The pulp, an extensively supplied and vascularized mass of connective tissue, is contained within a spaced compartment known as the pulp chamber in our teeth. The inflammation of this tissue is referred to as pulpitis.1 The primary roles of the pulp include the formation and nourishment of dentin, along with providing nerve supply and protective mechanisms for the tooth. The pulp plays a major role in determining the prognosis of the tooth. If the pulp chamber loses its structural integrity, the pulp is exposed to the risk of adverse stimuli from the mouth such as caries, cracks, fractures, and open restoration margins, all of which are possible routes for microorganisms and their toxins to enter the pulp. The pulp responds to perturbation by becoming inflamed, and if left untreated, this could lead to pulp necrosis and infection. The inflammation can spread to the surrounding alveolar bone, causing periapical pathosis.2 Timely and appropriate care and prevention are, therefore, crucial to avoid serious complications in the oral cavity.
Pain relief is a critical aspect of managing pulpitis. The inflammation and associated pain can be debilitating for patients, affecting their quality of life and daily activities. Providing effective pain relief not only alleviates the patient's suffering but also allows for proper diagnosis and treatment planning. Pain relief measures may include both pharmacological and non-pharmacological interventions, depending on the severity of the symptoms and the underlying cause of pulpitis.

Pulpitis, or inflammation of the pulp in human teeth, occurs most commonly as a result of dental caries that have penetrated the enamel and dentin and is usually accompanied by pain. Other potential causes of pulpitis in all species include bacterial penetration through pulp exposure due to attrition, abrasion, or trauma; chemical irritation (e.g., from irritant molecules directly applied to the pulp or by diffusion through adjacent dentin after insertion of restorative material); thermal damage (heat produced by motorised dental equipment); or iatrogenic pulpal exposure.3
Types of pulpitis
There are two main types of pulpitis depending on the extent of the infection: reversible and irreversible. Reversible pulpitis is characterised by the early stage of the infection. Because the swelling is moderate enough, the tooth can be saved using less intrusive procedures.4 In this case, the tooth may experience temporary sensitivity to stimuli such as hot, cold, or sweet foods, but the pain usually subsides once the stimulus is removed.
However, if the underlying cause of inflammation is not addressed promptly, reversible pulpitis can progress to irreversible pulpitis. Sensitivity and toothache are the primary symptoms of pulpitis. Irreversible pulpitis occurs in case of a more advanced inflammation when the tooth cannot recover since the damage is irreversible. The tooth may also become sensitive to temperature changes and pressure, making it difficult for the patient to chew or bite down. If left untreated, irreversible pulpitis can lead to pulp necrosis, abscess formation, and eventual tooth loss.5
Risk factors for pulpitis
Several risk factors contribute to the development of pulpitis. Firstly, tooth location plays a role, with decay more commonly occurring in the back teeth, such as molars and premolars. These teeth have intricate grooves, pits, and multiple roots that can trap food particles, making them harder to clean than front teeth. As a result, bacteria can accumulate, leading to inflammation of the pulp.6
Dietary habits also influence the risk of developing pulpitis. Foods that cling to teeth for prolonged periods, such as sugary snacks and beverages, provide fuel for bacteria to produce acids that attack tooth enamel. Similarly, frequent snacking or sipping on acidic drinks throughout the day creates a continual acid bath over the teeth, increasing the likelihood of pulp inflammation.6
Inappropriate feeding practices, particularly bedtime bottle feeding in infants, can contribute to pulpitis. When babies are given bottles containing sugary liquids like milk or juice before sleeping, these substances remain on their teeth for extended periods, promoting bacterial growth and tooth decay. Similarly, prolonged use of sippy cups filled with sugary drinks can have a similar effect, leading to what is commonly known as baby bottle tooth decay.6
Inadequate oral hygiene practices, such as infrequent brushing and flossing, allow plaque to accumulate on teeth, facilitating bacterial growth and the development of pulpitis. Additionally, factors like reduced fluoride exposure, either from insufficient fluoride in water or inadequate use of fluoride-containing dental products, can increase susceptibility to pulp inflammation.6
Certain medical conditions and medications can also elevate the risk of pulpitis. For instance, conditions that cause dry mouth, such as certain medications, medical treatments like radiation therapy, or systemic diseases, reduce saliva flow, impairing the mouth's natural defence mechanisms against bacterial infection. Moreover, worn dental fillings or devices can create spaces where bacteria can accumulate, leading to pulp inflammation underneath.6
Finally, lifestyle factors like heartburn caused by gastroesophageal reflux disease (GERD) and eating disorders such as bulimia can contribute to tooth erosion and cavities, increasing the risk of pulpitis. Stomach acid from frequent vomiting can erode tooth enamel, exposing the underlying dentin to bacterial attack. Understanding these risk factors is crucial for implementing preventive measures and promoting oral health to reduce the incidence of pulpitis.6
Clinical presentation and symptoms of pulpitis
Reversible ppulpitis:
- Sensitivity to colds or sweets that fade fast is frequently described as a sharp pain
- Absence of pain when the tooth is tapped by the dentist
- Absence of sensitivity to heat
Irreversible pulpitis:
- Pain when the tooth is tapped by the dentist
- Sensitivity to cold, heat or sweets that lingers. This may be an aching, throbbing or sharp shooting pain
Diagnosing pulpitis
There are several ways of diagnosing pulpitis. This is primarily done by determining the sensitivity of your tooth.4
- Electric pulp testing: A small electrical impulse is delivered to the tooth, and if the pulp is still alive, the patient will feel the stimulation. In contrast, if the pulp tissue is dead due to infection, no sensation will be experienced in response to the electrical pulse
- Heat or cold test: In this test, the tooth is exposed to either a hot or a cold substance. In the case of irreversible pulpitis, sensitivity to temperature changes persists for longer than a few seconds. If the pulp tissue is dead, the patient will not feel any sensation during the test
- Tooth tapping: By gently tapping on the tooth, dentists can assess the patient’s response to the stimulus. Pain or discomfort upon tapping is indicative of irreversible pulpitis, suggesting inflammation and damage to the pulp
- Radiographs: Dentists also rely on dental X-rays to further evaluate the tooth’s condition. X-rays enable dentists to visualise any defects in the tooth structure, such as decay or fractures, and identify signs of infection
Treatment options for pulpitis
Reversible pulpitis
Dentists can save a tooth affected by reversible pulpitis through minimally invasive procedures, which involve removing the decayed portion of the tooth followed by sealing with a normal filling. Symptoms associated with pain are typically relieved using over-the-counter medication. Additionally, a special numbing gel that penetrates the tooth and relieves the affected nerve may also be used.5
Irreversible pulpitis
During clinical testing, any tooth that exhibits a reaction longer than about 10 seconds is considered to have irreversible pulpitis and is indicated for root canal treatment. This involves the removal of the infected pulp and cleansing out the root. The empty root canal is then filled in order to seal the tooth. A crown is used later on to cap the tooth. In some cases, tooth extraction may be a viable option to consider, with several options for replacing the tooth, such as a dental bridge or a dental implant.5
Prevention of pulpitis
Maintaining good oral hygiene is an appropriate way to prevent pulpitis. This includes following good habits such as flossing every day and brushing your teeth twice a day. Seeing the dentist regularly for routine cleaning and check-ups is also essential to maintain optimum oral health. Wearing night guards during sleep to prevent the grinding of teeth is also an effective way to prevent trauma to the dental tissue. Consume foods that promote dental health. Certain foods and drinks are more beneficial for your teeth than others. Steer clear of items that tend to linger in the crevices and gaps of your teeth, or make sure to brush them shortly after consuming them. It is also of utmost importance to immediately report any new dental symptoms, such as discomfort and sensitivity to your dentist to salvage the situation.4
Pain relief strategies
In managing pulpitis and alleviating associated pain, dentists employ various pharmacological and non-pharmacological approaches.7 Pharmacological strategies often involve the use of analgesics such as nonsteroidal anti-inflammatory drugs (NSAIDs) and acetaminophen to reduce pain and inflammation. NSAIDs work by inhibiting prostaglandin synthesis, thereby reducing pain and inflammation, while acetaminophen primarily acts as a pain reliever. In cases where infection is present, antibiotics may be prescribed to address the underlying bacterial infection contributing to pulpitis.
Additionally, local anaesthetics and nerve blocks are commonly administered to numb the affected tooth and surrounding area, providing temporary pain relief during dental procedures. Adjunctive therapies such as cold compresses and elevating the head may also help alleviate the discomfort and swelling associated with pulpitis. These multidimensional approaches aim to effectively manage pain and promote patient comfort during treatment.7
