Introduction

Endometriosis is a disease which affects roughly 10% of reproductive-aged women and girls globally. In endometriosis tissue that mimics the lining of the uterus sets up shop outside the uterus. This can be in other areas of the pelvis, the bowel and other surrounding areas, rarely it may be found in more distant regions such as the lungs or even the brain!1,2,3

Endometriosis can cause life-changing pain to those who are burdened with this disease. Affected individuals may experience chronic pelvic pain, painful periods, pain when urinating, painful bowel movements, nausea, vomiting and, in some cases, depression, anxiety and infertility.3

The impact of endometriosis on women's health is multifaceted, affecting various aspects of their well-being and quality of life. 

Coenzyme Q 10 (CoQ10), also known as ubiquinol, is a naturally occurring compound in the body that plays a crucial role in producing energy in cells. It is an antioxidant, so it helps to protect our cells from cell damage (oxidative stress).4 Why are we speaking about CoQ10? A recent study on Sprague-Dawley rats yielded some pretty interesting results, which we will explore in this article. 

Understanding endometriosis

Endometriosis is a word derived from the Greek “endo” meaning inside “metra” meaning uterus, and “osis” meaning disease. This disease remains elusive, with the most common clinical symptoms being pelvic pain and infertility. Endometriosis is what we call a “estrogen-dependent” disease, this means it is caused by a kind of “sensitivity” to estrogen. Another hallmark of endo is the misplaced endometrial-like tissue outside the uterus. This tissue is frequently found in the ovaries, resulting in “chocolate cysts”. Still, it may also be found in other areas of the reproductive system and less often in more distant areas.5

A study investigating the quality of life of patients with endo assessed the occurrence of depression in women with endometriosis. In this study the most common symptom reported was pelvic pain, there were also significant reports of painful periods and painful sex. Symptoms of pain were directly linked with the onset of depression.6 This highlights the significant impact on the quality of life endo has, both physically and mentally.

Current treatment options and their limitations

No cure exists for endometriosis medications such as NSAIDs (ibuprofen etc.) and analgesics address pain, while hormonal methods are employed to control symptoms, this may not be ideal for someone who wishes to become pregnant and due to this other methods are considered. Fertility medicines and procedures are options for those wishing to start a family. Laparoscopic surgery is the current gold standard for treatment, as it removes lesions and scar tissues.3

CoQ10 is a potent anti-inflammatory and anti-oxidant agent, and it also plays a crucial role in ATP and mitochondrial function. In previous studies, the antioxidant properties of this supplement have been demonstrated. It has also been shown to have a positive impact on ovulation rate and improve uterine tissue.6

One study hypothesised that CoQ10 may prevent the formation and progression of endometriosis. To test this they evaluated CoQ10 in different doses in rats with endometriosis to compare the effects of CoQ10 with the effects of a GnRH agonist (a known treatment for endometriosis).6

The scientific basis for CoQ10 in endometriosis

The study found that CoQ10 treatment in rat endo led to a reduction in endometriosis implant, improved adhesion scores, and positively impacted cellular and molecular factors associated with endo, including immune cells and collagen fibre density.6 These findings suggest CoQ10 may be a potential therapeutic option in endometriosis treatment.

Safety and considerations

CoQ10 is a naturally occurring component found in all living cells, it is a licensed dietary supplement in many countries and has been for over two decades. There has been a recent boom in the popularity and daily dosage of CoQ10 supplementation.7 

I know what you’re thinking: is it safe? The answer is yes! Safety data from both human and animal studies indicate low toxicity and no identified adverse effects associated with CoQ10 supplementation.7

The ADI (acceptable daily intake) is 12mg/kg/day and this is based on studies done on… you guessed it rats.7

Future directions and research gaps

Researchers should now delve into the specific effects of CoQ10 on pain associated with endo. Understanding this could provide valuable insights into clinical applications. The effect of CoQ10 on reproductive outcomes is also a crucial area of research. 

While studies in rats offer us valuable insight, translating these findings to humans is essential. This determines the efficacy, safety and optimal dosage in a real-world setting. The long-term effects of continued CoQ10 treatment should also be investigated. 

Continued research in this area will contribute to a more comprehensive understanding of CoQ10’s potential in managing endo, paving the way for improved therapeutic strategies and patient outcomes.
