Tunnel vision triggered by stress is a cognitive phenomenon or thinking pattern of narrowed perceptual focus and attention with reduced span in response to high stress or pressure, causing fixation in a particular problem, concern or situation.1
When a person is under stress, emotions stemming from our vision and hearing are processed through the amygdala and hypothalamus for sympathetic nervous system (SNS) activation, which triggers fight-flight (stress) response. Heightened bodily responses arise, including increased heart rate, increased blood flow to the brain, muscle tension, plus elevated cortisol and adernaline (epinephrine) levels.
Neural pathways in the brain are disrupted in intensifying the hyperstimulation (hyperarousal) state. The perception and interpretation of incoming sensory information are distorted towards a selective stress-related stimuli amplification and prioritisation, with non-essential information filtering and neglection.2,3
As stress levels increase, intense and direct attentional focus will be diverted from peripheral stimuli towards the stressor, reinforcing tunnel vision.

- Sports performance: During races or competitions, athletes may experience tunnel vision, only intensely focused on winning a prize or a particular opponent, yet neglecting strategic opportunities or teammates' actions1
Tunnel vision during panic attacks
Tunnel vision during panic attacks is the narrowed and intense focus on perceived threats or internal sensations, with omission to the surroundings.
Overview of panic attacks and symptoms
During a panic attack, the body’s fight-or-flight response is activated associated with adrenaline (stress hormone) release, leading to a sudden feeling of intense fear or discomfort upon ordinary, non-threatening situations. It is often associated with a wide range of physical symptoms, such as chest pain, difficulty breathing, rapid heartbeat, nausea, sweating, and tingling or numbness among fingers or toes. Simultaneously, it may also cause extreme terror, choking sensation, derealisation, depersonalisation, and even fear of dying which can be very unpleasant and frightening.
Although frightening, these episodes are usually temporary, ranging from 5-20 minutes, but might last longer for some people (up to an hour).

This redirects blood flow away from the periphery towards the major muscles to promote physical action readiness. Visual processing, therefore, becomes focused on the central field, allowing for honing into perceived danger or threat.

Stress hormones, adrenaline and cortisol release are profound upon sensory processing during stressful or panic periods. Adrenaline (epinephrine) surge can sharpen sensory acuity, like vision and hearing, making individuals’ hyper-aroused to their surroundings for rapid detection and action towards perceived threats. Cortisol could also alter sensory perceptions through its disruptions to the hippocampus and prefrontal cortex (PFC) in decontextualising neutral contexts, meanwhile, inducing exaggerated emotional responses to benign sensations.7
Amygdala hyperstimulation
The amygdala, the brain’s emotional processing centre, contributes significantly to the occurrence of psychogenic tunnel vision. As the amygdala becomes highly stimulated due to cortisol increase during stress, this heightened emotional arousal could suppress activities of the prefrontal cortex (PFC), which is responsible for higher-order cognition, such as executive control.

Stress-induced tunnel vision makes people solely focus on their own perspectives while disregarding the viewpoint of others. The feedback loop of fight-or-flight response that perpetuates stress-induced tunnel vision and other anxiety or panic-related symptoms are interrupted.
Cognitive-behavioural therapy (CBT)
Cognitive-behavioural therapy (CBT), is a structured, goal-oriented psychotherapy targeting faulty or unhelpful thinking patterns and behaviours, and can be instrumental in addressing psychological causes of tunnel vision.10 Cognitive restructuring can identify and challenge distorted thinking patterns, contributing to heightened anxiety, panic, or perceived dangers.
Reframing of thoughts or internal narratives could facilitate individuals in interpreting their sensory input with a broader perspective more accurately, even during anxiety or panic attacks. Thus, lowering the occurrence of tunnel vision.11

- Mindfulness practices: Mindfulness practices, journaling, self-talk and positive affirmations can shift one’s focus and awareness from their preoccupied anxious thoughts back to the present reality, to promote here-and-now self-awareness and emotional resilience12
