What is chronic hypertension causing cardiomegaly?

  • High blood pressure that is unstable in the long term. causing chronic hypertension, which leads to heart enlargement, also called cardiomegaly. Over time, it places excessive strain on the heart, forcing it to work harder to circulate blood throughout the body. This prolonged stress can lead to cardiomegaly
  • Heart failure diseases are more at risk at an older age; people above 60, especially males, tend to have more cases
  • Cardiomyopathy, weakening, stiffening, and thickening of heart muscles, is one such condition due to the enlargement of the heart, causes impairs the ability of the heart to pump blood effectively

What causes an enlarged heart?

  • Anaemia: a reduced oxygen-carrying capacity of the blood, making the heart pump harder and fail to deliver the required blood supply to the body, potentially leading to hypertrophy
  • Myocarditis: viral infection leads to myocarditis, which weakens and eventually causes the enlargement of the heart muscles
  • Pleural effusion: fluid builds up around the lungs, can put pressure on the heart, indirectly contributing to cardiomegaly
  • Amyloidosis: this rare condition involves the accumulation of amyloid protein builds up in multiple organs and the blood,  interfering with normal cardiac function
  • Genetics: congenital heart defects due to alterations in genes can cause cardiovascular disease, and can be inherited from birth or develop later in life
  • Haemochromatosis: intake of higher amounts of iron can damage the heart muscle and cause swelling of the lower chamber of the heart

What are the early signs of heart enlargement?

  • Fainting: it is also known as syncope, loss of consciousness, irregular heartbeat excursion due to stressful work or heavy lifting exercise or excessive running
  • Chest pain: tingling sensation or pain that arises from the back to both the arms, and may be mistaken for muscular pain or indigestion
  • Swelling (Oedema): fluid retention in the legs, stomach, neck, feet, and bloating of the stomach can be an early sign of poor heart function
  • Fatigue: persistent, extreme tiredness throughout  the day, even after rest, often results from inadequate blood and oxygen delivery to tissues
  • Dyspnea: it causes shortness of breath, particularly during physical activity  or when lying flat
  • Palpitation: a sensation of the heart beating faster than the regular, after running at speed without a break
  • Heart arrhythmia: irregular heartbeat due to disruption of cardiac electrical impulses 
  • Diastolic dysfunction: abnormal relaxation of the heart muscle detected during the ECG test, fluctuation in the diastolic pattern

How to reduce risk and complications through preventive measures?

Medication

  • ACE (Angiotensin converting enzyme) inhibitors dilate blood vessels and reduce heart strain
  • Beta-blockers and calcium channel blockers are used to manage blood pressure and heart rhythm
  • Diuretics to reduce fluid overload
  • Oral potassium supplements if hypokalaemia is present

Medical interventions

  • Stent placement via angioplasty to open narrowed arteries
  • Pacemaker or defibrillator implantation in certain cases of arrhythmia

Lifestyle modifications

  • Regular, supervised cardiovascular exercise
  • A heart-healthy diet low in salt, sugar, and saturated fat
  • Avoidance of smoking and excessive alcohol consumption
  • Routine monitoring of blood pressure and glucose levels
  • Regular health check-ups, including ECGs, echocardiograms, and CT scans

Autoimmune disease is linked with cardiovascular disease

  • The immune system mistakenly attacks its healthy cells is an autoimmune condition
  • The heart is the most impacted organ in such conditions, and the effect is irreversible, with damage to the arteries and veins or malfunctioning of the cardiac muscles
  • Conditions like lupus or rheumatoid arthritis may lead to chronic inflammation of the blood vessels and heart tissue, potentially causing irreversible damage such as myocarditis, pericarditis, or heart valve disease

Summary

Chronic hypertension, or long-term high blood pressure, can lead to cardiomegaly—an abnormal enlargement of the heart caused by the thickening or weakening of the heart muscle. Preventive measures such as medication, lifestyle changes, regular exercise, and routine blood pressure and heart function monitoring can significantly reduce the risk of complications. Pregnancy and autoimmune diseases can also increase cardiovascular risks, highlighting the need for specialised care in such populations. Early recognition, diagnosis, and management are essential to improving outcomes and preventing life-threatening events such as sudden cardiac arrest.
