscle to relieve pain and muscle spasms. This procedure can provide temporary relief and facilitate participation in physical therapy exercises. 
  • Prolotherapy: Prolotherapy, also known as regenerative injection therapy, involves injecting a dextrose solution or other irritant substances into the injured or dysfunctional psoas muscle to stimulate tissue repair and regeneration. This treatment aims to strengthen the ligaments and tendons surrounding the psoas muscle, promoting long-term pain relief and improved function. 
  • Dry Needling: Dry needling is a therapeutic technique that involves the insertion of fine needles into trigger points or tight bands within the psoas muscle to release muscle tension and alleviate pain. This procedure targets myofascial trigger points and can help improve muscle flexibility and range of motion.

Surgical interventions

Surgical interventions such as psoas release or psoas tenotomy may be considered in rare cases of severe or refractory Psoas Syndrome that do not respond to conservative or interventional treatments9. These procedures involve surgically releasing or lengthening the psoas muscle to alleviate tension and reduce symptoms. However, surgery is typically reserved for cases where conservative measures have failed, and the benefits outweigh the risks.
