Introduction

A migraine is a recurring primary headache with symptoms such as sensitivity to light, sound, or movement. Unlike secondary headaches, this type is not linked to another underlying condition. Typically, it involves moderate to severe head pain, often on one side, and may include aura — neurological symptoms like visual disturbances or tingling sensations.1

Its cause remains complex and poorly understood, involving various neurons and regions of the brain. Risk factors include advanced age, head trauma, socioeconomic status, caffeine or medication overuse, stress, sleep issues, obesity, pain syndromes, and pro-inflammatory or pro-thrombotic (blood clotting) conditions.2

This condition exerts a significant burden, impacting economic stability, family dynamics, work, and education. Prevalence peaks between 25 and 55 years, affecting approximately one in seven individuals globally, with a higher incidence among individuals assigned female at birth (AFAB) at 33% than individuals assigned male at birth (AFAM) at 13%.

There have been notable strides in understanding its underlying mechanisms and developing treatments. Lifestyle adjustments have gained importance in managing the condition. Incorporating regular exercise, addressing obesity and adopting a balanced diet is essential in reducing the frequency and intensity of migraine episodes. For example, cultivating healthier habits like sufficient sleep and abstaining from substance abuse. These factors should be carefully integrated into comprehensive care plans for individuals with migraines.3

In order to prevent migraines, it is crucial to identify triggers, avoid them, and steer clear of previously mentioned risk factors. Lifestyle changes are a vital, cost-effective intervention offering numerous benefits to patients' lives. To summarise these interventions, the acronym SEEDS has been proposed:4

  • Sleep 
  • Exercise 
  • Eat
  • Diary
  • Stress

Specific interventions such as physical activity, managing daily caloric intake, maintaining a daily diary, undergoing continuous positive airway pressure therapy, avoiding alcohol and tobacco, staying hydrated with non-caffeinated fluids, and practising mindfulness-based stress reduction can reduce migraine frequency.3

Dietary interventions for migraines

Diet plays a crucial role in overall health, including for migraine sufferers, although there is not a specific diet tailored solely for them. Various diets can alleviate migraine symptoms, including elimination diets, those rich in specific nutrients, and epigenetic diets.3

Elimination diets involve identifying and removing trigger foods, such as gluten, in coeliac disease individuals, which can significantly reduce headache frequency. However, other elimination diets like immunoglobulin G-elimination or low-fat diets may yield conflicting results and lead to malnutrition if not carefully managed. 

Diets high in omega-3 fatty acids and low in omega-6 fatty acids have shown effectiveness in reducing migraine duration and frequency. Specific diets, such as the ketogenic diet for overweight individuals or low-sodium diets for certain populations, may also be beneficial, targeting mechanisms like neuroprotective and mitochondrial function.

An epigenetic diet, like one rich in folate, may also have a positive impact. Maintaining regular meal times, particularly breakfast, is also advised for migraine sufferers. Skipping meals can increase the likelihood of migraines due to changes in blood sugar levels.

Specific dietary strategies for migraine management

Navigating dietary choices can be challenging for individuals managing migraines, as certain foods can trigger or exacerbate symptoms. Understanding which foods to include and which to avoid is crucial for effectively managing migraine attacks. Some foods contain compounds called amines, which, when consumed in large quantities, can trigger migraines.4 Other substances, such as tannins, nitrates, monosodium glutamate, and aspartame, can also trigger migraines. These include:

  • Alcohol
  • Cheese
  • Chocolate
  • Cola-based drinks
  • Coffee, tea, and other caffeine stimulants
  • Citrus fruits and high-amine fruits
  • Pork and pork products
  • Cured meats
  • Very cold foods

It is important to note that not everyone will experience migraine attacks triggered by all of these foods.5 Seeking specialised and individualised advice from an expert is crucial. Additionally, other factors that can help prevent migraines include:

  • Avoiding periods of fasting
  • Eating meals or snacks between main meals
  • Steering clear of fried or very fatty foods
  • Ensuring adequate hydration

Some guidelines offer valuable insights into migraine-friendly food options across various categories. For instance, it is recommended to opt for plain bread, such as white, wheat, or rye varieties from the store, while avoiding fresh-baked bread or those with added ingredients like nuts or cheese.

Similarly, selecting cereals without nuts, chocolate, or dried fruit can help minimise migraine risk. By making informed choices and avoiding known triggers, patients can proactively manage their migraine symptoms and improve their overall quality of life. 

Impact of diet modifications on migraine frequency and severity

Various factors, including genetic, epigenetic, cultural, and environmental influences, contribute to migraine triggers. Based on individual characteristics and biomarkers, personalised dietary recommendations are becoming increasingly crucial in migraine management, aligning with the trend towards personalised medicine. It is essential to strike a balance between trigger avoidance and coping strategies tailored to each patient's needs, taking into account factors like age, sex, and other types of headaches they may experience.6 

Sleep and migraine 

Discussion on the relationship between sleep and migraines

For centuries, doctors and researchers have known that there is a connection between migraines and sleep problems. However, the exact nature of this link and how it works are not fully clear. Recent studies using advanced techniques like biochemical analysis and brain imaging have pinpointed specific brain parts and chemical messengers that play roles in migraine and sleep regulation.

This suggests that disruptions in these shared pathways within the nervous system might contribute to both conditions. In simpler terms, problems with specific brain processes could be at the root of migraines and sleep problems.7

The relationship between migraines and sleep issues is quite complex. Having trouble sleeping can make migraines worse, and vice versa. While morning headaches are often linked to a condition called obstructive sleep apnea (OSA), it is not entirely clear if OSA directly causes migraines. However, OSA could make migraines worse for some people. It is a similar story with restless legs syndrome (RLS) – it's connected to migraines and can make them more frequent and severe.8 

Another condition that sometimes goes hand-in-hand with migraines is narcolepsy, which involves sudden sleepiness during the day. Researchers think there might be similar reasons behind both conditions. However, more research is needed to understand this connection better. Lastly, some evidence suggests that changes in sleep patterns, like those caused by shift work or jet lag, might affect migraines. 

Sleep hygiene practices for managing migraines

While the connection between migraines and sleep remains somewhat mysterious, it is clear that they are linked, making good sleep habits crucial.9 Practicing good sleep hygiene involves maintaining a consistent sleep schedule, which can be achieved through the following measures: 

  • Establishing a consistent sleep schedule, aiming to go to bed at the same time every day to regulate your body's internal clock
  • Paying attention to your body's needs and finding a bedtime that feels right for you, aiming for 6 to 8 hours of sleep per night
  • Spending time outdoors or in natural light to help synchronise your brain with your body's rhythms
  • Creating a serene sleep environment by keeping your bedroom dark, quiet, and comfortable, and minimise electronic devices, especially those emitting light
  • Exercising earlier in the day and avoiding strenuous activity right before bed, as it can promote better sleep
  • Avoid smoking, as nicotine disrupts melatonin production and can interfere with sleep
  • Reserve your bed for sleep and intimacy only, refraining from activities like watching TV or studying there, and if you can't sleep, get out of bed until you feel tired
  • Avoid caffeine and alcohol before bedtime, as they can harm sleep quality rather than improve it

Stress management and migraine 

Examination of the stress-migraine connection

Numerous studies indicate that a significant percentage of migraine patients attribute stress as a trigger for their headaches. It has been suggested that acute stress can lower an individual's resistance to migraine attacks by causing biological changes. Additionally, higher stress levels have been linked to increased migraine incidence, implying that stress may not only precipitate individual attacks but also contribute to the development of the condition in susceptible individuals.10

Chronic stress can further exacerbate migraines, possibly due to increased pain sensitivity. Studies also suggest a correlation between adverse childhood experiences, such as abuse, and migraines in adulthood, although a direct causal link has not been established. Furthermore, there's evidence of a connection between migraine and stress-related psychiatric disorders like post-traumatic stress disorder.

Relaxation techniques, biofeedback, and cognitive behavioural therapies focused on stress management have shown efficacy in preventing migraines, particularly in children. While these therapies are recommended alongside medication for frequent migraine sufferers, it is important to note that many studies on their effectiveness have methodological limitations. Despite this, stress management therapies are included for migraine management as a prophylactic (preventative) measure.

Stress management strategies for migraine prevention

Stress is widely recognised as a major trigger for migraines, creating a cycle where stress can induce migraines, which in turn can lead to more stress, perpetuating the cycle. Managing daily stress levels is crucial for preventing migraines.11,12 Here are some recommendations to help you manage stress effectively:

  • Practice relaxation techniques like meditation, yoga, tai chi, or deep breathing
  • Simplify your schedule to reduce unnecessary stressors
  • Engage in regular physical activity to help relieve tension
  • Maintain a healthy diet with nutritious foods
  • Ensure you get enough sleep each night
  • Organise your day and prioritise tasks with a to-do list, focusing on one task at a time
  • Remain flexible in case plans change, reducing pressure and anxiety
  • Take breaks throughout the day, practice positive self-talk, and find opportunities to laugh
  • Address unhealthy habits like smoking and excessive alcohol consumption, as they can exacerbate stress levels and trigger migraines

By implementing these strategies, you can effectively manage stress and reduce the likelihood of migraine attacks.

Additionally, stress management is essential, as stress is a significant trigger for migraines. Managing lifestyle factors such as stress, sleep, and diet is essential for effective migraine prevention and management.
