Introduction 

Transverse fractures are a specific type of bone injury occurring in the world of orthopaedics, which can have a significant impact on an individual's life. Anatomy of bones

Bones are more than rigid structures, they are living tissues that make up the body’s skeleton providing shape and support to the body. They are the storage houses of minerals and proteins that provide strength, flexibility and material for growth. Bones are categorised according to their shapes; long, short, flat and irregular. 

Fractures 

Even though the bones are rigid living tissues, they do bend if an external force is applied. But if the external force is too much, it can crack or break the bone just as a plastic ruler breaks when it is bent too much. This breaking of the bone is known as a fracture. A fracture’s severity can be determined by the applied external force. If the applied force is slight then it can cause only a crack in the bone but if the external force is too much, it can shatter the bone.

Types of fractures

Fractures have a variety of names depending on their type.

  • Greenstick (partially broken bone causes the other side to bend)
  • Transverse (a straight line break across the bone)
  • Spiral (the break spirals around the bone)
  • Oblique (a diagonal break across the bone)
  • Compression (the broken bone appears wider or flat)
  • Comminuted (the bone is broken in three or more places)
  • Segmental (the bone is broken in two places)

Transverse fractures

Transverse fracture is a type of complete fracture that occurs when your bone is broken perpendicular to the axis of the bone. This means the fracture is across the length of your bone. This is the most common type of fracture that can happen to any bone of the body but is more common in long bones. The long bones which are usually affected are:

  • Femur (thigh)
  • Tibia (shin)
  • Fibula (calf)
  • Humerus (upper arm)
  • Radius and ulna (forearm)
  • Clavicle (collarbone)

Causes of transverse fracture

This type of fracture can happen to anyone due to a high-energy collision or impact e.g. car crash, falling from heights, gunshot wounds etc. In sports, athletes can have a transverse fracture when they experience sudden, forceful impact or twist their limbs in an unnatural way. Children who have weak bones due to incomplete calcification, and adults who have weak bones due to osteoporosis, are at great risk of having a transverse fracture and can have this type of fracture with only minor stress. 

Symptoms or clinical presentation of transverse fracture

Transverse fractures, like other fractures, present with characteristic symptoms and signs that doctors use for diagnosis. Common clinical presentations of the transverse fracture include:

  • Pain
  • Swelling
  • Tenderness
  • Inability to move a part of your body normally
  • Bruising or discoloration
  • A deformity or bump that’s not usually on your body

Diagnostic tools

Transverse fracture diagnosing is very important for determining the proper treatment approach. Physicians and orthopaedic specialists rely on physical exam and imaging techniques to confirm the presence of a fracture and assess its extent. These include:

  • X-rays: These are often the first choice for initial diagnosing, showing 2D images of the bone. Transverse fracture appears as a clear horizontal line on the x-ray film
  • CT scan: This provides a more detailed view about the bone tissue and bone structure than standard X-rays of the bone, thus providing more information related to injuries or fracture of the bone
  • MRI: This provides 3D detailed anatomical images. This test is done when nearby soft tissue injuries or damage are suspected in addition to fracture

These diagnostic tests play an important role in determining the exact location, size, and orientation of the transverse fracture, in turn guiding the treatment plan.

Treatment

Transverse fracture treatment depends upon the severity of damage to the broken bone. Your physician may undertake the following treatments:

Immobilisation

If you have a slight fracture and the bone is not displaced then your physician may immobilise the affected limb through casting or splinting. Pain management, physiotherapy, x-rays and regular follow-up appointments are essential components of this approach.

Closed reduction

If your bone has severe breaks, then it requires realignment by your physician. This non-surgical procedure of pushing and pulling your body from outside by your physician to realign the broken bones is known as closed reduction. For this treatment, your physician may use the following to prevent you from feeling any pain:

Surgical treatment

In cases where you have a displaced fracture or the bone is significantly unstable, surgical intervention may be necessary. One common surgical procedure used is open reduction and internal fixation. During this technique, your surgeon may insert pieces of metal to realign your bone. This prevents your fractured bone from healing abnormally. 

The following are used to stabilise the bone during the healing process:

  • Rods: inserted through the centre of your bone, these run from top to bottom
  • Plates and screws: to hold the broken pieces of bone together in place
  • Pins and wires: these hold pieces of bone in place that are too small for other fasteners. They are typically used at the same time as either rods or plates

The choice between non-surgical and surgical procedures is done based on careful evaluation of the fracture and overall health of the patients. Your orthopaedic surgeon considers factors such as age, activity level and bone quality when deciding the treatment plan.

Complications and risks

Treatment aims to promote healing and recovery but there are some complications and risks associated with transverse fracture surgery. These complications may include:

  • Malunion: bone heals in an abnormal way
  • Nonunion: bone does not heal properly or does not grow back properly
  • Infections: after the open surgery when metal pieces are placed inside the bone, you are at great risk of bacterial infections

Regular monitoring and timely intervention can reduce these risks and improve the outcomes.

Rehabilitation and recovery

Rehabilitation plays an important role in the recovery process of physiotherapy is often prescribed to improve mobility, strength, and function of the affected limb. Rehabilitation is a time-consuming process and can take several months depending upon the severity of the fracture, the type of the that was fractured. It's essential for you to follow their rehabilitation plans to achieve the best possible recovery outcomes.

Following the general tips can help you to reduce your risk of being affected by transverse fracture.

  • Maintaining proper bone health through proper nutrition and exercise
  • Using protective gear in high risk activities, like wearing helmets, fastening seat belts
