What is Filippi Syndrome?

Filippi syndrome is a very rare autosomal recessive genetic disease. It is also known as syndactyly type 1 (partial or complete union of the third and the fourth fingers or second and the third toes) with mental retardation and microcephaly (small head).1 Other notable features of this disorder include physical and developmental abnormalities, including craniofacial dysmorphisms, limb malformations, growth delays, and intellectual disabilities. Since it is an extremely rare condition only a few cases have been reported worldwide, so there is a significant challenge in gathering enough data or having enough insight about this syndrome. However, advancements in genetic research and clinical studies have provided much information about the cause, clinical symptoms, and complications associated with this syndrome.

Patients suffering from Filippi Syndrome are prone to frequent and severe respiratory complications. These can be due to the syndrome affecting the craniofacial structures, airway anatomy, and neuromuscular control.

What are the respiratory complications associated with Filippi syndrome?

Understanding respiratory complications in Filippi Syndrome:

The complications affecting the respiratory system can be due to a majority of factors. This is because of the anatomical and physiological abnormalities associated with this syndrome. These complications can impact the patient’s quality and overall prognosis of the affected individuals.

Craniofacial abnormalities in Filippi Syndrome are characterised by distinct craniofacial abnormalities, including microcephaly (a smaller-than-normal head), hypertelorism (widely spaced eyes), a broad nasal bridge, and a small mandible (micrognathia).2 These structural abnormalities can lead to airway obstruction, particularly during sleep, increasing the risk of obstructive sleep apnea (OSA). Micrognathia, for example, can result in a smaller oral cavity and a retracted tongue, both of which contribute to upper airway obstruction. Laryngeal and tracheal abnormalities are seen in some patients with Filippi Syndrome who may have laryngeal and tracheal malformations that contribute to respiratory complications. These abnormalities can include a narrow trachea (tracheal stenosis) or a floppy larynx (laryngomalacia), which can lead to breathing difficulties, stridor (a high-pitched breathing sound), and an increased risk of respiratory infections.

Filippi Syndrome often involves developmental delays and intellectual disabilities, which can be associated with neuromuscular weakness. Neuromuscular weakness can impair the ability to cough effectively, clear secretions, and maintain proper respiratory function, leading to an increased risk of pneumonia and other respiratory infections.  Several patients with Filippi Syndrome experience difficulties with swallowing (dysphagia) due to the syndrome’s impact on craniofacial structures and neuromuscular control.3 Dysphagia can result in the aspiration of food and liquids into the lungs, leading to aspiration pneumonia—a severe and potentially life-threatening respiratory complication.

Due to the anatomical and physiological abnormalities associated with Filippi Syndrome, affected individuals are at an increased risk of recurrent respiratory infections. These infections can be bacterial or viral and may require frequent medical interventions, including antibiotic therapy and hospitalisation.

What is the prevalence and impact of respiratory complications?

Given the rarity of Filippi Syndrome, not much information on the prevalence of respiratory complications within a particular population is available. However, respiratory complications are known to be a significant concern for many individuals with craniofacial syndromes, which share some features with Filippi Syndrome.

The impact of respiratory complications on individuals with Filippi Syndrome is very significant and profound. These complications could lead to frequent hospitalisations, the need for surgical interventions (such as tracheostomy or corrective jaw surgery), and reduce the quality of life. In severe cases, respiratory complications can be life-threatening, particularly if they lead to chronic hypoxemia (low blood oxygen levels) or respiratory failure.

How are the respiratory complications manifested clinically?

The complications could manifest as several symptoms. OSA or obstructive sleep apnea could be associated with patients suffering from complications in Filippi syndrome. The patient may have a disturbed sleep pattern, heavy snoring or gasping, and choking during sleep. Another major sign is stridor, which is a high-pitched wheezing sound that occurs from the lungs due to some sort of airway obstruction. In complications associated with Filippo syndrome, the stridor may develop later in life or could be present from birth. This could only be determined by the severity of the laryngeal or tracheal abnormalities. Any disease involving the lungs such as cough has the major symptoms. Here, a persistent chronic type of cough could be present. It could be dry or wet (mucus). Pneumonia can also develop. Aspiration pneumonia is when food, liquid, or any other substance gets inhaled into the lungs. Patients with pneumonia may present with fever, cough, difficulty in breathing, and chest pain. Respiratory complications can contribute to poor growth and development in younger patients with Filippi Syndrome, particularly if they lead to chronic hypoxemia or frequent hospitalisations. Failure to thrive is when children have inadequate weight gain and growth, which can be a consequence of both respiratory and feeding difficulties.

How are the respiratory complications diagnosed?

Diagnosing respiratory complications in patients suffering from Filippi Syndrome may need a multidisciplinary approach including paediatricians, pulmonologists, otolaryngologists (ENT specialists), speech and language therapists, and geneticists.

Initially, patients are thoroughly examined by the physician. A detailed medical history is taken and any known symptoms associated with the disease are noted. Physical examination may help examine the presence of airway obstruction, such as retractions (inward movement of the chest wall during breathing), wheezing, or abnormal breath sounds.

Chest X-rays are the first choice of investigation. They can show signs of pneumonia, atelectasis (lung collapse), or structural abnormalities in the chest and airways. A computed tomography (CT) scan provides detailed cross-sectional images of the chest and airways. This can help investigate tracheal stenosis, lung abnormalities, or other structural issues. Magnetic resonance imaging (MRI) may be used to assess soft tissue structures in the neck and upper airway, particularly if laryngeal or pharyngeal abnormalities are suspected.

Polysomnography is a sleep study and is the gold standard for diagnosing obstructive sleep apnea. Sleep studies are particularly important for individuals with Filippi Syndrome who present with symptoms of OSA. Pulmonary function tests (PFTs) are used to evaluate the function of the lung and determine the presence of restrictive or obstructive lung disease. PFTs may include spirometry, which measures the volume of air exhaled after taking a deep breath, and other tests that investigate lung volumes, airway resistance, and gas exchange. Bronchoscopy is an invasive procedure that allows direct visualisation of the airways using a thin, flexible tube with a camera (bronchoscope). It can be used to diagnose tracheal or bronchial stenosis, laryngomalacia, or other airway abnormalities. Bronchoscopy may also be used to obtain tissue samples (biopsies) for histopathological analysis or to remove foreign bodies or secretions from the airways.

How are the complications managed and treated?

Medications are required to treat infection, reduce inflammation and pain, and manage other symptoms. Antibiotics are employed for bacterial infections that could cause pneumonia or bronchitis. Corticosteroids are used to decrease the inflammation of the airways and improve breathing in patients who have stenosis. Bronchodilator drugs relax the muscles around the airways to improve breathing in case of asthma or obstructive lung conditions.

Surgeries are employed in case of severe airway obstruction and structural abnormalities that could reduce breathing significantly and are only used when medical therapy does not work. Tracheostomy is indicated in patients with severe OSA or tracheal stenosis. Corrective jaw surgeries are performed to treat micrognathia. Laryngotracheal reconstruction is to open and widen the trachea or larynx to improve airflow.

Supportive treatment may include respiratory therapy in patients to improve lung function and increase clearance. Chest physiotherapy, postural drainage, and the use of positive airway pressure (PAP) devices are employed to improve the symptoms. Speech therapists help patients with swallowing difficulties and provide exercises to improve their function to reduce the risk of aspiration pneumonia. Nutrition and dietitian support for children who fail to thrive is important to prevent malnutrition. Regular monitoring and follow-up are essential for patients suffering to catch the complications early and prevent them.

Respiratory complications are a significant concern for individuals with Filippi Syndrome, a rare genetic disorder characterised by craniofacial abnormalities, neuromuscular weakness, and developmental delays. These complications can arise from a variety of factors, including airway obstruction, laryngeal and tracheal abnormalities, and swallowing difficulties. Early diagnosis and a comprehensive, multidisciplinary approach to management are essential for improving outcomes and enhancing the quality of life for individuals with Filippi Syndrome.
