Tetanus is a potentially fatal infectious disease caused by bacteria, found in soil, street dust, and other environmental sources. The bacterium enters the body through wounds such as cuts, tears, bites, bruises, or even minor scratches. Once inside the body, the bacterium multiplies and releases a toxic substance known as a toxin, leading to the development of the disease.

Clostridium tetani spores are present in the environment, including in soil, domestic animals and human faeces, dust, and the gastrointestinal tract, where they are known to persist. Spores in the environment germinate to create bacteria when they get into damaged tissue.
How does tetanus infection occur?1
Tetanus doesn't spread from person to person. The disease exclusively manifests in individuals not immunized, typically following an infection of a skin wound. These wounds are caused by sharp objects that have come into contact with contaminated soil, such as nails, needles, thorns, chips, scissors, knives, or through scratches and animal bites.
Traumatic wounds typically serve as the origin of infection in many instances, with tetanus also being known to result from burn injuries, surgical abscesses, intravenous drug usage, circumcision, or gangrene.
All individuals, regardless of age, are vulnerable to infection. However, children and the elderly are at greater risk due to reduced immunity.

The incubation period typically lasts approximately seven days. Affected infants cannot suckle, which is followed by general rigidity, often resulting in opisthotonus. Mortality rates increase as infants develop complications such as cessation of breathing and severe infection.
The definition of non-neonatal tetanus by the World Health Organization requires at least one of the following indicators: a prolonged spasm of the facial muscles leading to a smile or painful muscular contractions. Although the definition includes a requirement for a history of injury or wound, tetanus can still occur in individuals who cannot remember a specific injury or wound.
- Localised Tetanus
This unusual type of tetanus shows up as muscle spasms near where the injury happened. A mild form of the illness can develop into the general form of the disease. The localized tetanus that arises from injury to the face or scalp has the potential to progress to cephalic tetanus, which can affect the musculature of the cranial nerves.

What is the appropriate course of treatment for patients diagnosed with tetanus?12
Early tetanus infection acquired through a wound has a local treatment, which includes thorough cleaning of the wound, removing foreign objects as needed, and applying an antiseptic solution.
The standard treatment involves the administration of ATPA toxoid, penicillin, anti-tetanus serum, or immunoglobulins. In addition to the specific treatment for the underlying cause, symptomatic treatment is provided with the goal of preventing muscle contractions. This may include high doses of barbiturates and benzodiazepines, particularly in cases of generalized tetanus infection.
Individuals who have recovered from tetanus do not possess innate immunity and remain susceptible to reinfection, necessitating immunization.
