Introduction

These wounds, by definition, involve the deliberate penetration of the skin, a protective barrier for the body. The purpose of these incisions ranges from diagnostic explorations to therapeutic interventions, with the overarching goal of addressing and rectifying different medical conditions.

The proper care of surgical wounds is paramount to a patient's overall recovery and well-being. The meticulous management of these wounds is not solely focused on aesthetic considerations, but extends to preventing complications that may arise during the healing process. Complications such as infections, dehiscence (the separation of wound edges) or delayed healing can significantly impact the postoperative recovery period, potentially leading to prolonged hospital stays and increased healthcare costs.

As the healing process progresses, the proliferative phase takes action, marked by tissue repair and regeneration. New blood vessels form, facilitating the growth of granulation tissue, and fibroblasts contribute to the synthesis of collagen which is the structural framework of tissues. This phase lays the foundation for the subsequent maturation phase, where collagen remodelling occurs, gradually strengthening the wound and leading to the development of a scar.

Understanding the intricacies of surgical wounds goes beyond their immediate creation in the operating room. Patient-specific factors, such as age, nutritional status and the presence of chronic illnesses, play a pivotal role in influencing the rate and success of wound healing. Additionally, surgical procedure-related factors, including the sterility of the environment and the precision of the surgical technique, contribute significantly to the overall outcome.1,2

Types of surgical wounds

Incision wounds

Incision wounds are cuts made purposefully during surgeries like caesarean sections or appendectomies. These intentional openings enable surgeons to access specific areas for diagnosis or treatment. The controlled nature of incisions allows for a more predictable healing process compared to other types of wounds.3

Abrasion wounds

Abrasion wounds happen when the skin is scraped or rubbed against rough surfaces, frequently occurring in orthopaedic surgeries or trauma cases. These wounds result from friction and can vary in severity. Proper care is essential to prevent infections and promote healing in these cases, considering the exposure of underlying tissues.4

Puncture wounds

Puncture wounds occur when sharp objects penetrate the skin, often seen in procedures involving needles or trocars (a sharp instrument used to extract fluids). The narrow, deep nature of these wounds can increase the risk of infections and complications, emphasising the need for careful postoperative monitoring and preventive measures.

Laceration wounds

Laceration wounds involve torn or jagged edges and are commonly observed in traumatic injuries or emergency surgeries. The irregular nature of these wounds presents challenges in the healing process, requiring specialised care to minimise scarring and promote optimal recovery. Close attention to wound management is crucial for successful outcomes in cases of lacerated wounds.

Inflammatory phase

The inflammatory phase begins immediately after surgery, involving blood clotting and immune cell responses to prevent infection.7

Proliferative phase

During the proliferative phase, the wound is filled with new tissue, blood vessels and collagen, promoting the rebuilding of damaged structures.6

Maturation/remodelling phase

Collagen remodelling and scar formation occur during the maturation phase, gradually restoring the wound's strength.5

Patient factors such as age, nutritional status and chronic illnesses significantly impact wound healing.1

Signs of infection, including redness and swelling, should be closely monitored and treated promptly with antibiotics.1

Dehiscence

Dehiscence, the separation of wound edges, may occur due to poor surgical technique or excessive strain on the wound site.

Hematoma

Hematoma formation can result from bleeding into the wound, requiring swift intervention to prevent complications.

Wound care guidelines

Preoperative care

Before surgery, ensuring proper skin preparation and following antiseptic measures are crucial steps. This preparation significantly decreases the likelihood of postoperative infections. By meticulously cleaning and sterilising the surgical site, healthcare providers create a safer environment for the procedure, minimising the risk of complications during the recovery phase.

Postoperative care

After surgery, consistent and careful attention is vital. Regular dressing changes help maintain a clean and protective environment for the healing wound. Additionally, vigilant monitoring for any signs of complications, such as infections or unusual swelling, is essential. This proactive approach in postoperative care plays a pivotal role in fostering optimal wound healing and preventing potential issues from escalating.
