Introduction

Kidney infections, also known as pyelonephritis, are serious and potentially life-threatening conditions that occur when bacteria travel from the bladder to the kidneys. These infections can lead to significant discomfort and, if untreated, can cause permanent kidney damage or systemic infections such as sepsis. The NHS defines kidney infection as “a painful and unpleasant illness usually caused by cystitis, a common infection of the bladder”. Kidney infections come across various symptoms, including feverish, back pain, the urge to pee more often than usual, blood in pee, a burning sensation while peeing, and smelly or cloudy pee.

Kidney infections can cause damage to the main bloodstream if not treated on time, hence proper management and effective treatment of the infection is extremely important to maintain well-being. It may also cause permanent kidney damage or may even infect the bloodstream causing sepsis or blood infection which can be fatal.  Early diagnosis can help prevent the infection from reaching later stages and increasing complications.

Medical treatment

In case of kidney infections, the doctor prescribes an antibiotic treatment to fight the most common type of bacteria. Before prescribing an antibiotic treatment to the patients, they first ask for the blood or urine test results and depending upon the results, they’ll prescribe the correct antibiotic. These antibiotics have also been given to patients based on other factors such as:

  1. The type of bacteria that has caused the infection
  2. The severity of the infection
  3. Pregnancy
  4. Olderthan 65
  5. Allergic reactions to certain antibiotics
  6. Local antibiotic resistance pattern

Antibiotic treatment

Medical treatment options for acute kidney infections are antibiotics, analgesics and antipyretics. Infections caused by bacteria such as E.coli, patients can be treated with oral cephalosporins or sulfamethoxazole-trimethoprim for 14 days or a fluoroquinolone for 7 days. Fluoroquinolones are the best choice of oral antibiotics if the bacteria is sensitive and the community resistance is low (10% or less). Sulfamethoxazole-trimethoprim for 14 days or cephalexin for 7-10 days can be used in some cases, but rising bacterial resistance makes them less reliable for initial treatment.

Ceftriaxone is a good option for the initial intravenous treatment of kidney infections. Carbapenems should be saved for very serious cases or when no other drugs work.

A single dose of gentamicin can be added to other treatments because it reaches high levels in kidney tissue, doesn't harm kidney function much, and helps improve outcomes.

Pain management

Analgesics should not be used if the kidneys have a decreased function as having these painkillers may reduce the blood flow to the kidneys. A higher dose of these painkillers may harm the kidneys thus they should be used carefully. Kidney infections may increase if non-steroidal anti-inflammatories (NSAIDs) such as ibuprofen are prescribed to relieve pain from kidney infections. Paracetamol may help to relieve the pain and high temperature caused by the infection. Analgesics should be used under three conditions for kidney infections; should be consumed in the exact amount as prescribed on the label, consumed for a short period, and at the lowest dose possible.

Hospitalization

Hospitalization in patients is referred to when the patients are having a problem with the urinary tract which makes the patient vulnerable to kidney infection. Hospitalisation is also needed in cases like:

  1. Severe dehydration
  2. Difficulty in swallowing or taking any medications
  3. Blood poisoning
  4. Having high temperature in pregnancy
  5. Diabetes
  6. Over the age of 65

Patients admitted to the hospital generally will be attached to a drip so that the doctors can give fluids through the IV to keep them hydrated. Antibiotics can also be given through the drip. To check the effect of the antibiotic treatment, patients have to undergo blood and urine tests to monitor how effective the antibiotics are in fighting off the infection. If there is no risk of complications, the patients are discharged from the hospital within three to seven days. Antibiotic treatment will resume back to tablets after the patient has stopped taking antibiotics through a drip.

Preventative measures

Preventing kidney infections is crucial to maintaining overall health and well-being. Preventative measures focus on reducing the risk factors associated with urinary tract infections (UTIs) and promoting habits that keep the urinary system healthy. By adopting specific lifestyle practices and hygiene habits, individuals can significantly lower their chances of developing kidney infections. Understanding and implementing these preventative strategies can help safeguard kidney function and enhance quality of life.

Kidney infections can be prevented by:

  1. Drinking plenty of water
  2. Urinating on time
  3. Urinating after having sex
  4. For women, wipe from front to back after using the bathroom to prevent bacteria from spreading from the vagina or anus to the urethra
  5. Avoid using deodorant sprays or douches in the genital area

Summary

Kidney infections are serious conditions that require prompt and effective treatment to prevent complications, such as permanent kidney damage or sepsis. Medical treatment primarily involves the use of antibiotics tailored to the specific bacteria causing the infection and the patient’s individual circumstances. Pain management and, in severe cases, hospitalisation play crucial roles in the comprehensive care of kidney infections. Preventive measures, including proper hygiene and avoiding irritants in the genital area, are essential to reduce the risk of infection. Early diagnosis and adherence to prescribed treatments are key to ensuring a full recovery and maintaining kidney health. By understanding the treatment options and preventive strategies, individuals can better manage and prevent kidney infections, leading to improved overall health and well-being.
