Definition of Salmonella infection 

Salmonella infection, also known as Salmonellosis, is a prevalent bacterial illness primarily affecting the intestinal tract. It is caused by the bacteria known as Salmonella. The primary mode of transmission is through the consumption of contaminated food or water. The majority of patients afflicted with this infection often experience diarrhoea, which may include bloody stools, accompanied by fever. Occasionally, patients may exhibit symptoms such as nausea, vomiting, malaise, and headache.

Vulnerability of older adults to infections

The ageing process is an inherent and unavoidable aspect of life which presents important health-related challenges. It is caused by the accumulation of unrepaired damage to cells and molecules over time, which usually leads to a gradual reduction in physical activity. This increases the risk of diseases. Additionally, the presence of other diseases such as diabetes, hypertension, heart disease etc increases susceptibility to infections.1

Importance of understanding salmonella infections in older adults: 

Salmonella infection being a foodborne infection is of global importance. Older individuals are especially at risk of Salmonella infection as it can lead to severe complications and increased mortality rates2.

Factors contributing to increased risk in older adults 

Age-related changes in immune function

With an increase in age, the immune system undergoes dramatic changes leading to abnormal immune system function. These kinds of changes in the inbuilt immune system lead to an altered ability of the immune cells to destroy foreign bodies or infections. All these factors together impact the ability of an individual to effectively fight infections.3.The incidence of inflammatory diseases, cardiovascular diseases, diabetes, and cancers are higher in older individuals. Therefore, ageing is associated with decreased immunity along with an increasing incidence of inflammatory diseases.

Prevalence of underlying health conditions

Underlying health conditions also play a vital role in the efficient function of the human immune system. For example, diabetes mellitus affects about 422 million people globally and increased sugar levels are known to indirectly affect the immune system by impairing the function of the immune system. Cancer affecting millions of people worldwide is the most common chronic disease. Cancer patients have suppressed immune systems and hence are highly susceptible to infections.4

Clinical presentation of salmonella infections in older adults

Typically the incubation period(the time between infection and manifestation of symptoms) of Salmonella infection ranges between 7 days to 14 days. Common symptoms of this infection are fever, diarrhoea, vomiting, nausea and headache. In young adults, the disease is less severe however, in older people the infection persists for longer periods causing dehydration and in some cases, the disease is invasive where the infection reaches the bloodstream causing septicemia. The most effective method of diagnosing the disease involves assessing clinical symptoms and conducting blood tests to identify specific types. Culture of stool, pus and blood is another way to confirm infection. Today, there are also advanced techniques to confirm the disease.5

Prevention strategies

Since Salmonella infection is caused by contaminated food or water, it can prevented by employing the following strategies:

  • wash your hands thoroughly with soap before handling food items and water
  • keep the kitchen table tops and refrigerators clean
  • avoid drinking unpasteurized milk or eat raw eggs
  • cook poultry products and meat well for consumption

Older adults are particularly vulnerable due to age-related changes in immune function and the prevalence of underlying health conditions like diabetes and cancer. This infection can lead to severe complications and increased mortality rates in this population. Symptoms include fever, diarrhoea, vomiting, nausea, and headache, with the potential for dehydration and spread to the bloodstream. Diagnosis involves assessing clinical symptoms and conducting tests, while prevention strategies include handwashing, maintaining cleanliness in the kitchen, avoiding unpasteurised milk and raw eggs, and ensuring thorough cooking of poultry and meat.
