Overview
Sports-related injuries such as muscular contusions or extrinsic muscle lesions are frequent; in fact, they are the second leading cause of morbidity among athletes. Participating in contact sports like soccer, basketball, rugby, and football, is a primary risk factor for muscle contusion.1 Thigh contusion, also known as ‘quadriceps contusion’, ‘Cork thigh’, ‘Dead leg’ or ‘Charley horse’, is a common musculoskeletal injury that happens when a direct hit to the anterior thigh damages the muscle fibres and small blood vessels, causing pain, swelling, and discolouration.2
The severity of the injury, determined by the degree to which you can bend your knees without pain, is used to grade thigh contusions. This method is essential for managing treatment and evaluating the likely recovery process. The grades include;2
- Grade I (Mild): This is defined by the ability to bend your knee more than 120°. People with mild thigh contusions usually feel only mild discomfort; however, they might also see some swelling or bruises. Although this injury is mild, it must be properly managed to avoid complications
- Grade II (Moderate): For this grade, the range of bending your knees is 90° to 120°. This involves moderate pain and swelling, with a clear decrease in the range of motion. It is also characterised by pronounced bruising and requires an extended period of rest and recovery
- Grade III (Severe): This is the most severe grade where you cannot bend your knee above 90°. This is usually followed by severe pain, widespread bruising, and swelling. Recognising Signs and Symptoms
Usually, when you feel a focused pain that worsens with passive bending of your knee and have a history of anterior thigh injuries or an abrupt discomfort following a forceful unusual contraction of the quadriceps, you most likely have a thigh or quadriceps contusion. You may be able to resume your activities based on the grade of the thigh contusion, that is, depending on if it is a mild thigh contusion or a severe thigh contusion. The signs and symptoms of thigh contusions are;3
- Bruising and Discoloration: The injury area may look red, purple, or blue at first, changing to yellow-green as it heals

The use of over-the-counter pain relievers and topical treatments is also recommended in the first phase of the injury.
Over-the-Counter Pain Relievers
Painkillers such as NSAIDs (Non-Steroidal Anti-Inflammatory Drugs) and corticosteroids can be administered during the first week of the injury. These drugs reduce pain and swelling of the injury but should not be used excessively as they may slow muscle healing if used long-term. According to a study, their use in mild thigh contusions has been stopped.5
Topical Treatments
Topical gels and creams containing menthol, arnica, or anti-inflammatories can provide local pain relief to the injured area.5

For quadriceps stretch, stand on one leg, grab your ankle, pull it towards your glutes, and hold for 15 to 30 seconds. For Hamstring stretch, sit with one leg straight, reach towards your toes, and hold for 20 seconds.6
Strengthening Exercises
To strengthen your thighs after a contusion, start with isometric quad contractions by sitting with your leg extended, tensing the thigh muscle for five seconds, and repeating 10 times. Follow this with straight leg raises, lie on your back, lift your injured leg a few inches off the floor, and then lower it slowly. Finally, do mini squats by standing with your feet hip-width apart, lowering yourself slightly, and returning to standing, aiming for two sets of 10 reps.6
Gradual Return to Normal Activities
Start with light walking or cycling if it is pain-free, then progress to bodyweight lunges and step-ups to regain strength. Add jogging and controlled running drills before attempting high-impact movements.6
